From plans with access to a nationwide network of nearly 1 million providers to plans centered on a medical home concept that lets you choose a primary care clinic near you. About this provider directory This directory encompasses three different networks Providence EPO Network, Providence Choice Network and Providence Neighborhood Network. The chart below provides a visual representation of each network, its level of access and associated plan(s). When you select the Choice plan, your designated medical home care team from the Providence Choice Network supports and arranges all of your health care needs, including referrals to specialists as needed. Choosing a health care provider A primary care provider specializes in internal medicine, family medicine, general practice, pediatrics or obstetrics/gynecology. Primary care providers perform exams, provide preventive care and treat many routine issues in their offices. We encourage you to choose one provider who can serve as the primary contact for your health care needs; however, you are not required to select a primary care provider nor are you required to have a referral when seeking the care of a specialist. In the event you see a specialist, it is recommended that you notify your primary care provider so that your medical information may be shared between providers. Should you change providers at any time, you may wish to request that your medical records be forwarded to your new provider s office. Native American members Native American members may also access covered services from Indian Health Service (IHS) facilities. These services are provided for the same or less cost than services offered by participating providers.
