4 Geriatric Care Management Training Session 3 The Facts About Advance Directives Test With Answers 1. TRUE or FALSE. A Power of Attorney becomes effective only when a physician determines the client to be incapacitated. 2. A Living Will must state: a.) The client s beneficiary b.) What specifically the client wants withheld or withdrawn c.) The client s current medication list 3. TRUE or FALSE. All individuals, regardless of age and medical status, should have a DNR (do not resuscitate order). 4. A Health Care Surrogate has the right to: a.) Review medical records b.) Provide Informed Consent c.) Apply for Public Benefits d.) All of the Above 5. TRUE or FALSE. A Power of Attorney is no longer valid once the client dies. 6. TRUE or FALSE. A person can have only one Power of Attorney. 7. TRUE or FALSE. Guardianship is the least restrictive intervention a person can experience. 8. TRUE or FALSE. Guardianship is never necessary if a person has executed a Power of Attorney. 9. A Power of Attorney is valid until: 3

7 Facts About Advanced Directives Resource Advisor Liz Barlowe, MA, CMC CEO of Innovate Today As Chief Executive Officer of Innovate Today, Liz Barlowe brings more than 25 years of experience in the aging and disability field. For the past ten years she was Chief Operating Officer for Aging Wisely, LLC, a professional care management and consultation company serving the greater Tampa Bay area of Florida. She was the founding administrator and is a shareholder of EasyLiving, a private duty homecare company. Liz has received the Fellow designation from the National Association of Geriatric Care Managers. She is the immediate past president of the Association s Florida Chapter and is a current Board member. Liz has a master s degree in Gerontology and a bachelor s degree in Social Work. NEVCO video educational programs are prepared using specific criteria designed by National Educational Video, Inc. All educational programs are coordinated and reviewed under the direction of the NEVCO Director of Education, who is a master s prepared nurse. 6
