2 Introduction The choices that you make every day play a crucial role in helping you to maintain your health and well-being both tomorrow and in the years ahead. AmeriHealth New Jersey Commit2Wellness programs offer you support for living a healthy life and preventing illness. From giving you incentives to make smart lifestyle choices, to providing you with individualized solutions and information for health issues that concern you, AmeriHealth New Jersey Commit2Wellness programs are designed to help you and your family stay well, prevent illness, and benefit from healthy lifestyle choices.
3 Commit2Wellness Rewards Overview Through AmeriHealth New Jersey Commit2Wellness Rewards, you earn Wellness Points for completing a variety of healthy activities, such as seeing your primary care physician (PCP) or gynecologist (GYN) for a well visit, or by going to the gym. As you take steps toward a healthier you, you ll work toward receiving a reward. Wellness Points are awarded for completing milestones in each program. Once you earn Wellness Points, you can redeem them for a wide selection of gift cards from hundreds of retailers. To be eligible to redeem any of your point accruals, you must complete an online Personal Health Profile (PHP) on amerihealthexpress.com and visit either your PCP or GYN for a well visit. Once you complete the PHP and visit to your PCP or GYN, you can elect to redeem points for completing those two activities, each worth 25 Wellness Points.
4 Fitness Program Earn 100 Wellness Points for joining and using a gym. Among other things, regular physical activity can help you reduce cholesterol and high blood pressure, manage your weight, maintain bone strength and muscle tone, and even boost your mood. Complete 120 workouts in 365 days and we ll reward you 100 Wellness Points annually. Healthy Weight, Healthy You Earn 200 Wellness Points when you complete an approved weight loss program. No one ever said weight loss would be easy, but support from others can make the challenges feel more manageable. Enroll in a weight loss program, and you can get encouragement to stick with it for the long haul. Complete your program and we ll reward you 200 Wellness Points annually for program fees. Tobacco Cessation Earn 200 Wellness Points when you complete a program to help you quit using tobacco products. Quitting isn t easy, and many people try more than once before they succeed. To help you quit for good, we ll reward you 200 Wellness Points for completing a tobacco cessation program. Nutrition Counseling Earn 50 Wellness Points for attending two nutrition counseling visits with a registered dietician or physician. Registered dieticians can counsel you on using food and nutrition to help prevent and control disease, gain or lose weight, improve fitness, or live healthier.
5 Parenting Class Earn 50 Wellness Points for attending a parenting class (childbirth preparation, prenatal exercise, education of expectant siblings). Other Programs Overview The primary goal of AmeriHealth New Jersey Commit2Wellness is to promote healthy behaviors that prevent illnesses and medical issues from occurring. Health and Wellness Products and Programs Save up to 40 percent on a broad selection of health and wellness products, including vitamins, dietary supplements, homeopathic remedies, sports nutrition products, and health-related books. You can purchase from a list of products either on our customer portal or over the phone.
6 Early Detection The best way to live and stay healthy is to prevent health problems before they occur. Yet no matter how well you take care of yourself, the effects of heredity, the environment, and other factors may not prevent illness. That s where early detection comes in. By scheduling regular health screenings, you re more likely to identify and treat a potential problem during its early stages. And, early detection and early treatment mean you have the best chances for a complete recovery. With our Connections SM Health Management Programs* and Wellness Guidelines, we will help you make decisions about and schedule important screenings in the following areas: Cholesterol Colorectal cancer Prostate cancer Abdominal aortic aneurysm (AAA) Diabetes Breast cancer Cervical cancer and human papillomavirus (HPV) Sexually transmitted diseases Osteoporosis Family Health AmeriHealth New Jersey Commit2Wellness is dedicated to keeping you and your family healthy. Because making health decisions can sometimes be difficult and overwhelming, we offer information and tools to assist you. Case management for those facing serious/complex illnesses or medical conditions. Wellness Guidelines help you keep track of physicals, immunizations, and health screenings for you and your family. Good 2 B Me is a website for adolescents and their parents that educates on teenage health issues. Baby Footsteps helps you prepare for your new baby s arrival. Adoptive parent support provides information about health resources for adoptive parents and children. Health Coaches* are specially-trained health professionals, including nurses, dieticians, and respiratory specialists, who are available 24 hours a day to address your health concerns. Spanish-speaking Health Coaches are also available. *Standard for fully-funded groups. Available as an option for self-funded groups.
7 Stress Management Learn techniques to restore balance because severe or prolonged stress can really threaten your health. It s been linked to six of the leading causes of death, including heart disease and cancer. Through AmeriHealth New Jersey Commit2Wellness, you can discover ways to let go of stress or transform it into positive energy. By listening to our audio guide CD, you can learn to balance your life, manage your stress, and do more in less time. Baby Footsteps Baby FootSteps will help you prepare to welcome your baby by providing you with prenatal resources and information about each stage of your pregnancy and your child s development. Alternative Health Receive special discounts on services and products. To keep yourself healthy, you may wish to seek services from alternative or complementary health practitioners in addition to visiting your regular doctors.
