2 Our service values The Twilight District Nursing Service in Milton Keynes gives support, advice and skilled quality nursing care to patients and their carers when a patient s care is best provided at home. We believe in treating everyone as an individual with care, confidentiality, privacy, compassion, respect and dignity. How are you referred to the service? We provide nursing care to adults who are temporarily or permanently housebound. We have an open system of referral. This means that you can refer yourself or, with your consent, somebody else can refer you, for example, a GP, practice nurse, family member, carer or the hospital. Once we have received your referral we will see if our service best fits your needs. If we do not feel we are the best service to meet your needs, we will advise you of a more appropriate service.
3 What can you expect from the Twilight Nursing Service? We work closely with the District Nursing Service. District nurses will provide your nursing care during the day between 9.00am and 5.00pm. However, there will be times when you will need a second visit in the evening, for example, if you have a blocked catheter. We visit patients between 8.15pm and 1.00am. We also visit patients who have been discharged from hospital in the evening. We try to see the majority of patients before 11.15pm but there are times when we may have to visit later, for example, if the time you contact us for a visit is later in the evening. We are unable to give you a specific time of when we will visit due to the changing needs of the patients we see. With your consent a member of the Twilight Nursing Team will visit you at home. A healthcare assistant will accompany the nurse during your visit. If you need assistance between 12.30am am, please call 111. In an emergency, call 999.
4 Why do we work with other services? By working closely with other community services, professionals and voluntary organisations, we will be able to support you fully at home. Some of the services we work with include: Community matrons Social services Continence nurse specialists Mental health services Age UK Willen Hospice Tissue viability nurses We will only ever involve other professionals and share information about you with your consent. If you are admitted to hospital while under our care, we may inform the hospital of the care you were receiving. This will make sure you can be discharged safely. Please let the nurse know if you would prefer us not to inform the hospital about your care. Visiting you at home As visitors to your home we will we will treat you as an individual and listen to your needs. We will be respectful, honest, open, trustworthy and polite. We will always show you our ID badge and if we forget to do so, it is ok to ask us to show it.
5 You can help us by: Notifying us if your circumstances change and you are able to attend a clinic or you GP practice. Providing us with clean hand-washing facilities. Not smoking whilst the nurses are visiting. Putting family pets away whilst the nurses are visiting. Notifying us if any of the equipment we have provided is faulty. Returning your notes to the district nurses once your treatment is complete and you have been discharged. Notifying us in advance, if for any reason you won t be at home for your planned visit. Please respect us; we re here to help you Our staff have the right to work safely, securely and free from abuse at all times. We reserve the right to use audio recording equipment to protect staff when necessary.
