3 1.0 POLICY 1.1 The National Ambulance Service (NAS) is committed to ensuring that Controlled Drugs (CDs) are managed safely and effectively, thus enhancing staff, patient and public safety. The Medical Directorate endorses Morphine Sulphate as the opiate analgesic of choice. The NAS provides authorisation for individual practitioners to use controlled drugs. 2.0 PURPOSE 2.1 To outline the conditions for the requisition of drugs controlled (CD) under the Misuse of Drugs Acts from the Pharmacy Departments of HSE Hospitals to the National Ambulance Service. 3.0 SCOPE 3.1 This Procedure applies to persons authorized to requisition Controlled Drugs on behalf of the National Ambulance Service 4.0 LEGISLATION/OTHER RELATED POLICIES A. National Ambulance Service Parent Safety Statement B. Misuse of Drugs Regulations 1988 (SI no. 328 of 1988) C. Misuse of Drugs (Amendment) Regulations 1993 (SI 338 of 1993) D. 4 5.0 GLOSSARY OF TERMS AND DEFINITIONS 5.1 Supervisor - Leading EMT (Paramedic/Advanced Paramedic) 5.2 Manager - Ambulance Officer 5.3 Practitioner - the term practitioner identifies healthcare professionals who are registered with the Pre-Hospital Emergency Care Council (PHECC). 5.4 Operationally, this refers to NAS employees who hold registration as Emergency Medical Technicians, Paramedics and Advanced Paramedics. 5.5 For the purposes of this Procedure, the term Practitioner refers to Advanced Paramedics. 5.6 Morphine Sulphate is a Controlled Drug. For the purposes of administration by Practitioners within the NAS, it currently (subject to change) has two presentations: A. Injection: 10 mg in 1ml B. Oral Suspension (10mg in 5ml) 5.7 For the purposes of this Procedure, a controlled drug (CD) is a drug named in Schedule 2 (CD2) or schedule 3 (CD3) of the regulations under the Misuse of Drugs Acts 1977, 1984 and 1988 (S.I. No. 328 of 1988), and any amendments, and also any drugs which it is considered necessary to control for risk management reasons. The Misuse of Drugs (Amendment) Regulations 1993 (S.I. No. 342 of 1993) includes Midazolam as a Schedule 4 Drug. Both Midazolam for injection and oral Midazolam are included in this Procedure. 6.0 ROLES AND RESPONSIBILITIES 6.1 It is the responsibility of the Education and Competency Assurance Team to ensure that each Practitioner is aware of and understands this Procedure. 6.2 It is the responsibility of each Practitioner to adhere to this Procedure. 6.3 The implementation of this Procedure from a Control of Dispensing perspective will be supervised by the designated Pharmacist in charge or deputy. 6.4 The relevant Area Operations Manager, or designate, will be responsible for designating Officers to monitor the correct application of requisition procedures and the safe storage of received Drugs. 5 7.0 PROCEDURE 7.1 Requisitions A CD may only be ordered on an official Controlled Drug Requisition Form, as appended to this procedure (Appendix II) All requisitions must be written clearly and legibly and should be printed in block capitals if necessary Information required on the requisition includes: A. Ambulance Station B. Signature and PIN of the Advanced Paramedic requisitioning the drug C. Date D. Name of the Drug E. Strength of the Drug (e.g. 10mg) F. Form of the drug (e.g. tablet/capsule/injection) G. The total quantity, in both words and figures (e.g. 44 (forty four) Only one drug should be included on each Requisition Form Quantities ordered should be appropriate for the packaging of the drug and the expected use by Advanced Paramedics Requisitions should be brought to the relevant Pharmacy by the designated Supervisor/Manager The designated Supervisor/Manager must sign for accepting the drugs for delivery having witnessed the checking and counting of the drug(s) by the relevant Pharmacy Controlled Drug requisitions may not be faxed. 7.2 Out of hours Routine Pharmacy Service services are not available out of hours. NAS staff must ensure that an appropriate supply of drugs are available for immediate use Where adequate supplies are unavailable, an authorised person must complete/submit a requisition in time to secure new stock. 7.3 Returning Drugs to the Pharmacy Dept Controlled drugs must be returned to the relevant Pharmacy if not required, or out of date for appropriate destruction. Oral Morphine will be returned once the original seal is broken on the container. Page 5 of 7
6 7.3.2 The designated Supervisor/Manager will complete the Controlled Drug Return Stock to Pharmacy Book (see Appendix III) The Pharmacist will acknowledge the receipt of the returned drug by signing the Controlled Drug Return Stock to Pharmacy Book A designated Supervisor/Manager should sign to acknowledge this transaction. 8.0 IMPLEMENTATION PLAN 8.1 This Procedure will be circulated electronically to all Managers, all Supervisors and Staff 8.2 This Procedure will be circulated electronically to all Pharmacy Managers in each Area 8.3 This Procedure may also be placed in hardcopy in the Operations Equipment Procedure Manual in each Ambulance Station and Ambulance Control for ease of retrieval and reference 8.4 Each Supervisor responsible for updating Procedure Manuals will return the Confirmation Form to each Area Headquarters to confirm document circulation to all staff. 9.0 REVISION AND AUDIT 9.1 This Procedure will be reviewed whenever circumstances, and/or operational practices or a relevant event dictate. 9.2 The Pharmacist in Charge at each Pharmacy will monitor compliance in relation to the requisition process. 9.3 The Education and Competency Assurance Team will audit compliance with this procedure on a monthly basis at minimum in relation to the requisition of controlled drugs within the National Ambulance Service and any deviation will be reported to the appropriate Manager for remedial action Page 6 of 7
