1 Visitation Assessment and Planning Worksheet Factors to consider in making A) initial visitation plans, B) quarterly visitation reassessments and plans, and C) reporting visitation status in Court Reports. Review OPP Chapter 10-5: Visitation Protocol for comprehensive policies and procedures. A. Initial Visitation Plan In developing the location, frequency and duration, and level of supervision for the initial visitation plan, consider the following factors: The primary social worker has supervised at least one visit and made an initial assessment of the family interactions. The social worker has reviewed CAT tools for an understanding of the family factors. Consideration has been given to the cultural or language needs of the family Consideration is given to the age of the child, for example, scheduling infants for more frequent visits of shorter duration. There has been a discussion with the child s caregiver concerning the possibility of having visits at the caregiver s home/facility or having the caregiver supervise visits, if appropriate. Consideration has been given to the following safety issues: Behavioral or mental health issues Active alcohol or illegal substance use by the parent Domestic violence issues Transportation arrangements can be made for both parents and child. Arrangements have been made, if appropriate, between the child and his or her siblings and between the child and his or her grandparents as required under state regulations. There is an understanding of who the child identifies as family, and visitation is scheduled with other significant people in the child s life, if possible. 1

2 B. Reassessment of the Visitation Plan A. Frequency, Duration, Supervision, and Venue of Visits Where are the visitations occurring (venue/location)? Who supervises the visits? What is the level of supervision ( unsupervised, monitored/observational, interactive, therapeutic, or security), What is the frequency and duration of the visits? How many visits have been scheduled this quarter? How many visits has the parent made with the child? Mother Father Father Has the parent missed/cancelled visits and, if yes, how many missed/cancelled visits were there and the reason that the parent cancelled or missed the visit? Mother Father Father Reasons: Were the parents late for any of the visits? If so, how many and what were the reasons given? Were any of the visits stopped early? If so, who stopped the visit (parent, caregiver, social worker, therapist, etc.) and why was the visit stopped? Do the parents visit separately or together? 2

Placement/Caretaker Involvement in Visitation What type of placement is the child(ren) in ( relative/nrefm, county foster home, Foster Family Agency foster home, Community Care Facility) and is the caregiver involved in and supporting the visitation arrangements? Has the issue of the caregiver supervising the visits been discussed with the caregiver? If not, why? Has an Ice Breaker between the caregiver and the parent(s) occurred? Date(s): If the child(ren) s concurrent plan is guardianship with relatives and it is likely that the child(ren) will continue to have contact with the parents, does the current visitation arrangement appropriately reflect that reality? III. Visitation Considerations Is the child(ren) currently receiving Family Reunification Services or receiving Permanent Placement Services? (for children receiving visitation services at Clover House [6 month maximum], they must be in FR) The assigned social worker has reviewed all of the supervised visitation reports prepared since the last reassessment. The assigned social worker has supervised at least one visit between the parent and child as part of their clinical assessment regarding visitation issues. Date(s): The assigned social worker has assessed the parent s progress in resolving the issues that resulted in the removal of the child from the family home. 3

4 IV. Quality of the Visitations What were the nature of/quality of the interactions between the parent and child during the visits? What examples have occurred during visits that demonstrate that the parent(s) have been able to more effectively and appropriately respond to the child(ren) s behaviors and needs? Do any examples demonstrate an integration of information and direction provided from parenting classes, counseling or interactive feedback? Have the parents been making progress in addressing the issues that originally resulted in the child(ren) s removal, and is it appropriate to change the visitation, consistent with progress made? Have there been any issues that have been identified during visits that are of concern? Has the parent been made aware of the concerns? What was the parent s response? What steps has the social worker taken to address the concerns? What were the children s reactions or responses to the visits? 4

Visitation with Siblings/Grandparents, Other Relatives, or Other Significant Adults If there are multiple children in the family and they are not residing together in the same home, what has been the visitation between the siblings? What are the level, venue, supervision, frequency and duration of the visits? If there are visits between the child and the grandparents, what has been the visitation between the child(ren) and the grandparents? What are the level, venue, supervision, frequency and duration of the visits? If there have been visits between other relatives or significant others and the child(ren), what has been the visitation arrangements between them? Who was present during the visits? What are the level, venue, supervision, frequency and duration of the visits? Has the visitation been appropriate? C. Transition Planning Consideration has been given to the various options available for venues and supervision of the visits (caregiver, relatives, bureau SW I, assigned SW, Clover House, Volunteer Case Aide, Chamberlin s Day Center, Kindred Souls, etc.)(locations Julian Street, Children s Shelter, Clover House, Family Resource Centers, neutral locations, etc.)? If there is a need for a change in visitation (such as moving from monitored/observational to interactive, or from interactive to therapeutic, etc.), what is the projected visitation level, venue/location, supervisor, and frequency and duration for the visitation? What is the transition plan (timetable, venue/location, supervisor; other intermediate steps needed to bridge the gap between the current visitation arrangements and the less restrictive or more restrictive visitation that is recommended? 5

6 D. Reporting to the Court: In conjunction with the information in Section A and B: I. Reassessment of Current Visitation Arrangements for the Parent and Child Quantitative and qualitative summaries of the visits over the last reporting period are written in the court report. Based on the parent s involvement, progress, and visitation pattern and on the parent s progress in addressing the issues that led to the current dependency: What is the recommended venue/location that would best meet the child s and parent s current visitation needs? What is the recommended supervision level that would best meet the child s and parent s current visitation needs ( unsupervised, monitored/observational, interactive, therapeutic, security)? If there are sibling and/or grandparent visits with the child(ren) in the family, what are the current commendations for visits? I. Statement of Current Recommendations for Visitation What is the recommended venue, frequency and duration, and type of visitation that would best meet the child s and parent s current visitation needs? Recommendation for ongoing visits between child and siblings, child and grandparents, and/or child and others. What is the current venue, frequency and duration, and type of visitation that best meets the current needs of the child(ren)? 6
