3 Foundations of Public Health Program Description 3 Foundations of Public Health is a free online certificate program that provides learners with basic knowledge and principles related to Public Health. The certificate program addresses Tier 1 competencies in each of the 8 domains of the Council on Linkages Core Competencies for Public Health Professionals. The certificate includes 11 self-paced, online courses and takes approximately hours to complete. Individual courses provide continuing education credit for nurses, physicians and certified health education specialists. There is no time limit to complete the certificate, but participants are strongly encouraged to complete all online coursework 6 months post-registration. Foundations of Public Health is sponsored by the Empire State Public Health Training Center (ESPHTC), a partnership between two accredited schools of public health: the University at Albany School of Public Health (UA-SPH) and the University at Buffalo School of Public Health and Health Professions (UB-SPHHP). The mission of ESPHTC is to improve the public health infrastructure in New York State by strengthening the current and future public health workforce, particularly within medically underserved areas such as rural communities and American Indian Reservations. 6 Foundations of Public Health Program Requirements 6 Requirements: The Foundations of Public Health Certificate Program is targeted towards professionals new to the field of public health, professionals currently working in public health with limited or no formal education in the field, and professionals who want a basic understanding or refresher on the foundations and principles of public health. However, anyone is able to complete the program, regardless of profession or geographic location. Enrolling: Candidates should enroll through the New York State Department of Health Learning Management System website. Please see complete REGISTRATION DIRECTIONS located here in the Student Guidebook on pages Helpful Hint: Use the same username and password for each system. You may use the space below to keep track of your username(s) and password(s). New York State Department of Health Learning Management System Username: Password: Empire State Public Health Training Center courses (ex. ESPHTC-070) Username: Password: The New York State Department of Health Learning Management System (LMS) is a registration system used to group many courses together. The courses are from various educational institutions and are not housed on the LMS. You will need to go into the courses systems and create a username and password.
7 Foundations of Public Health Tracking & Application 7 Tracking your progress: Participants may want to track their progress to ensure they meet the requirements for the Foundations of Public Health Certificate. Page 8 includes a tracking sheet that may be used for this purpose. At the completion of your courses NYLearnsPH.com LMS Administrators will be made aware of your status and your course record will be updated within two weeks of completion in the LMS. Applying for your Foundations of Public Health Certificate: Once you have completed all requirements, you may apply for your certificate. Foundations of Public Health is entirely paperless, which means that once you complete your course requirements, you simply need to complete the online application, which can be found at The application will ask for basic information, including what you would like your certificate sent to, and will be checked by our staff for accuracy. NOTE You will need to enroll in ESPHTC-FPH: Foundations of Public Health Application. You only need to Enroll in this course and do not need to do anything else. 9 Foundations of Public Health Registration - Learning Management System 9 Step 1: Register on LMS To register for the Public Health Nurse Ready certificate program, please go to the New York State Department of Health s Learning Management System: Click on Click Here on the left upper hand side. (below Login) Step 2: Create a Profile You will be asked to create a profile. Please complete all required fields and include your agency name in the first line of your work address, in addition to the Agency drop down menu. (You DO NOT need to take any assessments. Those are optional and are not a part of this certificate.) Once you have completed your profile, please click on Submit your Registration. If you are already registered on the LMS, you DO NOT need to create a new profile. You should now be logged into the NYS Department of Health Learning Management System.

Foundations of Public Health Certificate Program Step 4: Please read the information on the page and scroll down to the bottom of the page and click Begin Program Enrollment.
12 Foundations of Public Health Registration - Course Requirements 12 Step 5: You will see a Course Status Table. Scroll down to enroll for each required course. Click on Enroll. Step 6: After clicking Enroll, you must select your credit type. **PLEASE NOTE** The LMS does not provide credits. The courses and their educational institutions provide the credits.
13 Foundations of Public Health Registration - Course Requirements 13 Step 7: Continue to enroll in each of the requirement courses. Once you have enrolled for all the courses, you will see a Program Enrollment Summary page. Click Launch Course to begin any of the requirement coursework.
14 Foundations of Public Health Registration - Course Requirements 14 Step 8: Once you are on the PHTC website simply login or complete the one-time registration and you will be launched directly into the course you requested. Whenever you return to this, or any other ESPHTC course the system will remember you and ask you to login using the Username and Password you selected during the PHTC registration. Once you login you will be launched directly into the course you requested. NOTE At the completion of your courses NYLearnsPH.com LMS Administrators will be made aware of your status and your course record will be updated within two weeks of completion. Anytime you enroll in and launch a new ESPHTC course from you will be recognized and will only need to enter your PHTC password to access a course.

16 Orientation to Public Health 16 Description: Orientation to Public Health is a web-based course that provides learners with a basic understanding of the mission and functions of public health. Designed to orient newly hired support and technical staff to the field, the course consists of two parts: Part One introduces the mission and six obligations of public health. Part Two explains the ten essential services. The course uses the metaphor of a new worker orientation (computer-based) to present information. By reading s, completing assigned tasks, and participating in simulated desktop conferences, the learner discovers that public health is very much a part of everyday life, and learns how many different agencies work to carry out its mission. Newly registered learners login to find a virtual "desktop" they will use to navigate through the course. Sections called "Inbox," "Documents," "Tasks," and "Conferencing" provide access to content. Course components are generated dynamically in response to the learner's progress. Advocates for public health programs and resources. Identifies how public and private organizations operate within a community. Describes the role of government in the delivery of community health services. Utilizes appropriate methods for interacting sensitively, effectively, and professionally with persons from diverse cultural, socioeconomic, educational, racial, ethnic and professional backgrounds, and persons of all ages and lifestyle preferences. 17 Field Epidemiology 17 Description: Disasters can strike anyone, anytime, and anywhere. Community volunteers, hospital workers, and public health staff members may be called on to help with a community response to a disaster. In order to respond effectively in such an emergency, these workers must have plans in place to be sure their families, other dependents, pets, etc. are cared for. This course provides a step-by-step process for developing a family disaster plan and a personal emergency plan. This course is intended for anyone who might be called to respond to an emergency that might keep them away from home for an indeterminate period of time, including public health or healthcare professionals and volunteers, such as members of the Medical Reserve Corps (MRC) or Community Emergency Response Teams (CERT). 18 Microbiology: An Introduction 18 Description: Microbes contaminate our air and water. They cause infectious and communicable diseases. Much of the work in public health involves identifying these microbes and finding ways to prevent their adverse effect on society. "Microbiology: An Introduction" provides a very brief introduction to microbiology. After taking this course, the learner will have a general understanding of cell biology and a better understanding of the five microbial groups that are of concern to the public health practitioner: bacteria, fungi, protozoa, helminths (worms), and viruses. The course may be used as either a refresher for people with backgrounds in microbiology or as a primer for public health professionals with little background in the subject. This course is a prerequisite for "Microbiology and Communicable Disease," a module of the Basic Environmental Health Course (BEHC) required for sanitarians working in New York State. Course Objectives Credit Hours Time Requirement Describe the basic characteristics of these live microbial groups: bacteria, fungi, protozoa, helminthes and viruses. 19 Program Development & Evaluation 19 Description: "Program Development & Evaluation is the first in a series of trainings dealing with the development and evaluation of public health programs. This training will walk you through the basic steps of program development, including identifying the problem, planning, implementing and evaluating the program. The latter part of the training explores both formative and summative evaluation, and why these components are so critical to the process. Course Objectives Credit Hours Time Requirement Recognize how evaluation fits within the program development process. List three reasons why public health program evaluations are important. Explain the difference between formative and summative program evaluations. Describe at least five aspects of a public health program that can be measured. Identify at least two factors external to a public health program that may affect its evaluation. 1 CHES 1 CNE 1CME Total: 1-2 hours
20 Doing a Lot with a Little: Economic Analysis in Public Health 20 Description: This self- paced, online course will focus on how to use various economic analysis tools in public health settings. The advantages and disadvantages will be discussed and participants will be taken through the process of completing a cost-benefit analysis. Course Objectives Credit Hours Time Requirement Determine when economic evaluation is appropriate. Enumerate the benefits of economic analysis for program planning and evaluation. Discriminate between the various models of economic analysis. Provide examples of data necessary to perform the various economic analysis. Describe the potential intended and unintended consequences of economic measurement and analysis. Complete a worksheet that can be used for the cost-benefit analysis. 1 CHES 1 CNE 1CME Total: 1.5 hours
21 Messenger Chronicles: Introduction and the Four C s 21 Description: The Messenger Chronicles: Effective communication strategies for difficult conversations presents a new framework for communication in difficult situations. This framework shifts focus away from managing difficult people towards an understanding of the process of difficult conversations and accepting responsibility for one's own performance. Given realistic situations and real-world conversations, learners will experience communication strategies and practical techniques in context. The Messenger Chronicles is organized into a number of modules that simulate realistic conversations. Many of the conversations are split into three sections: Read, Think and Analyze. Learners will read a conversation and then will be asked to think about certain aspects of the conversation by answering questions or engaging in activities. Further information can be gained from an interactive analysis of the conversation. Course Objectives Credit Hours Time Requirement List the four aspects of communication (content, context, conduct, and character) for which individuals are responsible. Describe each step in the process of a difficult conversation. Describe strategies for effective conversations. Analyze conversations in terms of content, context, conduct, and character. 22 22 Exploring Cross-Cultural Communication Description: Exploring Cross-Cultural Communication is a web -based course that invites learners to spend time thinking about and developing their own responses to a variety of ideas and situations about culture, communication and public health. Learners will explore the meaning of culture, methods of communication, and strategies for communicating more effectively by taking part in virtual group conferences, reading and responding to simulated s, and utilizing resource documents. Course Objectives Credit Hours Time Requirement Give examples of discriminating and non-discriminating practices in providing public health services. Recognize effective methods/strategies/techniques for unbiased communication. Explain how effective cross-cultural communication influences public health service and program acceptance. Identify specific factors that influence an individual s or group's acceptance of public health information and services. Describe job-specific benefits of effective cross-cultural communication. Define culture. Explain the importance of a diverse public health workforce. Develop increased awareness of diversity. Understand how and why stereotypes/generalizations are created. 23 Health Literacy & Public Health: Introduction 23 Description: Health Literacy and Public Health: Introduces the concept of health literacy. Dispels common assumptions about people with low literacy. Presents the consequences of low health literacy. Describes public health activities with a health literacy perspective. This training is the first of two that address health literacy and public health. After completing this training, it is recommended that you enroll in Health Literacy & Public Health: Strategies for Addressing Low Health Literacy. This training contains four sections. The content and activities demonstrate how the public s literacy skills affect interactions with medical and public health staff. Course Objectives Credit Hours Time Requirement Define fundamental literacy. Define health literacy. Describe how health literacy relates to public health. List the four domains of health literacy. Give an example of each of the four domains of health literacy. List some coping strategies people use to compensate for their low literacy skills. List some groups that are more likely to be less literate. List some reasons why people may have low literacy. List the consequences of low health literacy for individuals. Describe why people, regardless of literacy skills, may fail to understand health information. 24 Addressing Health Equity: A Public Health Essential 24 Description: How healthy we are when we are born, how likely we are to get sick as we age, and how long we can expect to live are all determined to a surprising extent by our place in society. Disparities in health among income, racial, and ethnic groups in the U.S. are significant and, by many measures, expanding. This course serves as a primer for illustrating the root causes that shape health and health disparities. In addition to describing the complex interplay of social conditions associated with health disparities, it also provides a framework for exploring public and community health frameworks for addressing health equity. Course Objectives Credit Hours Time Requirement Describe terms related to health equity. Identify how historically major advances in health status resulted from broad-based social reforms. Identify the health outcomes of affected populations. Describe the social determinants of health and how they contribute to health disparities and inequities.
