1 Mississippi Access To Care Plan Major Focus Area: Population Identification - To identify Mississippians with disabilities. Goal 1: To obtain the number of people currently living in facilities or receiving community services. Progress Item Completion Date Responsible Party Survey appropriate state agencies to begin population identification with the current number of individuals receiving institutional and community services as well as those waiting for either service. State agencies will include the Department of Health, Mental Health, Education, Rehabilitation Services, and Human Services. 1. Sent survey (Attachment A). 2. Received completed surveys. 3. Compiled numbers. (Attachment B) April 2001 and ongoing Group (Attachment C) and identified Partners, Stakeholders, state agencies

Agencies to be surveyed will include Private facilities, nursing homes, PTRFs, and PCHs. 1. Sent letter and survey to identified facilities. (Attachment D). 2. Received completed surveys. 3. Compiled numbers. (Attachment E) May 2001 and ongoing Group, Licensure and Certification, DOH Develop system to collect number of people in facilities which are ready for placement in the community. Report to be compiled. immediate MAC Committee, Utilize the collected baseline data to develop a report on the current numbers of individuals with disabilities being served and those waiting for each service and support. Completed report to be compiled by MAC committee August, 2001 MAC Committee, Identify populations not residing in facilities surveyed. MAC Committee
3 Major Focus Area: Population Identification - To identify Mississippians with disabilities. Goal 2: To receive Legislative mandate for the designation of the agency responsible for the implementation of the MAC Plan including ongoing data collection. Progress Item Completion Date Responsible Party. Report the need for the designation of a single agency that will be charged with the oversight of the implementation of the MAC Plan and maintenance of data system. Recommendation made to the MS Legislature in the MAC Plan Group, MAC Committee, and Request and secure funding appropriation for the designated agency. Recommendation made to the MS Legislature in the MAC Plan Group, MAC Committee, and Mandate that public and private agencies that provide any service to people with disabilities cooperate in the data system with penalties for non-compliance. Request the enactment of Legislation that will make participation in the data system State law in the MAC Plan. Group, MAC Committee, and

Goal 3: To develop an on-going data system that allows for the identification of those individuals in need of services and supports to remain in the community. Progress Item Completion Date Responsible Party. Develop consensus of the reasons for the ongoing collection of data on individuals that can be publicized and shared with the citizens of the State of Mississippi. In other words, why are we doing this and what will we do with the information. Written report which is signed off by agency heads, other partners, stakeholders Group and identified Partners/Stakeholders and MAC committee members
5 Receive public feedback (via survey) on the development of a statewide data collection system and evaluate the willingness to participate by individuals and their families. Individuals and families should be involved in the development of the system so that they feel their participation will help them receive needed services and supports. Returned surveys and on-going Population Task Force and identification of partners, stakeholders, agency personnel Continue development of a single, uniform data collection system with information from all applicable task forces and within the parameters of the wishes and choices of individuals and their families so that participation will occur. Include: a. A new way for individuals to be identified as they are informed of Mississippi services. b. A tracking system to follow each person when they leave an institution. c. A tracking system to follow each person who is already in services. d. A tracking system to follow each person in an institution waiting for community services. e. Clear descriptions of each of the diverse groups of individuals with disabilities from whom data is being collected. f. Clear descriptions of each current services and supports Single registry on-going MAC Committee, State Plan, and
6 Launch a statewide media campaign to target those individuals who are not currently receiving any services. Release Public Service Announcements about MAC, website addresses, etc Immediate and on-going MAC Committee and Contract with local advertising agency to develop a creative, eyecatching television and radio campaign as well as written materials. Develop one page fact sheet to be distributed to each physician in MS.

Goal 4: To request and receive funding for development and continuation of needed services and supports for individuals with disabilities so that they can live in the most integrated setting possible. Maximize federal funding through establishment or expansion of various Medicaid funded waiver programs and any other services allowed by federal Medicaid regulations to support and expand community options.
