4 Get the coverage you need. Interest-sensitive whole life insurance is offered to all eligible employees ages 15 to 80 who are actively at work. You decide if it s right for you or your family. Four features that add value 1. Accumulates cash value Guaranteed at a rate of 4%. You can borrow from the cash value or use it to buy a reduced policy with no more premiums due. 2. Living Benefit Option Rider Included on all policies. You can request an advance, up to 100% of the benefit amount (maximum of $150,000), if you re diagnosed with a terminal illness limiting life expectancy to 12 months or less Adds more coverage that s affordable If your employer gives you term life insurance, you have some coverage, but it may not be enough for your needs. You can buy interestsensitive whole life insurance at an affordable premium that is fixed and guaranteed for life. 4. Requires no physical exam During enrollment, you can get this insurance up to a specified amount without taking a health exam. You may be asked a few health questions. Additional purchase option Accidental Death Benefit Rider Available at initial enrollment to employees and spouses ages 15 to 65, this rider pays an additional death benefit equal to the base policy amount (subject to a $150,000 maximum) if the policy holder dies before age 70 as the result of a covered accident. Available family coverage Who can have it? Spouse policy Ages 15 to 80 Child term rider With purchase of employee or spouse policy, available to eligible children, stepchildren, legally adopted children and grandchildren (14 days to age 25*) of the primary insured adult. What s the benefit amount? Minimum policy amount of $2,000. Actual benefit amount based on coverage amount chosen and age at issue. $1,000 to $10,000 one rider covers all children. How long can they keep it? Even if you leave your employer, you can keep your spouse s policy and be billed directly at home. Rider ends when your policy ends or when children turn 25. At that time, children are guaranteed the right to buy an individual whole life policy at 5 times the amount of their rider. Child policy No employee or spouse purchase needed. Same eligibility and issue ages as child rider. Policies are individual. Benefit amounts based on age at issue and premium selected. Your children can keep it, even if you leave your employer. Benefits that pay for covered accidents while you are on the road to recovery Unum s coverage provides a lump sum benefit based on the type of injury (or covered incident) you sustain or the type of treatment you need. Accident insurance is offered to all eligible employees ages 17 to 80* who are actively at work. You decide if it s right for you and your family. Five reasons to buy this coverage at work 1. Your base coverage is guaranteed renewable for life. 5. Premiums are conveniently deducted from your paycheck. Additional coverage option Hospital Confinement for Sickness Rider If offered by your employer, you may choose this coverage for an additional premium. This rider pays the insured employee, spouse or child(ren) a daily benefit if he or she is in the hospital for a covered illness. Amount is $100 per day for employee and spouse; $75 for children. The eligible age for employee and spouse is 17 to 67* and 14 days through 24 years for children. This rider is available to family members who are covered by the base plan. The rider includes a 12-month pre-existing condition limitation and a nine-month pregnancy exclusion. Employees and their spouses need to answer certain health questions when applying for this rider. 21 Voluntary specified critical illness insurance Can your wallet survive a serious illness? The answer can be yes with specified critical illness insurance from Unum. * * * * Life can change in a heartbeat. Heart problems run in Ann s family. But she has two active boys to keep up with. Every 26 seconds an American will have a coronary event, such as a heart attack or stroke. 1 Many patients pay as much as $5,000 or more per month out of pocket for cancer-treatment drugs even for generics. 2 Use your critical illness benefit any way you choose. Critical illness insurance pays you a lump sum benefi t at the fi rst diagnosis 3 of a covered illness.* It can be used however you choose for the expenses health insurance doesn t cover. Illnesses covered by the base plan include: heart attack stroke major organ transplant permanent paralysis due to a covered accident end-stage renal (kidney) failure coronary artery bypass surgery (pays 25% of lump sum benefi t) Illnesses covered by the enhanced plan include all of the above, plus: cancer carcinoma in situ 4 (pays 25% of lump sum benefi t) Please refer to the policy for complete definitions of covered critical illnesses. 22 Get the coverage you need. Critical illness insurance is offered to all eligible employees age 16 to 69 (64 in California) who are actively at work. Choose the benefi t amount that s right for you from $5,000 to $50,000, in $1,000 increments. Three reasons to buy this coverage at work 1. You get affordable rates when you buy this policy through your employer, and the premiums are conveniently deducted from your paycheck. 2. You own the policy so you can keep it even if you leave the company or retire. Unum will bill you directly for the same premium amount. 3. Coverage becomes effective on the fi rst day of the month in which payroll deductions begin. Employer-selected benefit option Your employer may automatically include this option as part of your benefi ts, for an additional premium. Health Screening Benefit Rider This benefi t pays $50 per calendar year per insured individual if a covered health-screening test is performed, including blood tests, chest X-rays, stress tests, mammograms 5 and colonoscopies. Eligibility begins 30 days after the coverage effective date. A full list of more than 20 covered tests will be provided at your enrollment. Employee-elected benefit option Your employer may make this option available to you to purchase, for an additional premium. Enhanced (two payout) plan This plan includes all covered conditions previously listed and can provide an additional lump sum benefi t following diagnosis of cancer and/or carcinoma in situ. Policy provisions Reduction of benefits The benefit amount for the employee and spouse reduces by 50% on the first policy anniversary after the insured individual s 70th birthday, or five years after the policy effective date, whichever is later. Premiums for the policy will not be reduced. If partial benefits for coronary artery bypass surgery or carcinoma in situ have been paid prior to the reduction of benefits, then the new benefit amount will be calculated by applying the 50% to the benefit amount reduced by the prior payout. The additional Cancer and Carcinoma In Situ Benefit No benefits will be paid for cancer or carcinoma in situ if the date of diagnosis occurs during the first 30 days from the coverage effective date.
