PBWDB is a regional organization serving the same 17 counties and similar clientele as that of Texas Permian Basin, making this a suitable partnership for continuous support and growth of both programs to serve the citizens of the Permian Basin. In May 2009, our Area Information Center successfully attained the AIRS Accreditation. The accreditation process assesses the ability of an I&R program to demonstrates full compliance with AIRS Standards for Professional Information and Referral. How TO USE THIS DIRECTORY Agency Listing: All agencies are listed alphabetically and by county. Each agency complies with the Inclusion and Exclusion criterion. Service Index: This is a keyword listing of agencies by service provision. Agencies are listed alphabetically within the category and page numbers are noted. Some agencies will be listed in multiple categories depending on services they provide. In addition agencies, providing services in more than one county will be identified by (*). Disclaimer is pleased to offer the Permian Basin Community Directory 2012 edition free of charge as a community resource. Every effort has been made to supply complete and accurate information based on information supplied by the agencies listed. Inclusion in the directory does not imply endorsement nor does exclusion imply disapproval of any organization. Each agency must comply with Texas Permian Basin inclusion and exclusion criteria and must fill out database forms to be included in the directory and database. Please report changes or additions to. 4 Area Information Center (AIC) Inclusion/Exclusion Criteria The s Inclusion and Exclusion Criteria is based on the Alliance of Information and Referral Systems (AIRS) Standards for Professional Information and Referral. Accordingly agencies who meet the following inclusion /exclusion criteria are eligible for inclusion in the Texas Permian Basin s database. The information maintained by the Area Information Center (AIC) includes any governmental, non-profit or critical for-profit agency that provides a health or human service required to address the needs of all groups in the Permian Basin. recognizes that the size and make-up of each community in the 17 county areas will influence the depth and complexity of its database. The following types of agencies and information will be included in our database: All state-funded health and human services, and government agencies Nonprofit health and human service agencies Community service organizations that offer a social service to the community at large; such as churches and social clubs that do not require membership in the organization (NOTE: must have been in operation for a minimum of six months) Critical for-profits that provide a service that is not addressed by a non-profit organization and/or offer free services or a sliding scale fee or that are Medicare, Medicaid, or CHIP approved Hospitals, health clinics, licensed home health agencies, nursing homes that are Medicaid or Medicare approved, and other medical agencies that are Medicaid or Medicare approved and/or offer free services or a sliding scale fee Advocacy groups, civic groups, self help and mutual support groups Health and human service professional organizations Licensed and registered day care centers Federal, State, County, and Municipal government offices reserves the right to exclude certain organizations from its database. Potential grounds for exclusion or removal from the database may include, but not limited to, service non-delivery, fraud, misrepresentation, discrimination, criminal activities, serious complaints lodged against them with any regulatory body, and/or private companies that do not provide either free or sliding scale fee services. If an agency/service provider does not meet the guidelines of the inclusion/exclusion criteria, will notify that agency of this decision by letter or . In the event that an agency/service provider wants to file a complaint or appeal that agency may do so in writing within 30 days of receiving the letter notifying the agency that they do not meet the inclusion/exclusion criteria. The complaint will be presented first to the Project Director and /or to the Advisory Board. After the application is review based on the inclusion/exclusion criteria the decision is considered final. The agency will be notified in writing as the outcome of the complaint or appeal with in 30 days of this review. Revised August 30, 2011
