2 Pre-Employment Medical Ability Test (M.A.T.) This Orientation and Preparation guide is provided to each potential candidate to encourage you to prepare for the test. We hope that this guide will assist you with the proper preparation needed to successfully complete this test, and we wish each candidate the best of luck on the test. This packet contains: Copy of Medical Ability Test Criteria Requirements for Medical Ability Test: Successful completion of the P.A.T. (Physical Ability Test) General Preparation Our hiring process is very competitive and the pre-employment Medical Ability Test is one of the initial hurdles in the selection process. Please take the time to thoroughly prepare in order to ensure your success.

3 Pre-Employment Medical Ability Test (M.A.T.) Orientation and Preparation Guide The Monroe County Fire Academy Medical Ability Test is based upon the National Registry Emergency Medical Technicians curriculum. The NREMT certifies EMS Professionals. Currently this includes First Responders, EMT-Basics, EMT-Intermediates who have completed either an EMT-Intermediate/85 course of training or an EMT-Intermediate/99 course of training and EMT-Paramedics. The NREMT strives to achieve high levels of validity with all standards and processes. The NREMT follows a peer review psychometric development process to establish the cut score for all examinations. The performance examination (practical examination) is developed by national committees of experts, peer reviewed, pilot tested and adopted by the NREMT Standards and Examination Committee and the Board of Directors. All other NREMT policies and procedures are reviewed by the multi-disciplinary Board of Directors. Competent practice is an important process of the NREMT mission. The NREMT sets its competency levels at the entrylevel via committees of providers, regulators and medical directors. NREMT examinations are tied directly to practice via data obtained from the NREMT practice analysis. Items on all NREMT examinations are directly related to tasks within the practice analysis and areas of emphasis on the examination are balanced to data received from practicing EMTs on the important tasks required to deliver the knowledge and skills of the occupation.

4 The Medical Ability Test is designed to simulate medical activities. The following outline will familiarize the candidate with what will take place on the day of the test. Candidates are encouraged to wear comfortable clothing, athletic shoes, long pants and a shirt. You will be performing various types of Medical activities therefore you should not wear clothing that will hinder your performance. Candidates can be expected to get dirty during the performance of the skills listed. Monroe County Fire Academy will provide nitrile gloves for each candidate to use while completing the test. All components of the Medical Ability Test are not timed however, Monroe County Fire Academy Staff reserves the right to terminate an evaluation, if the evaluation has extended beyond a reasonable amount of time required to perform the skill successfully. During each component of your test, there will be an evaluator assigned to you. You may ask the evaluator questions regarding the test prior to beginning and while performing the skills; however the evaluator will not provide you with instruction or information regarding the accuracy of your performance. The events that compose the EMT-B and EMT-P portion of the Medical Ability Test are outlined for you on the following pages. Each event has a description of the skill required.

5 EMT-Basic Components Required Skills 1 Patient Assessment/Management- Trauma: You will be required to perform a "hands-on'', head-to-toe, physical assessment and voice treatment of a simulated patient for a given scenario. The skills include: Scene Size-up, Initial Assessment, Focused History and Physical Examination/Rapid Trauma Assessment, and Detailed Physical Examination. successfully perform patient assessment and provide treatment based on information received verbally, physically, and visually 2 Patient Assessment/Management- Medical: Given a scenario, you will be required to evaluate and treat a medical patient including Scene Size-up, Initial Assessment, Focused History and Physical Examination/Rapid Assessment and Ongoing Assessment. successfully perform patient assessment and provide treatment based on information received verbally, physically, and visually 3 Cardiac Arrest Management/AED: Given a scenario, you will be required to evaluate and treat a cardiac arrest patient. Your evaluation and treatment will include Assessment, Transition, Integration, and Transportation. successfully perform patient assessment and provide treatment based on information received verbally, physically, and visually

6 4 5 Spinal Immobilization (Seated or Supine): Given a patient you will be required to maintain c-spine control, apply appropriate size extrication collar, assess motor, sensory, and circulatory function. You will direct the movement of the patient without compromising the integrity of the spine. Properly secure a spinal immobilization device and reassess patient. assess, stabilize package a patient suspected of having a spinal injury Bag-Valve-Mask Apneic Patient with a Pulse: You must assess the airway, insert appropriate adjunct, and properly ventilate a patient using a bag-valve-mask. You must transfer ventilation to a second EMT prior to completion. assess, secure and administer artificial respirations Random Skills One of the following skills will be randomly selected: 6 Immobilization Skills- Long Bone Injury Immobilization Skills- Joint Injury Immobilization Skills- Traction Splinting Bleeding Control/Shock Management Upper Airway Adjuncts and Suction Mouth-to-Mask with Supplemental Oxygen Supplemental Oxygen Administration

7 EMT-Paramedic Components Required Skills 1 2 Patient Assessment- Trauma: You will be required to perform a "hands on", head-to-toe, physical assessment and voice treatment of a simulated patient for a given scenario. The skills include: Scene Size-up, Initial Assessment/Resuscitation, Focused History, Physical Examination, Rapid Trauma Assessment, and Detailed Physical Exam. successfully perform patient assessment and provide treatment based on information received verbally, physically, and visually Ventilatory Management: Given a scenario of having just found an apneic patient with a palpable carotid pulse, you must demonstrate immediate management of the patient using simple airway maneuvers, adjuncts, bag-valve-mask device, and supplemental oxygen. You will then complete the presentation by placing an endotracheal tube. This component is designed to assess your ability to recognize airway compromise and to successfully establish a patent airway 3 Cardiac Management: Dynamic Cardiology- You will be evaluated on your ability to manage a cardiac arrest situation, including actual delivery of electrical therapy and "voicing" all interpretations and treatments given a scenario. Static Cardiology- Given prepared ECG tracings with associated patient information, you must verbalize the interpretation of each rhythm and voice all associated treatments. This component is designed to assess your ability to recognize cardiac compromise and to successfully interpret and treat patient condition

8 4 Oral Station: You will be evaluated on your ability to verbally manage all aspects of an out-of-hospital call given two separate cases. In each of the cases you will be evaluated on the following categories: Scene Management, Patient Assessment, Patient Management, Interpersonal Relations, and Integration (verbal report, field impression, and transport decision) This component is designed to assess your ability to evaluate, categorize and implement patient care and to verbally communicate your actions and decisions 5 IV and Medication: Intravenous Therapy-You will be required to establish a patent IV in a manikin arm in accordance with a given scenario. Intravenous Bolus Medications- After establishing a patent IV line, you will be required to administer an IV bolus of a medication in accordance with a given scenario. This component is designed to assess your ability to establish an IV line and to evaluate your ability to administer the proper medication 6 Pediatric: Pediatric Ventilatory Management- Given a scenario of just having found an apneic infant with a palpable brachial pulse, you must demonstrate immediate management of the patient using simple airway maneuvers, adjuncts, bag-valve-device, and supplemental oxygen. You will then be required to place an endotracheal tube. Pediatric Intraosseous Infusion- You will be required to establish an intraosseous line in a pediatric IO manikin. This component is designed to assess your ability to properly manage a pediatric patient
