2 1. Statement of Purpose The is seeking proposals for the purchase of self-contained breathing apparatuses (SCBA s) and related components. The SCBA and related components shall meet the minimum specifications listed in this request for proposal. Options are listed immediately following the minimum specifications. Proposals on the options are at the discretion of the vendor. All vendors must complete the Vendor Responsibility and Minimum Qualification form as part of their proposal. All proposals must conform to the minimum specifications and be presented on the forms provided for that purpose. This procurement action shall be conducted in accordance with s procurement policy. 2. Background Information is a rapidly growing, combination department that provides structural fire suppression, wildland fire suppression, advanced life support and specialized hazardous materials response. The district has fifteen, full-time staff in operations as well as fourteen parttime staff. We have continued to see a rapid increase in call volume as our jurisdiction continues to become closer to complete build-out. This proposal is for the purchase of SCBA units, with twenty-five (25) total face pieces sizes to be determined by and one spare cylinder each. The cost of this is not to exceed $86, These SCBA units shall be in accordance to NFPA edition. 3. Request For Proposal Timeline August 30, 2016 September 14, 2016 by 5:00 pm: Request for Proposal (RFP) Advertised September 14, 2016 by 5:00 pm: Request for Proposal (RFP) Opening September 19, 2016 September 27, 2016: Vendor Presentations October 11, 2016: Publish Intent to Award Notice

Minimum Specifications of Self-Contained Breathing Apparatus It is the intent of these minimum specifications to describe certain equipment in sufficient detail to obtain competitive proposals from qualified vendors for the furnishing and delivery of said equipment to be used by. All parts not specifically mentioned which are necessary to provide the described equipment shall be included in the proposal and shall conform in strength and quality or material and workmanship to what is usually provided for the trade in general. Any omissions of components in these specifications are inadvertent and should be included in the proposed SCBA. The SCBA shall meet NFPA 1981 Standard in Open- Circuit Self-Contained Breathing Apparatus for Fire and Emergency Services current edition. The SCBA shall be NIOSH CBRN certified and NIOSH title 42. The vendor shall supply documentation of CBRN certification. SCBA s shall be of an existing brand or model currently in production. SCBA shall have the following components: Minimum 4500-psi air supply system 45-minute carbon bottle or comparable (with pressure gauge), minimum 4500-psi Second stage regulator Heads-up display Integrated PASS device Adjustable, padded shoulder straps; and Adjustable waist strap Facepiece shall include: Nose cup with each mask Standard speaking diaphragm or comparable; and Neck strap or other means of securing mask when not in use Voice Amplification: To be included as part of the minimum specification for the SCBA or Facepiece Spare Air Cylinder: 45-minute carbon bottle or comparable (with pressure gauge), minimum of 4500-psi

4 5. Exceptions to Specifications Any exceptions to the specifications above must be outlined in writing with the proposal. North View Fire District may choose to reject proposals based on exceptions. Any exception that make the SCBA non-compliant with NFPA 1981 Standard on Open-Circuit Self-Contained Breathing Apparatus for Fire and Emergency Services or will not allow the SCBA to be NIOSH CBRN certified or NIOSH title 42 will result in the proposal being rejected. 6. Number of Items to be Purchased The intends to purchase as many SCBA units as possible, total of twentyfive (25) facepieces and a spare 4500-psi bottle not to exceed the dollar amount of $86, Vendor Responsibility and Minimum Qualification Process Each vendor interested in submitting a proposal shall submit a minimum qualifications page. The minimum qualification page shall include and will be graded on the following: A. Cost of additional parts, services, testing equipment, training and annual cost-ofoperation; B. Vendor s past performance record with C. s experience with the product proposed; D. Vendor performance with other customers; E. Manufacturer s history of performance within the industry; and F. s evaluation of vendor s ability to perform. 8. Evaluation Process A. Unit Price; B. Vendor s answers to Responsibility and Minimum Qualification Process Questions; C. Additional Options (functionality, availability and pricing) D. Annual cost of Operations (required maintenance, parts, testing, cleaning, etc.) E. On-site service response capabilities; F. Terms and conditions of warranty; G. Evaluations of vendor presentations as outlined

5 H. Firefighter field performance evaluations; and I. Information gathered from pre-qualification evaluations 9. Pricing Pricing on sales proposal shall denote unit price for complete SCBA (backpack harness with 45- minute bottles/one spare 45-minute bottle, twenty-five (25) masks with built in amplifiers or equivalent. Any additional costs for individual facepieces, spare cylinders, options, training, testing equipment, etc. will be listed separately in the proposal, and not included in the unit cost of the complete SCBA. The vendor shall provide a guaranteed purchase price on the items for the number of the items to be purchased for a period of twelve (12) months from the award of the contract. 10. Training Vendor shall supply facepice fit testing for proper sizing prior to the delivery of SCBA and related items. This includes sizing for proper facepiece and nosecup fit. The vendor shall supply initial training for field use of SCBA after delivery of all breathing apparatus and related components. Training shall include but not be limited to: A. Donning and Doffing B. Equipment Use and Operation C. Cleaning and Decontamination D. Repair and Replacement of Components E. Inspection and Evaluation F. Warranty and Return Training will be scheduled in cooperation with successful vendor, but at the convenience of and will include a minimum of three (3) two-hour sessions to operations staff. The successful vendor shall train three (3) employees to perform field level maintenance. Any training cost shall be itemized and be included as part of the proposal.

6 11. Vendor Presentation Each qualified vendor shall send a representative to provide one (1) presentation of the sample SCBA. The presentation will be no longer than thirty (30) minutes. An additional ten (10) minutes may be utilized by the SCBA Committee to ask any questions they may have. The presentation to the committee will cover features, field use, post-use inspection and cleaning procedures. 12. Sample Self-Contained Breathing Apparatus Qualified vendors shall submit one (1) functioning SCBA unit (meeting minimum specifications, Section 4) for evaluation by our committee. If the vendor chooses to include any options in the proposal, the SCBA shall include all of the proposed options. The SCBA shall be delivered on the day the vendor is scheduled for the presentation and left in the possession of the North View Fire District for a period not to exceed 14 calendar days. The SCBA submitted for evaluation will be returned after the evaluation period. Cost of delivery and pick-up will be the responsibility of the vendor. The SCBA will be used in field evaluations such as fireground operations, search and rescue drills, and maneuverability drills. will not be responsible for any damages and/or repairs to these SCBA units resulting from normal wear and tear that may occur during typical fireground evolutions and/or operations. 13. Delivery Complete delivery of all SCBA and related components shall not be longer than 90 calendar days from the award and contract and shall be delivered at the vendor s expense to the Station 21 located at:

Terms and Conditions At the time of the vendor s presentation the vendor shall submit to the District one signed, original, three (3) copies of: 1) Vendor Responsibility and Minimum Qualification Questionnaire; 2) Price Sheet; and 3) any other documents or proposal information deemed pertinent by the vendor. All documents shall be sealed in an envelope addressed to Deputy Chief Jeremiah Jones, showing on the outside of the envelope the name of the firm and the words SCBA-RFP. Failure to perform the scope of work identified in the awarded contract or failure to meet established performance standards may result in, but are not limited to, the following consequences: A. Reducing or withholding payment; B. Requiring the vendor to perform, at the vendor s expense, additional work necessary to perform the identified scope of work or meet the established performance standards; or C. Declaring a default, terminating the contract and seeking damages and other relief available under terms of the public contract or other applicable law(s). 15.
