1 LEGAL FORMS On this journey, you will likely speak with or work with young people who are under 18. If you plan to meet with minors, be sure to get permission from their families first. In this document, you will find the necessary legal documents both informational notices and release forms. FORM 1 Notice and Limitation of Liability for Interactions with Other Participants This form limits the liability of the XQ Institute, and it is provided for your information. FORM 2 Parental Notice and Limitation of Liability for Interactions with Other Participants This form limits the liability of the XQ Institute, and it includes a note for the parents of minors. FORM 3 Parental Consent/Participation Form This form enables parents to provide consent for a child to participate in the XQ Super School Project. Important: This form must be filled out as soon as the youth joins the team, and it must be included as part of the team s application. FORM 4 Student Interview Release Form This form enables parents to provide consent for their children to be interviewed and for the interview to be recorded. Important: This form must be signed before you start recording a conversation or interviewing a youth. Any materials in your application that feature the voices or images of young people must be accompanied by a signed release.

2 SUGGESTED SAFETY TIPS You agree to review and follow our suggested safety tips below. + Get to know the other team members before meeting them offline. + Always meet in public. + Meet for the first time in a populated, public place not in a private or remote location. + Tell a friend or family member of your plans. + Tell them who you are meeting, where you are going, and when you will return. Bring your phone with you. + Arrange your own transportation to and from the first meeting. Do not accept rides from people you have not met before.

XQ does not supervise or facilitate these meetings in any way and cannot guarantee the security of the meetings or the safety of members participating in any meetings. You are solely responsible for your interactions with other XQ Super School Project participants and you assume all risk for such interactions and agree not to hold XQ liable in any way for your interactions, including any team meetings that may be set up by you or other participants. We also do not inquire into the backgrounds of our participants or attempt to verify the statements of our participants. We make no representations or warranties as to the conduct of participants or their interactions and meetings with any current or future participants. We do not conduct criminal background checks or screenings on our participants. You agree to take all necessary precautions in all interactions with other participants, particularly if you decide to meet in person. YOU AGREE THAT XQ IS NOT RESPONSIBLE FOR THE CONDUCT OF ANY USER OF THE XQ WEBSITE OR ANY CONTEST PARTICIPANT. 5 FORM 3 PARENTAL CONSENT/PARTICIPATION FORM Parent or legal guardian, please read the accompanying Contest Rules, the XQ Terms of Use, and Notice and Limitation of Liability for Interactions with Other Participants carefully because they will be binding on you and your child. If you wish to give consent for your child to participate in XQ Institute Super School Project and Contest, please complete and sign this form. This Parental Consent/Participation Form must be completed within [7] days from the date it was sent to you in order for your child to be able to participate in this project and enter the contest. 6 FORM 4 XQ INSTITUTE: THE SUPER SCHOOL PROJECT STUDENT INTERVIEW RELEASE For good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I hereby (a) consent to have my voice, image, likeness, name, biographical information, conversations, statements, opinions, answers, and sounds filmed, photographed, videotaped, interviewed, or otherwise recorded (the Student Interview ) and (b) irrevocably grant to XQ Institute ( XQ ), its licensees and assignees, and each of their respective parents, subsidiaries, affiliates, successors and assigns, and to other such persons or entities as XQ may designate from time to time (collectively, Designees ), the absolute right and permission to reproduce, display, publish, broadcast, synchronize, telecast, distribute and/or otherwise use and reuse the Student Interview in any manner, in whole, or in part, composite or distorted, in any media now known or hereinafter known, including, without limitation, alone or accompanied by other material, including, without limitation, a fictitious name and other biographical material, or distorted in character or form, throughout the world, in perpetuity (collectively the Materials ). I hereby waive any rights to inspect and approve the Materials or their use. Nothing herein will constitute any obligation on XQ s part to make any use of any of the Materials or the rights granted herein. I agree that all right, title and interest in and to the Materials, including the copyrights therein, shall vest exclusively in XQ. I hereby release and discharge XQ from any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may exist in any media or in any publications thereof. Further, to the extent permitted under applicable law, I waive and release and covenant not to assert any moral rights that I may have in the Materials. I hereby warrant and represent that (1) I have the right to enter into this Agreement and grant all of the rights and assume all of the obligations set forth herein, and that use of the Materials by XQ or its Designees will not conflict with any obligations I may have with any third party; (2) I will not hold XQ or its Designees responsible for any claim or liability resulting from the use of my image or likeness in accordance with this Agreement, including, but not limited to, claims of copyright infringement, defamation, disparagement, slander, false light, invasion of privacy or publicity or the like, and (3) I will not object to, interfere with, or otherwise inhibit use of the Materials by XQ or its Designees. I have carefully and thoroughly read, understood and approved this Agreement before executing it, and I agree to be bound by all its provisions. This Agreement contains the entire understanding between the parties regarding the subject matter hereof and supersedes all prior understandings. No waiver, modification or additions to this Agreement shall be valid unless in writing and signed by both parties hereto. I agree that this Agreement will be construed and enforced in accordance with the law of the State of California without regard to choice of law principles. I am over 18 years of age and have the right to make this Agreement OR my parent/legal guardian has signed below.
