History of previous incidents: Missing child from outside the northern region. The service went to Sydney aquarium without conducting a risk assessment, which resulted in a missing child. Risk of impact from Vehicular traffic Falling down bus stairs Children running onto the road from footpath PROBABILITY (Refer Probability Matrix) CONTROL MEASURES (include reference to legislation, codes and standards) 1E Children to be monitored by YMCA staff whilst being transported to the venue Children supervised at all times Staff have First Aid Training Appropriate first aid equipment available All children to be seated while travelling on the bus 1E Children wait on footpath, away from edge of the road All staff are first aid trained Children are instructed to hold onto hand rails on bus when departing the bus WHO IS RESPONSIBLE BY WHEN Page 2 of 10

3 Potentional to loose a child Grazes, cuts and bruises due to slips and falls Ensure the children are aware of other members of the public. Appropriate ratios are enforced. Supervisors to use Excursion Group List with regular roll calls. During transitional periods extra roll calls and head counts are conducted Children have name clearly on clothing. 1E Running only in appropriate areas designated by Staff Staff First Aid trained First Aid Kits available Educate children before arriving at excursion about appropriate behavior Staff are educated about the facilities they will be visiting at Staff meeting s prior to the Holidays Ratio of 1:5 Staff to ensure that all children are wearing their YMCA T-shirt Children aren t allowed to wear socks whilst at the venue. Childrens bags and shoes will be placed in a room and socks must remain on at all times. Staff are to monitor when children are walking through venue. This will avoid any incidents to occur. Children wear labels with centre phone number and YMCA excursion t-shirts Staff do regular head counts to ensure Page 3 of 10

4 all children are accounted for. staff spread themselves out amongst children to ensure that maximum supervision is achieved. Follow BE Sharp/SCAN Supervision Techniques. General Lost/Missing Child 3D - Children wear labels with centre phone number and YMCA excursion t-shirtsstaff do regular head counts to ensure all children are accounted for. - Staff spread themselves out amongst children to ensure that maximum supervision is achieved. Participant welfare issues such as: toilet facilities, shade, clean water, disabled access Familiarise children with site Toilets available Disabled access available Staff accompany children to bathrooms at least two children Check bathrooms before children enter to ensure they are clean and have sufficient toilet paper Staff not to leave the children alone with members of the public in bathroom Venue Specific Dim lights in walkways 2C Ensure the children are aware and prepared for walking thru Syndey Aquarium. Children to wear appropriate footwear. No sharing of food with staff members or children. Every 2 hours rehydrate children with a water break. Page 5 of 10
