CHE Plus Benefits is designed exclusively for colleagues of CHE and CHE affiliated facilities. During your two week Annual Enrollment, you may select the plans in this guide to complement and supplement the core benefits package you receive as a colleague of CHE. The information in this enrollment guide and our Benefit Counselors will show you how CHE Plus Benefits work with your core benefits to provide choices, options and convenience for you and your family. 3 Choices and Options to Help Protect You and Your Family SmartSavings EMPLOYEE DISCOUNT MARKETPLACE Group Cancer/Critical Illness Insurance Plan Cancer/Critical Illness provides benefits to help with the cost of cancer treatment as well as a lump sum benefit upon the diagnosis of not only one covered illness, but for each covered illness as well. Conditions covered are heart attack, stroke, cancer, renal failure, coma, paralysis, burns, loss of sight, loss of hearing, loss of speech and Occupational HIV. Individual Short Term Disability Income Protection Short term disability income protection insurance can provide benefits if you become disabled and cannot work due to a covered illness or accident. Coverage of up to 50% of your salary may be offered 1, subject to salary requirements and existing coverage, and your policy is guaranteed renewable to age 72 as long as premiums are paid. In NY, this product is referred to as LPSD short term disability insurance. Legal Benefit Every twelve months, the average law abiding citizen will have the need for the services of an attorney according to studies by the American Bar Association. The LegalEase Group attorney network makes hundreds of free and low cost legal services available to members. Accident Insurance Accidents can impact the rest of a person s life. And if not prepared, there s often little protection for a family s financial security. This policy is designed to help provide benefit payments against the high cost of covered accidents. Not available in CT. Additional Benefits Available: CHE Plus Benefits has also put together several benefits that will be available to all eligible employees after this Open Enrollment period. Meaning, you can enroll in these benefits outside of this scheduled enrollment. These benefits are: SmartSavings Discount Marketplace SmartSavings is a members only discount marketplace that provides you with access to hundreds of brand name retailers and local merchants - all from one website. This benefit will continue to be available throughout the year. Long Term Care Long-Term Care Insurance (LTCI) covers a range of personal care services you might need if you cannot care for yourself due to a temporary or permanent illness or injury. Home care and nursing home expenses can be very expensive, and the cost is not generally covered by either medical insurance or Medicare. This benefit will continue to be available throughout the year. Home and Auto Insurance Compare special group auto and home insurance rates and quality coverage from two of the nation s most trusted insurance carriers Travelers and MetLife Auto & Home. Request free, no obligation quotes and apply throughout the year. Pet Insurance Pet Insurance is offered by VPI and is an affordable pet health coverage that helps you pay the treatment costs for your pet s accidents, illnesses and routine medical care. This benefit will also be available for enrollment throughout the year. 1 In CA and NJ the benefit amount is limited to 40% due to state provided disability coverage 3

4 Plan Features*: Cancer Expense Benefit - Benefits are payable if the insured incurs eligible medical expenses for internal or invasive cancer that is initially diagnosed while coverage is in force. For the treatment of skin cancer (must be initially diagnosed while coverage is in force), we will pay 10% of the actual expenses incurred for eligible medical expenses. Calendar year maximum is $5,000. Please refer to the certificate for a list of covered cancer expense benefits. First Occurrence Benefit - Lump Sum Benefits payable upon initial diagnosis of a covered critical illness. Employee benefit amounts available from $5,000 to $50,000. Spouses may purchase up to 1/2 of employee benefit amount. Additional Occurrence Benefit - If an insured collects full benefits for a Critical Illness under the plan and later has one of the remaining covered critical illnesses, then we will pay the full benefit amount for each additional illness. Occurrences must be separated by at least 6 months. Re-Occurrence Benefit - If you collect full benefits for a covered critical illness and are later diagnosed with the same condition, we will pay the full benefit again. The two dates of diagnosis must be separated by at least 12 months or for cancer 12 months treatment free. Child Coverage at no Additional Cost - Each dependent child is covered at 25 percent of the primary insured amount at no additional charge. Spouse Coverage Available - You may purchase group cancer/critical illness coverage for your spouse in amounts up to one half the employee amount to a maximum benefit of $25,000. If you are deemed ineligible due to a previous medical condition, you still retain the ability to purchase coverage for your spouse. Add-a-Buck provides guaranteed insurability by providing benefit increase bases on $1 per week or $52 per year. Rates Guaranteed - cannot be individually increased due to change in age, health or individual claim. Portability - The plan is portable, take your coverage with you if you leave your job (with certain stipulations) *NOTE: Benefit is 25% of face amounts for Carcinoma in situ and Coronary Artery Bypass Surgery. Payment of these benefits will reduce the cancer and/or the heart attack benefits by 25%. All covered conditions are subject to the definitions found in your certificate. 4 Covered Critical Illness Group Cancer/Critical Illness Insurance Plan New for 2010! We will pay this benefit regardless of the results of the test. There is no limit to the number of years the insured can receive the health screening benefit; it will be paid as long as the certificate remains inforce. This benefit is not paid for dependent children. Examples of covered screening tests: CA 125 (blood test for ovarian cancer) Stress test (On a bicycle or treadmill) Colonoscopy Hemocult stool analysis PSA (blood test for prostate cancer) CA 15-3 (blood test for breast cancer) Breast Ultrasound Bone marrow testing Fasting blood glucose test Chest X-Ray Flexible sigmoidoscopy CEA (blood test for colon cancer) Serum protein electrophesis Thermography Triglycerides/Serum (blood test for myeloma) cholesterol test to determine levels of HDL and LDL *Refer to page 13 for additional information

5 Individual Short Term Disability Insurance (LPSD short term disability insurance in NY) Individual Short-Term Disability Insurance coverage not available to colleagues of St. Francis Medical Center (NJ), St. Mary Medical Center (PA), St. Francis Hospital (DE), St. Mary s Healthcare Systems (GA), and certain colleagues of St. Agnes Continuing Care Center who are covered under another plan. If you were out of work due to an illness or injury, how would you cover the costs of daily living or the expenses associated with a disability? Unum voluntary individual short term disability insurance can help replace a portion of your salary if you are unable to work due to a covered injury or illness. This means you can still have some income during a time of need. It s estimated that an accident or illness will keep three out of every 10 employees between the ages of 35 and 65 out of work for a period of three months or longer. 1 The five leading causes for Unum STD claims lasting less than six months include: normal pregnancy; injuries (not including back); digestive/intestinal diseases; pregnancy (complications); and reproductive/urinary system diseases. The advantages of our short term disability insurance Affordable coverage Voluntary individual short term disability insurance is available through your employer. Your premiums are based on your age when you buy the insurance and will not increase because you get older. Eligibility Available for eligible employees ages (64 in CA) who are actively at work. 2 Coverage levels You may be able to apply for a benefit percentage of up to 50% 3 of your gross monthly salary, from a minimum of $400 up to a maximum of $5,000, subject to salary requirement. Policy features You will select an elimination period (the period of time following a disability before the benefits are paid) and a benefit period (maximum length of time disability benefits could be paid). Convenient payroll deduction If you apply and qualify for coverage, your premiums are automatically deducted from your paycheck. Coverage effective date Your coverage becomes effective on the first day of the month in which payroll deductions begin. Individually owned policy If you leave your current company or retire, you can continue your coverage and pay the same premium. You ll be billed directly at home. 7 Accident Insurance Unum s supplemental accident insurance provides benefits for covered injuries 1 and specified accident-related expenses for an individual or family. Since health insurance only goes so far, this individual policy is designed to help cover the out-of-pocket expenses that can result from a sudden accident. Available to employees ages (64 in CA) who are actively at work 2. Base plan covers a wide variety of injuries and accident-related expenses such as hospitalization, physical therapy, emergency room treatment, fractures and dislocations, transportation, lodging and more. Benefits are paid for accidents that occur on and off the job Family coverage options are available. Coverage for the base plan benefit is guaranteed renewable for life as long as premiums are paid. Premiums are paid through convenient payroll deduction. The policy is individually owned so you can take your coverage with you if you leave your employer. 10 Long-Term Care Long-Term Care Insurance (LTCI) covers a range of personal care services you might need if you cannot care for yourself due to a temporary or permanent illness or injury. 12 VPI Pet Insurance Voluntary Pet Insurance (VPI) is the affordable health coverage that helps you pay the treatment costs for your pet s accidents, illnesses and routine care. Your VPI coverage pays for your pet s diagnostic tests, treatments, office visits, lab fees, prescriptions, hospitalizations, x-rays and surgical procedures. These pages contain more detail about the CHE Plus Benefits Group Cancer/Critical Illness Insurance Plan and Short-Term Disability Income Protection Insurance plans. For even more information, go to: CAIC LIMITATIONS AND EXCLUSIONS CANCER (Expense Benefits) AND CRITICAL ILLNESS This Plan provides Benefits only for the treatment of Internal Cancer and/or Skin Cancer and lump sum benefits for Critical Illnesses, as defined in The Group Master Policy CAI2800PA. This Plan does not provide benefits for any other disease, sickness or incapacity. No benefits will be paid for expenses incurred outside the United States or its Territories. Diagnosis must be made in the United States. This Plan contains a 30-day Waiting Period. This means no benefits are payable for any Insured who has been diagnosed before their coverage has been in force 30 days from his Effective Date. If an Insured is first diagnosed during the Waiting Period, benefits for treatment of that Critical Illness will apply only to loss commencing after 12 months from his Effective Date; or, at the Employee s option, he may elect to void the Certificate from the beginning and receive a full refund of premium. CANCER (Expense Benefits) No benefits will be paid for any Cancer treatments that have not been approved by a Physician as being medically necessary. CRITICAL ILLNESS The applicable benefit amount will be paid if: the date of diagnosis is after the waiting period; the date of diagnosis occurs while the policy and certificate are in force; and the cause of the illness is not excluded by name or specific description. PRE-EXISTING CONDITIONS LIMITATION (Critical Illness) Pre-existing Condition means a sickness or physical condition which, within the 90-day period prior to the Effective Date of an Insured s coverage resulted in medical advice or treatment. We will not pay benefits for any critical illness starting within 12 months of the Effective Date of an Insured which is caused by, contributed to, or resulting from a Pre-existing Condition. A claim for benefits for loss starting after 12 months from the Effective Date of an Insured will not be reduced or denied on the grounds that it is caused by a Pre-existing Condition. A Critical Illness will no longer be considered Pre-existing at the end of 12 consecutive months starting and ending after the effective Date of an Insured s coverage. 14 Short Term Disability Income Policy Details from Unum. Policy provisions: Waiver of premium Payment of premiums can be waived after 90 days (or after the elimination period is satisfied if longer than a 90 day elimination period) of total disability, as defined in the policy. Guaranteed renewable The policy is guaranteed renewable until the policy anniversary on or following the insured individual s 72nd birthday, as long as the required premiums are paid when due. Pre-existing provision Benefits for a disability due to a pre-existing condition will not be paid if that disability begins within 12 months of your coverage effective date. A pre-existing condition is defined in your policy as a condition for which symptoms existed that would cause you to seek treatment from a physician or for which you were treated, received medical advice from a physician or took medicine, within 12 months before your coverage effective date. Pregnancy is considered as any other covered sickness nine months after coverage becomes effective. The available monthly benefits will be paid upon fulfillment of the elimination period. Benefits will not be paid if the insured individual gives birth within nine months after the coverage becomes effective. However, medical complications of pregnancy may be considered as any other sickness, subject to pre-existing condition limitations.
