3 NIH Nursing and responding to flatter world challenges Nurses role at NIH Clinical research nursing is nursing practice with a specialty focus on the care of research participants. In addition to providing and coordinating clinical care, clinical research nurses have a central role in assuring participant safety, ongoing maintenance of informed consent, integrity of protocol implementation, accuracy of data collection, data recording and follow up. Examples of roles include protocol coordinator, primary clinical research nurse, nurse manager, nurse scientist and research nurse coordinators. 3

4 NIH Nurse practitioners and responding to flatter world challenges Evolving Nurse practitioner role at NIH In 1996, there were approximately 7 nurse practitioners (NPs) at NIH. NPs coordinate all necessary testing and referrals for comprehensive workup and management of the medical problem seen in the clinical research trial. In other settings, the NP may fill in for a temporary need and/or provide services that the principal investigator may routinely do in assessing patients. Many serve as dual roles as associate investigators and as clinical research coordinators responsible for the data enrollment and implementation of the study. 4

5 II. Innovative clinical nursing strategies within NIH Expansion of clinical research nursing services. In 2010, Nursing and Patient Care Services completed a four-year initiative to define the specialty and scope of clinical research nursing. Clinical research nursing model of care was developed. This step was done to formalize their specialty & provide framework for responsibilities related to deliver of clinical care in research setting. 5

Innovative training and workforce development strategies within NIH NIH hospital offers training and education at opportunities offered from high school to college and masters programs, and beyond. Introduction to principles and practice of clinical research Bioethics and clinical research training Establishment of workforce development programs. NIH Internship program since Of the 45 interns graduating, 28 have been hired at NIH. US Clinical research nursing residency program established in NIH partnerships with academia to increase awareness and open doors for future NIH employment. NIH-Duke Master s clinical research program 6

Innovative scientific strategies within NIH NIH repurpose of drugs for new applications. 1 st drug for HIV AIDS (AZT) was not developed for HIV/AIDS but rather cancer Effort now underway to partnership to expand new applications for drugs - this is one of the goals of newly established NIH National Center for Advancing Translational Sciences. Partnerships with international government to address and improve health care outcomes. Fogarty International Center focuses on advancing science through global health initiatives and training international scientists. 7

8 II. Innovations involving patient care and safety at NIH Admissions redesign to improve the patient waiting time on admissions. NIH recognized as a stage 6 institute by the Health Care information and management systems society. Examples- Advanced electronic medical records system and medication barcode system enhancing patient safety. Research match program within the Office of Patient recruitment through creative research strategies Program matches patients who want to participant with scientists who need volunteers. Patient portal system 6,500 patients are actively using the portal launched in 2013 that allow patients access to medical records and NIH resources. 8

9 Innovations in response to US government and NIH mission critical efforts Multi-institute agency and nursing department responsiveness to emerging infectious disease and disasters NIAID Ebola Vaccine trial Multiple roles nurses play to optimize care of patient in NIH special clinical studies unit while meeting the goals of NIH research The NIAID Wat San program ensures individuals providing direct patient care, put on, and take off, the personal protective equipment (PPE) correctly providing 24/7 coverage of the unit to ensure health and safety. 9

10 Summary of health care challenges and NIH agency and nursing response Expanding knowledge and skills for the nurse and other health care professionals Drawing on existing resources within NIH in response to mission critical efforts. Programs allow for a sustainable workforce, collaborative practices, innovation, and increasing responsibility. New partnerships to address research initiatives and improve health care outcomes. 10
