3 Awards & Recognition Program Overview Welcome! Welcome to the Government of Manitoba s Awards & Recognition Program! This program has been developed to honour Manitoba Civil Servants for their career achievements with the Province. Employee loyalty & dedication throughout the years is an inspiration to all civil servants. In recognition, we would like to offer the following award selection as a symbol of appreciation. Thank You! Thank you to each long service employee and retiree. Congratulations on this important career achievement. 2

4 Awards & Recognition Program Guidelines Ordering Details 1. Employees should place their gift order with their Department, Division, or Branch s contact. 2. Employees may choose a gift from a lesser category than they qualify. 3. Employees are entitled to one gift only. 4. Please allow approximately 3 weeks for delivery from date ordered on non-personalized products, subject to availability. 5. Please allow approximately 6 weeks for delivery from date ordered on personalized products, subject to availability. Return Policy Items can be sent back to MDA for exchange or refund within 10 business days from the date of your event. Please call Client Services at Personalized items are not returnable. Additional Information Treasury Board General Manual of Administration Employee Appreciation Guidelines Departments are authorized to spend a limited amount of public funds on appreciation activities. For retirement and long service awards, the amounts are as follows: Retirements With a minimum of 15 years of service A framed certificate A gift valued at no more than $5 per year of service. [e.g. 15 years = $75; 40 years = $200] Long Service Benchmarks of 25, 30, 35 and 40 years of service A framed certificate An item from page 14 of the Awards & Recognition Catalogue (maximum value of alternative item $50.) 3

5 Awards & Recognition How to Order Order Form 1. Fill out the following mandatory fields on Order Form: Contact Name = your name Department/Branch = your department and branch Phone Number = your contact number Date Ordered = date you are submitting the order to MDA Date Required = date you need the items delivered by MDA Client Number = number found on previous invoices received from MDA Invoice to = mailing address where MDA should send the invoice Ship to = physical address where your order should be delivered Special Instructions = any special or additional information pertaining to your order 2. In the appropriate columns for each item indicate: The quantity required with personalization The quantity required without personalization Total quantity ordered 3. Total Each Column Personalization Form For each item requiring personalization: Write the SAP # for the item Print the name as it should appear on the item. Indicate the # of years of service to appear in the plate. 4. Sign the Order Form and fax it to MDA Client Services at
