This fee includes one hour of arborist review. If additional time for review is required, the applicant will be charged at an hourly rate of $85. **A City of Lake Forest Park business license is required to operate in Lake Forest Park. A contractor that is licensed by Lake Forest Park is required to be licensed by the state, bonded and insured. Any contractor conducting earthmoving or tree work in the City is required to review and sign a Statement of Tree Canopy Preservation and Enhancement Acknowledgment. Release/Hold Harmless Agreement A permit issued pursuant to this application signifies only that the proposed project complies with City ordinances and regulations, and the applicant is advised that the City has not performed a risk assessment with respect to the project. In consideration of the issuance of the permit, the applicant, on behalf of him/herself and heirs and assigns, shall release, indemnify, and defend and hold the City of Lake Forest Park harmless from any and all damages and/or claims for damages, including reasonable attorneys fees, arising from or in any way related to the issuing of a permit pursuant to this application. Endangered Species Act, U.S.C , et. seq., may apply to this project, and that the applicant is responsible for complying with such other laws, the applicant shall release the City of Lake Forest Park, indemnify and defend it from any claim, damages, injuries, or judgments, including reasonable attorney's fees, arising from or related to violations of such other laws or regulations. The applicant further understands that the applicant s contractor must procure and maintain for the duration of the project, insurance against claims or injuries to persons or damages to property which may arise from or in connection with the project authorized by the permit. The City of Lake Forest Park is not responsible to verify whether contractors not included on this permit application are licensed, insured and bonded. By signing below you are certifying that the trees mentioned in this application to be removed are to the best of your knowledge on the property of the owner listed on this application. Before making application you should confirm that the limits of the property have been correctly identified. Affadavit of Posting Agreement I agree to post the subject property as described in Lake Forest Park Municipal Code section A notice must be posted a minimum of two days before removal and shall remain posted at least one week after the approved activity has been completed. When removal is taking place an approved tree permit must also be posted onsite. Failure to correctly post the property will result in the cancelation of an issued permit and enforcement under Lake Forest Park Municipal Code section

4 ARBORIST REVIEW PERMIT ASSESSMENT, REMOVAL, AND REPLACEMENT PLAN CHECKLIST For applicants who have hired an arborist 1. A tree inventory and assessment report to include: Individual trees to be removed, protected, or that are in the vicinity of construction, in a chart or table with the corresponding numbers on site plan, and the following information for each tree: o Species o Diameter at breast height (DBH) o Condition (health) o Minimum required critical root zone (CRZ) o Canopy coverage square footage, accounting for expected growth of young trees*** o Indication of whether the tree is to be removed or protected Canopy coverage calculations that clearly show: o Lot size (use a professional survey or use figures from King County records) o Existing canopy coverage o Canopy coverage goal in square feet o Canopy area to be removed o Amount of replacement canopy needed to bring lot to canopy coverage goal, if applicable**** o Projected canopy coverage to be provided by new trees, if applicable A description of tree protection measures for forest stands or clusters of or single significant trees to be protected, and any off-site trees that may be impacted by tree removal, excavation, or grading during the project Replacement trees to be planted, in a chart or table with corresponding letters on site plan, if applicable, and the following information for each tree: o species o specific height or caliper o expected canopy coverage square footage, at maturity o or alternatively, the amount to be paid into the city tree account in lieu of replacement trees Signature of qualified tree professional 4. A timeline for tree protection activity to include: A pre-job conference with the city s qualified tree professional to discuss tree protection activity. Installation, inspection and maintenance of the tree protection fences. Measures taken to ensure health of the tree(s) during construction. Removal of fences at end of construction. Final inspection of protected trees by city s qualified tree professional. 5. New development projects must include: A maintenance bond of 125 percent of the projected cost of tree replacement. This bond must be maintained for 60 months after new trees are planted.

5 *Scaled site plans. If you do not have software that allows you to create a to-scale site plan, you may use King County imap. This website provides property information and printable graphics that can help you assemble your site plans. **Tree Protection Measures. 1. To ensure long-term viability of trees identified for protection: a. All required tree protection measures shall be shown on the demolition, grading, and tree protection plan along with a timeline for tree protection activity. b. All construction activities, including staging and traffic areas, shall be prohibited within five feet of the dripline of protected trees. c. Tree protective fencing shall be installed at the limits of disturbance and completely around trees to be protected prior to any land disturbance unless otherwise delineated by the qualified tree professional. d. Tree protective fencing shall be a minimum of four feet high, constructed of chain link or polyethylene laminar safety fencing or similar material, subject to approval by the city. The fence must be constructed on steel posts with a minimum spacing of eight feet on center. Tree Protection Area Keep Out signs shall be posted visibly on all sides of the fenced areas. e. Where tree protection areas are remote from areas of land disturbance, and where approved by the city, alternative forms of tree protection may be used in lieu of tree protective fencing; provided, that the critical root zones of protected trees or stands of trees are clearly delineated with continuous rope or flagging and accompanied by Tree Protection Area Keep Out signs. 2. Tree Health. In addition, the applicant shall support tree protection efforts by employing, as appropriate, the following preventative measures, consistent with best management practices (BMPs) for maintaining the health of the tree: a. Pruning of visible deadwood on trees to be protected; b. Application of fertilizer to enhance the vigor of stressed trees; c. Use of soil amendments and soil aeration in tree protection and/or replanting areas; d. Mulching over tree dripline areas; and e. Providing watering during and immediately after construction. 3. Alternative Methods. The city may approve the use of alternative tree protection techniques if a protected tree will be protected to an equal or greater degree than through the techniques listed above, as determined by the city s qualified tree professional. ***Measuring Canopy Coverage. 1. Existing Open-Grown Trees. a. Measure the radius of the tree s canopy at its widest and narrowest points and calculate the average canopy radius for the tree. b. Calculate the tree s canopy coverage (ft 2 ) using the average canopy radius and the formula: r 2 where = and r = the canopy radius in feet. c. Add up the calculated canopy coverage for all trees on the parcel in square feet and divide by the total lot area to determine the percentage canopy cover. 2. For contiguous stands of trees, traverse the perimeter of the stand within the parcel to calculate the area under the tree canopy and the resulting canopy percentage. In some cases, use of high-resolution aerial photography may be used for larger stands. 3. For newly planted or immature trees less than 30 years old, calculate the projected canopy coverage at age 30 using the information provided in the approved general tree list for the city of Lake Forest Park. Sum these canopy coverage areas to project the lot canopy coverage as trees reach an age of 30 years.

6 ****Tree Replacement Requirements. 1. If the arborist finds that the plans for tree removal will cause the lot s canopy coverage to fall below the goal percentage, a tree replacement plan that brings canopy coverage to the applicable percentage specified in the table below will be required. (2) Mandatory replacement plan including one replacement tree for each significant tree removed or equivalent payment into tree fund. 2. Any substitutions of trees that are not on the list or alternative methods of replacement must be approved by the city s qualified tree professional. 3. The minimum size of replacement trees should be: a. Deciduous trees, two inches caliper. b. Conifers, a minimum of six feet tall. 4. All trees must be containerized or balled and burlap stock and be planted according to industry standards. 5. All trees shall meet the minimum standards for size and quality according to the current edition of the ANSI Z60.1 standard for nursery stock. 6. The applicant is required to provide watering until replacement trees are independently viable. Replacement trees that die prior to meeting the definition of significant tree must be replaced by a replacement tree meeting the criteria of this section and any canopy coverage goal requirements that the expired tree was intended to provide. 7. Replacement trees shall be planted on the site from which significant trees are removed. The administrator may waive the on-site tree replacement requirement; provided, that the applicant pays an amount determined by a qualified tree professional into the city tree account that is equivalent to the total cost for purchase and installation of each replacement tree and three years of maintenance for each replacement tree.
