We provide health care services through 29 facilities, including 12 community-based health centers, nine homeless medical sites, two specialized health centers, two health services sites in DC jails, three community correctional health sites and one mobile health unit. 4 Unity Health Care the largest nonprofit provider of health care in the nation s capital has been caring for the District s medically underserved residents for 25 years. Unity is supported by the U.S. Department of Health and Human Services Health Resources and Service Administration and private contributions, corporate giving and grants, nominal patient fees for those able to make payment, private insurance, and other contracts and grants from local and federal governments. As a Federally Qualified Health Center, Unity receives cost-based reimbursement from Medicare, business-related cost savings, access to highly trained professionals and drug discounts. All of these benefits are passed on to our patients. ~ 2

5 Unity provides primary care, preventative care and continuing treatment. W hen you first visit a Unity Health Care center, you will meet your primary care provider. You can see that same doctor or clinician every time you visit that clinic. And he or she will refer you to medical specialists and other support services such as a family practice physician, OB/GYN, nutritionist or social worker as needed. At Unity, you will receive primary and specialty medical care of the highest quality. Every person who walks through our doors receives such care. More important, we play a central role in our patients lives. Our health care providers spend time with their patients, listening to them, responding to them and helping them improve their lives. We treat the whole patient, getting to know each individual and his or her unique situation. ~ 3

Standard Hours of Operation Monday Friday 8:15 a.m. 4:45 p.m. Some sites have extended hours, including evening and Saturday hours. For information about extended hours, please contact the health center closest to you. After-Hours Care When a Unity patient has a medical emergency outside of regular hours, he or she can reach a Unity doctor by calling Payment and Insurance Unity provides services regardless of ability to pay, and many of our patients pay nothing at all for care. We use a sliding scale based on family size and income to determine each patient s share of costs. U Unity accepts DC Healthcare Alliance*, Chartered Health/Alliance, Unison Alliance, Medicare, Medicaid, Amerigroup and some private insurance plans. *DC Healthcare Alliance is a public-private partnership that acts as a health insurance provider for District of Columbia residents who do not have health insurance and have income at or below a certain percentage of the poverty level as predetermined by the DC government. DC residents who qualify for Medicare will receive prescription benefits only if they are over the age of 64, have income at or below the federal poverty level, and are not enrolled in any private health insurance plan that coordinates with Medicare or Medicaid. 7 Unity Health Care Services nity addresses the physical and emotional well-being of adults, adolescents and children. We think about immediate care and long-term needs. And we provide primary care, preventative care, ongoing treatment and specialty medical services. 8 Unique Services In addition to the services provided at all of our facilities and mobile units, Unity maintains some unique programs, such as: Congress Heights Health Center, named a Center of Excellence in 2003, focuses on decreasing the incidence of illnesses related to heart disease, asthma and diabetes, as well as decreasing infant mortality rates. Using a team approach and including the patient on the team responsible for his or her care the Congress Heights center provides health care, sets self-management goals, and teaches patients habits and skills that lead to better overall health. Unity s mobile unit is a medical outreach unit for homeless people. It is fully equipped to provide medical services, medications and other treatment and is staffed by a team of physicians, nurse practitioners and HIV/AIDS social workers. The team does not wait for patients to come to the van. They seek out patients by walking through neighborhoods and visiting gathering places of potential patients. Over time, they convince patients to come to the mobile units for care, and when additional treatment is needed, they refer patients to Unity s health centers. The Re-entry Health Center (RHC) provides medical care and case management services to DC residents recently released from federal and local prisons. The RHC was funded by The Robert Wood Johnson Foundation, The Morris and Gwendolyn Cafritz Foundation, The William S. Abell Foundation, Consumer Health Foundation and the Meyer Foundation. RHC provides mentoring services, job training, clothing, identification, housing and employment referrals. ~ 6
