4 Boarding Information: All dogs must come in prior to boarding for a temperament test. For a dog to participate in Doggie Daycare with boarding a 5 hour temperament/evaluation test will be necessary. A $15 charge. Where does your dog usually sleep? 5 Owner Agreement I, hereby certify that my dog(s) is/are in good health and has/have not been ill with any communicable condition in the last 30 days. I further certify that my dog(s) has/have not harmed or shown aggressive or threatening behavior towards any person or any other dog. I have read and understand the following: 1. I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) is/are attending Ruuff Luv Doggie Daycare & Hotel. 2. I further understand and agree that Ruuff Luv Doggie Daycare & Hotel and their staff will not be liable for any problems that develop, provided reasonable care and precautions are followed, and I hereby release them of any liability of any kind whatsoever arising from my dogs attendance and participation at Ruuff Luv Doggie Daycare & Hotel. 3. I further understand and agree that dogs can sometimes receive injuries from a active day at daycare. Including, but not limited to puncture wounds, cuts, scratches etc The staff of Ruuff Luv Doggie Daycare & Hotel, at their sole discretion, will treat my dog as deemed necessary and that I assume full financial responsibility for any and all expenses involved. 4. I authorize Ruuff Luv Doggie Daycare & Hotel staff to do whatever is deem necessary for the health and well being of my dog(s) and agree to pay for any and all expense relating too. If medical attention is needed Ruuff Luv Doggie Daycare & Hotel will make every effort to contact you or your emergency contact. However, if you or your emergency contact are unreachable and an immediate decision must be made please, inform the veterinarian to limit the treatment cost to the following amount.$, until I can be reached. 5. I, agree to pay for any auxiliary services or products requested by me such has transportation, grooming, medications, food etc. 6. I understand, that Ruuff Luv Doggie Daycare & Hotels NOT responsible for any items brought in that may be lost, damaged or eaten. I certify, that I have read and understand the policies of Ruuff Luv Doggie Daycare & Hotel as set forth on the preceding pages and that I have read and understand the conditions, and statements of this agreement. DOGGIE DAYCARE HOURS: Monday through Friday from 7:00 am. to 6:30 p.m. Any dog left past Daycare 6:30 will be feed placed in the Dog Hotel and charged accordingly.
