1 Parent-Child Assistance Program (PCAP) FETAL ALCOHOL & DRUG UNIT, BOX UNIVERSITY OF WASHINGTON SCHOOL OF MEDICINE SEATTLE, WASHINGTON (206) PCAP Fidelity Checklist Overview: The Parent Child Assistance Program (PCAP) Fidelity Checklist is a quality assurance tool to be used in the pre-implementation phase of PCAP replication site development. The checklist reviews the core characteristics necessary in designing a PCAP replication site. First, the PCAP Fidelity Checklist asks questions in two areas important to consider when determining the feasibility of developing a successful PCAP replication: the community setting and the hosting agency setting. Next, the Fidelity Checklist covers eight core components (each with related characteristics) that PCAP considers necessary to ensure faithful replication of the model. PCAP Setting Community Setting Community need Community provider network Community support Hosting Agency Setting Hosting agency Agency support Are data available to demonstrate a significant maternal substance abuse problem in the community served? Is there strong evidence of collaboration among agencies serving high-risk populations? Do policymakers and funding sources support services for high-risk families and substance abusing populations? Is PCAP operated within a hosting agency known as a successful provider of services to high-risk families or substance abusing populations? Does the hosting agency endorse the implementation and operation of PCAP, and do staff understand the PCAP intervention model?
2 Instructions for Fidelity Checklist: The PCAP Fidelity Checklist is organized into eight core components, with several characteristics per component. Each characteristic has one anchor statement that describes what the ideal replication of the PCAP model would look like. For each characteristic, consider the elements of your PCAP site relevant to that characteristic. Next, select a rating on the three-point scale [3, 2, 1]. A score of 3 indicates the PCAP replication site is planning to replicate that characteristic. A 2 means the PCAP replication site can probably replicate the characteristic with coaching and guidance from PCAP Washington staff. A 1 indicates the PCAP replication site will not be able, or does not plan to replicate the characteristic t. stands for don t know and indicates the characteristic cannot be scored for your site. Component 1: The Clients 1.1 Client eligibility 1.2 Client enrollment 1.3 Relapse or setbacks among clients capacity to implement this characteristic of PCAP? Referrals will meet all 3 eligibility criteria to be enrolled in PCAP. Clients will give signed consent to participate. Clients will not be asked to leave the program because of relapse of setbacks. don t know
3 Component 2: Client Intervention Setting 2.1 Structure of relationship with clients 2.2 Program length 2.3 Frequency of interaction with clients 2.4 Case Manager Client Ratio 2.5 Case manager accompaniment Case managers will work with clients one-to-one. Case managers will work with clients for three years beginning at enrollment. Case managers will meet with clients face to face a minimum of twice monthly. Case managers will work with 16 active clients or less. Case managers will regularly accompany clients to service provider agencies and case consultations
4 Component 3: Characteristics of Staff 3.1 Case Manager professional qualifications 3.2 Case Manager background 3.3 Clinical Supervisor professional qualifications Case managers will have a minimum of a two-year degree and two years of community based experience working with high-risk populations. Case managers will have experienced similar life circumstances as clients, have surmounted these difficulties, and demonstrate continued success. Supervisors will have a minimum of a BA degree in a mental health or social work field; have a minimum of four years of experience working with high-risk populations, and administrative and supervisory experience

6 Component 5: Conducting the Intervention 5.1 Theoretical Practice Relationship Theory 5.2 Theoretical Practice Stages of Change & Motivational Interviewing 5.3 Theoretical Practice Harm Reduction 5.4 Core protocols on boundaries and standards 5.5 Core protocols on boundaries and standards 5.6 Continual assessment of client strengths and problems 5.7 Interactive assessment and planning of client goals supervisors will understand the concepts of Relationship Theory and will use it in daily practice with clients. supervisors will understand the concepts of Stages of Change and Motivational Interviewing; and will use it in daily practice with clients. supervisors will understand the concepts of Harm Reduction; and will use it in daily practice with clients. Staff will be familiar with and understand the PCAP protocols on boundaries and standards. Case managers will work continually with clients to identify individual goals and incremental steps necessary to meet those goals
7 Component 5: Conducting the Intervention (continued) 5.8 Continuity between program goals and client goals 5.9 Inclusion of client social and family network 5.10 Case manager operates within a network of service providers 5.11 Integrity of Client File Case managers will coordinate program goals with client goals to create individualized intervention plans for each client. Case managers will develop a network of contacts with family and friends involved in a client s life. Case managers will develop and maintain professional relationships with community service providers and help clients utilize appropriate and available services. Case managers will maintain client files according to PCAP client file protocols
8 Component 6: Clinical Supervision of Case Managers 6.1 Supervisor-Case Manager Ratio 6.2 Quality of relationships between Clinical Supervisors and Case Managers 6.3 Frequency of supervision 6.4 Content of supervision 6.5 Accessibility of Clinical Supervisors 6.6 Frequency of staff meetings Clinical supervisors will provide supervision to a maximum of 9 case managers. The clinical supervisor will elicit from the case managers honest observations and personal responses to interactions with clients. Clinical supervisors will meet individually with each case manager a minimum of every other week to review each case. During supervision, clinical supervisors will monitor case manager paperwork due, discuss and review each case, assess progress toward client goals, determine next steps, and identify accomplishments and challenges. Clinical supervisors will be accessible to case managers for consultation and will accompany case managers in the field as the need arises. Clinical supervisors will organize staff meetings on a weekly basis
9 Component 6: Clinical Supervision of Case Managers (continued) s of PCAP Model 6.7 Content of staff meetings During staff meetings, case managers will offer peerfeedback on cases and clinical supervisors will periodically invite community service providers for discussion on topics relevant to the service population
10 Component 7: Clinical Supervisor in the Community 7.1 Clinical supervisor as intervention gatekeeper 7.2 Clinical supervisor role in the community The clinical supervisor will oversee the screening of community referrals, determine eligibility, and offer enrollment to potential clients. The clinical supervisor will be involved in the community in order to: identify services available, prevent service barriers by understanding various agency operations, and address service barriers as they arise
11 Component 8: Program Evaluation 8.1 Data collection 8.2 Dissemination of evaluation data to staff 8.3 Dissemination of evaluation data to community All case managers and supervisors will collect data according to PCAP protocols. PCAP data will be distributed to staff (clinical supervisors and case managers), used to improve the quality of intervention practices, and to assess the outcomes among clients. Data generated on the PCAP model will be conveyed to a wider audience, e.g. presentations to the community, at conferences and meetings; articles in newsletters, etc
