4 CARE TRANSITIONS Adventus provides comprehensive in-hospital planning and home follow-up for chronically ill high-risk older adults in an effort to improve care transitions, increase quality of care and reduce readmissions. Patients who received this program were significantly less likely to be readmitted to the hospital. Patients are also more likely to achieve self-identified personal goals around symptom management and functional recovery. Transition coaches using our proven Coleman based model have teamed up with healthcare providers and have managed to lower 30-day readmission rate among Medicare Patients. Adventus Health Care Logistics Center The purpose of the evidence-based model is not to provide ongoing care to patients but to optimize patient outcomes throughout and following an acute episode of illness. The core benefit of these programs for hospitals is that they are relatively low cost to implement and if successful, they have the potential to produce a good return on investment (ROI) in terms of clinical and financial results
5 POST VISIT CALLS Adventus Health Care Logistics Center conducts post-visit calls which provide an in depth opportunity to extend the care you offer patients outside the hospital walls. Engaged and informed patients are much more likely to understand and follow their discharge instructions correctly, making it less likely they will need to be readmitted. Not only do these calls improve clinical outcomes and patient perception of care, they also help organizations maximize reimbursement. The Patient Protection and Affordable Care Act requires hospitals to reduce preventable 30-day readmissions or face financial penalties. Hospitals saw an increase in scores of 12% for their Overall Rating, a 26% increase in the Would You Recommend response and overall improvement in 14 our of 17 categories. Data consistently shows that when patients receive a post-discharge call, they rank their care in the 90th percentile on average, in patient satisfaction. When they do not receive a call, they rank their care in the 30th percentile, on average.
