1 Arizona Department of Health Services Office of Child Care Licensing Child Care Physical Plant Architectural Evaluation Dear Applicant, The expectation of the general public and a preliminary requirement for the Office of Child Care Licensing (OCCL) is that a building or physical plant of any facility used for the care of infants and children be evaluated for construction safety and building code compliance. It is a requirement for the applicant to obtain a Child Care Physical Plant Evaluation by hiring their own contracted architect to certify that new and existing facilities have been checked for compliance with the local building codes and with the child care rules for the physical plant. * An architect may call for assistance with the building s design, remodeling or in preparation for the building s use as a child care facility. The Department of Health Services - Architectural Review staff can be reached by phone at or by fax at The Office of Child Care Licensing has compiled a list of architects who are interested in evaluating child care facilities statewide. This list is available on the Arizona State Health Department/Child Care Licensing s website at or by calling the Child Care Licensing office at *Note: It is the responsibility of the architect hired by the applicant to complete the Child Care Physical Plant Evaluation.
2 DRAWING REQUIREMENTS Arizona Department of Health Services Office of Child Care Licensing Child Care Physical Plant Evaluation AAC R The Architect must provide the following: A brief narrative description of the new facility, existing facility or proposed modification, including the location and assigned age groups of each room in which activities are or will be conducted. If the facility will be in an existing building: A copy of the drawings can be obtained and these plans may be utilized in completing this Evaluation, even if the drawings were prepared by a different architect. These drawings must be supplemented by indicating the current as-built condition on a revised floor plan. To obtain such drawings, contact the building owner, landlord, architect of record, the city or the county. If the physical plant of a facility is less than 3,000 square feet and is occupied by less than 20 enrolled children and staff members, then submit a copy of all building or zoning permits or certificate of occupancy issued by the local city/county to the facility. If the building is newly constructed, added to or remodeled, provide a copy of the Certificate of Occupancy issued by the local City/County Building Department. Drawings shall include only the following: NOTE: Full sets of construction documents will not be accepted. 1. One copy each of the architect s final drawings as described below, approved by the city/county, with necessary information to show compliance with these rules. 2. An electronic.dwf format on a CD diskette. 3. An 11 x17 or an 8½"x11" reduction of the drawings (floor plan, site plan and building section with graphic scales.) Full sets of construction documents will not be accepted. I. SITE PLAN A site plan, drawn to scale which includes: Location of building(s) with site dimensions. Accessible routes and ramps. Location of parking and driveway paths. Major street names. Adjacent buildings, if the facility is located in a shopping center or school. North arrow, drawing scale and site plan key notes. Fenced playground adjacent to building, with area, type and height of fence and gates. Proposed shade. The location of swimming pool, if applicable, and the protective fence.
3 II. FLOOR PLANS The floor plan shall be an architectural drawing, drawn to scale. Indicate the rooms to be licensed with the following information: Room number or name. Purpose of room. Square footage of each room. Note: The definition of a room is a space or area enclosed on more than 80 percent of the perimeter of such space or area. When determining the enclosed area, openings less than 3 feet in clear width and less than 6 feet 8 inches high need not be considered. Age group of children using each room (infant, one year olds, preschool, school age.) Toilet rooms and fixtures, including accessibility requirements, if applicable. Diaper Changing Area: Indicate location of counter top and adjacent sink within each room that will have diapered children. All cabinets, sinks, drinking fountains, fire extinguishers, smoke detectors, fire alarm-pull stations, horns/strobes, etc. Wall and floor finishes. Direction of door swings and the width of the doors. North arrow, drawing scale and floor plan key notes. III. BUILDING CROSS SECTION The building cross section shall be an architectural drawing, drawn to scale, which includes the following information: IBC construction type. Building materials. Structure (roof and floor framing, etc.) Foundation and footings. Any additional comments, if needed. IV. SPECIAL REQUIREMENTS: FACTORY-BUILT BUILDING Factory-built buildings and their foundation systems shall be approved for permanent installation, at the facility address, by the State Office of Manufactured Housing (OMH), Department of Building and Fire Safety, per ARS 41-Chapter 16 and R (E). Submit the following: *Copies of OMH approved documents, site plan, floor plan and foundation plan, drawn to scale. *OMH Installation Permit. V.
