It is important to differentiate the scope, volume and variety of cases. Report only major procedures report only one operation per patient. The mortality for each of the procedures listed must be reported on this form. All information should reflect the procedures from the previous residency training year. Departmental segregated totals are due to the ACOS July 31 st of each year.
