2 Welcome We understand that the process of enrolling in school is an important and involved process. To assist you through this process we have put all the important information and forms into one document for you. The Enrolment Procedure (on page 6) is your starting point and will step you through the process of enrolment for your particular circumstance. Use this guide to ensure you provide all the important documents photocopied with your Enrolment Application. The Enrolment Application is the next part of the package. This form is a requirement of Education Queensland and must be completed accurately and in full. The Enrolment Interview Profile is used to collect background information on your child so as to assist us in providing a program that is responsive to their needs. You can assist us by completing the responses before send in your application. We will then talk through your answers with you at the Enrolment Interview to ensure we understand all of your child s strengths and needs. The Catchment Map shows the area where students have priority for enrolment. If you reside outside of this area, you must complete the Catchment Exemption form included in the Enrolment Application for consideration in conjunction with your application for enrolment. The Enrolment Agreement must also be completed and indicates that you understand the conditions upon which enrolment occurs at The Gap State High School. Finally this booklet is made up of a number of other Consent and Agreement Forms that must be completed as a part of your enrolment. These forms provide access to school computers, the Leadership Training Centre, the School Resource Scheme; the school Chaplaincy program and a Student Consent Agreement (should you chose to give it) to use photographs and images of your students. Submit your Enrolment Application and required paperwork to our Enrolments Officer, Sharon Lyon, who will organize a time for an interview. Enrolment Applications for the next Year 7 intake will be processed the year before the year of enrolment. Enrolment interviews will also commence in March for new students attending the following year. All other Year Level enrolments will be processed when an application for enrolment is received accompanied by all the required documentation. Live out of catchment? Submit your Enrolment Application accompanied by all documents listed below, also please complete the Out of Catchment Form on page 6. The application will then be submitted to the Principal for consideration along with all the relevant paperwork. You will be notified via mail of the outcome of your application. 8 Education (General Provisions) Act 2006 Section 155(1) Approved form SEF 1 V4 Frequently Asked Questions Does my child need to participate in an enrolment interview? What do I need to do to prepare for the enrolment interview? Yes. Every new student enrolment at The Gap State High School involves parents making an application for enrolment by completing the required paperwork and attending an interview with either the Guidance Officer or another delegated staff member. Read the enrolment process flowchart in this enrolment application, noting all the documentation you will need. Sign and complete all forms in the enrolment guide BEFORE sending in the enrolment application, once all documentation is received you will be notified regarding an enrolment interview if your application is successful. What do I need to do if I have a student with a verified impairment or diagnosed learning difficulty? Eg intellectual impairment, Autistic Spectrum Disorder How do I know if I live in the catchment area and can I still apply if I live outside the catchment? My child is interested in Instrumental Music, what should I do? What happens on the first day of school at the beginning of the year? Is there a school dress code? Complete your enrolment application and provide all relevant copies of the verified impairment or diagnosed learning difficulty as well as the documentation needed for the enrolment process. See the catchment map on Page 4. All students can still make an application to the school. However, priority is given to in-catchment enrolments, remaining student admissions are at the consideration of the Principal. Please fill in the Instrumental Music form in this enrolment application. Students will be required to complete a small music theory test and audition. These will give the teachers an indication as to how to group students for lessons. On a student s first day at high school they are introduced to their Roll class, receive their timetables and are taken on a tour of the school. Students are introduced to the many student support services in the school including their form teacher, year level coordinator, school administration, guidance officer and chaplains. Time is taken to ensure that the needs of all students are addressed and they have a successful and enjoyable introduction into The Gap State High School. Yes this is a uniform school and the students are expected to wear the appropriate uniform. All uniform items are available at our Uniform Shop both new and second hand. There are photos and information about the correct shoes that meet the Workplace Health and Safety requirements in our Parent Reference Guide as well on The Gap State High School website. In winter students must wear the correct winter jumpers sold at the Uniform Shop NOT non-uniform jumpers. What sport options are available for students? 9 Where can I find more information about the consequences of unacceptable student behavior? What support is there for the social/emotional welfare of my child? When are we notified regarding the outcome of our application to the school? If my child is absent, what should I do? If my child is late for school, what should they do? My child needs to attend an appointment during the day. What should I do? Does the school have insurance cover for my child? If you live in catchment your enrolment is automatic. If you live out of catchment and there are places available you will be informed within a week of your application being received or at the time of interview. Parents should leave the name and year level of the student on the message bank on the day of the absence preferably before 9.30 am. SMS: studentabsences@thegapshs.eq.edu.au. Phone: Students arriving late to school must report to the office for a late slip which will indicate the reason for lateness and time of arrival. The note is shown to the teacher to gain late entry to class. A note or phone call explaining the reason for the late arrival is required. A leave pass will be issued if a student requests to leave school during the day for a specific purpose such as a dental, orthodontist, physiotherapy, optometrist or doctor s appointment. A note/phone call/ must be received from a parent/guardian explaining the reason for leaving early. These situations during school hours are to be avoided unless absolutely necessary. Curriculum and physical activities, particularly contact sports, carry inherent risks of injury. Parents are advised that the Department of Education and Training does not have Personal Accident Insurance coverage for students. Education Queensland has public liability coverage for all approved school activities and provides compensation for students injured at school only when the Department is negligent. If this is not the case, then all costs associated with the injury are the responsibility of the parent or caregiver. It is a personal decision for parents as to the type and level of private insurance they arrange to cover students for any accidental injury that may occur. Sections of the form not marked (*) are optional. However, failure to complete these sections may result in the school not being eligible for important Commonwealth and state funding reliant on such information. Parents of all students in Australia have been asked to provide information on their family background as part of a national initiative towards providing an education system that is fair to all students, regardless of their background. The required information includes the Indigenous status and language background of the student, and the education, occupation and language background of the parents. If you have any questions about the enrolment form or process, or require assistance completing this form, including translation services, please contact the school in the first instance. PRIVACY STATEMENT The Department of Education and Training (DET) is collecting the information on this form for the purposes outlined in the Education (General Provisions) Act 2006 (EGPA 2006), and in particular for: i. assessing whether your application for enrolment should be approved ii. meeting reporting obligations required by law or under Commonwealth state funding arrangements iii. administering and planning for providing appropriate education, training and support services to students iv. assisting departmental staff to maintain the good order and management of schools, and to fulfill their duty of care to all students and staff. v. communicating with students and parents. This collection is authorised by ss. 155 and 428 of the EGPA DET will disclose personal information from this form to the Queensland Studies Authority when opening student accounts, in compliance with Part 2A of the Education (Queensland Studies Authority) Act Personal Information from this form will also be supplied to Centrelink in compliance with ss.194 and 195 of the Social Security (Administration ) Act 1999 (Cth). De-identified information concerning parents school and non-school education, occupation group and main language other than English and students country of birth, main language other than English, sex and Indigenous status, is supplied to the Commonwealth Department of Education, Employment and Workplace Relations in compliance with Commonwealth state funding agreements. Personal information collected on this form may also be disclosed to third parties where authorised or required by law. Your information will be stored securely. If you wish to access or correct any of the personal information on this form or discuss how it has been dealt with, please contact your child s school in the first instance. ENTITLEMENT TO ENROLMENT Under the EGPA 2006, an applicant for enrolment at a state school must be enrolled if they are entitled to enrolment. While not exhaustive, the following matters may affect an applicant s entitlement to enrolment at a state school: failure to adequately complete this enrolment form if the school has an Enrolment Management Plan or an Enrolment Eligibility Plan, (enrolment is subject to eligibility under the plan) the applicant is a mature aged student, (the applicant may not be enrolled without a positive notice) the applicant is not of correct age for enrolment (relates to Preparatory Year and Years 1 to 7) the applicant has been excluded or cancelled from enrolment or is subject to suspension from a state school at the time of the application the school is a state special school and the applicant does not meet the criteria for enrolment in a special school the school principal reasonably believes that the applicant presents an unacceptable risk to the safety or wellbeing of members of the school community (application is referred to Director-General) the proposed enrolment requires approval as part of a flexible arrangement under s.183 of the EGPA 2006, and the arrangement has not yet been approved the student is not an Australian resident or citizen or the child of an Australian permanent resident or citizen (visa restrictions may apply, fees may be charged, in some cases legislation requires that the applicant must obtain approval from the Chief Executive via Education Queensland International (EQI) to enrol). 11 Application to enrol in a Queensland State School Education (General Provisions) Act 2006 Section 155(1) Approved form SEF 1 V4 This sheet contains information on how to complete the Application for Student Enrolment Form (SEF Version 4). Entitlement to enrol Under the Education (General Provisions) Act 2006 (Qld) a state school must enroll an applicant if they are entitled to enrolment. While not exhaustive, a list of the matters which may affect an applicant s entitlement to enrolment are included on the previous page of this enrolment form. Questions which must be answered* The Application for student Enrolment Form contains a number of questions marked with an (*) which must be answered. These include Student demographic details, Student address details, Student family details, Country of birth, Emergency contact details, student medical information and the Application to enroll. These questions and consent are considered necessary to ensure the school can undertake its administrative and care responsibilities. Sections of the form not marked (*) are optional. However, failure to complete these sections may result in the school not being eligible for important Commonwealth and State funding. Parent s occupation and education All parents across Australia, no matter which school their child attends, are being asked to provide information about family background (answering this question is optional). The main purpose of collecting this information is to promote an education system which is fair for all Australian students regardless of their background. Sighting of birth certificate Schools are required to sight and copy a child s birth certificate. An alternative to a birth certificate will be considered where it is not possible to obtain a birth certificate (eg child born in a country without a birth registration system passport or visa documents will suffice). Name on enrolment form A child should be enrolled under their legal name as per their birth certificate. There is provision to also record a child s preferred family and given name. The legal name will appear on semester reports, however, at the parent s request, the preferred name can be used. The preferred name only will be used on internal school documents such as class rolls Evidence of Student s Immigration Status This section is required to be completed by a student who is not an Australian citizen and requires information to be recorded about a student s passport and visa. Medical information and emergency contacts A child s medical condition, symptoms, management and medication/s must be documented. Medical conditions may include (but are not limited to) seizures/epilepsy, fainting, diabetes, asthma, heart problems, anaphylaxis and allergies (such as food or insect stings). Parents must indicate if they are an emergency contact. Three additional emergency contacts are also required. Religion (not applicable to High School) Parent/guardians are asked to identify a child s religion. From Year 1 a child will be placed in the nominated religious instruction class if it is available. If this section is left blank or marked no religion or a non-religion is nominated, your child will be recorded a No Religion nominated and placed in alternative activities. Court Orders Any court orders concerning the welfare, safety or parenting arrangements of children should be provided to the school by parents, and the school should also be provided with any new or updated orders. Office use This section is to be completed by the school and will assist in documenting specific details in relation to a student s enrolment, including confirmation of the sighting of documentary evidence such as a student s birth certificate, passport or visa and student s mature age status. DETE will only use this information to make a decision about a student s eligibility for enrolment. The information will only be used by authorized employees of the department and DETE will only record, use and disclose the medical information in accordance with the confidentiality provisions at Section 426 of the Education (General Provisions) Act It is essential that you advise the school before your child s first day of attendance if he or she has any medical conditions.
