5 Instructional Objectives Participants will: 1. Apply the material covered in the training to real-life situations. 2. Experience the agitated individual's point of view. 3. Provide the best possible Care, Welfare, Safety, and Security for the individuals in their care. 13 Facilitation Dynamics Use examples that are common in your work environment. Tell participants not to become physically acting-out individuals. Discussion following the role-play is a necessity. Focus on the positive aspects of the interventions. Start with input from the observers themselves

6 Role of Family Member You are a family member of a gentleman who was recently checked into a facility for addiction/mh rehab. He called you up and said he was being mistreated. This was his first phone call in three days and he was not allowed to do anything the others were, like have free time, leave the grounds, etc. You come into the facility very angry and aggressive, verbally trying to advocate for your brother. 16 Role of Staff Member You are a staff member in the facility. In your program, there is a 72-hour observation time for new residents. During this time, residents have very tight, structured routines. After the 72 hours, they are placed in group levels appropriate to their needs and can begin to make phone calls. 17 Discussion Questions (page 21 of the Participant Workbook) 1. What levels of behavior did you observe? How did staff respond to those behaviors? 2. How was nonverbal communication used by the role-play participants? How was paraverbal communication used? 3. Did you see any signs of staff anxiety? If yes, what were they? 4. Did staff use the team approach? If so, how was the team summoned? 18 6

7 Discussion Questions (page 21 of the Participant Workbook) 5. Was there a team leader? If so, who was it? How did you know who the team leader was? 6. Which levels of the CPI Verbal Escalation Continuum did you observe? How did staff respond to each? 7. Was the least restrictive form of intervention used? 8. Did the role-play reach the level of Tension Reduction? If so, how did you know it had reached that point? 19 Optional Role: You are the family member who brought your loved one to the facility. You've gotten a call from your loved one saying, I called and he/she is coming to get me out of here. You go to the facility to stop your sibling (who doesn't really know what's going on) from taking your loved one out of the program. You arrive at the facility after your sibling has already started yelling at staff. 20 Optional Role: You are a staff member down the hall working with a group. You see an angry family member charge into your supervisor's office. You will assist only if called in by your supervisor, or if the situation escalates to a point where you feel you need to respond. 21 7

8 Transitions Transitions help link chunks of content. Transition into Unit IX: We have had the chance to see all of the concepts individually, but in a real situation, they may all be overlapping and happening at once. Let's participate in a role-play to see the application of these concepts. Transition within the unit: Let's share some of our observations of the role-play starting with the persons in care. 22 Poll Question 2 Check the best examples of transitions from Unit IX: Role-Plays to Unit X: Postvention. Time after a crisis is important for changing behavior. Allow participants to tell how they feel; leads to Postvention. How could you tell they were reaching Tension Reduction? Let's use our role-play to start our debriefing process. 23 Transitions Transitions help link chunks of content. From Unit IX to X: The intervention is not complete once the person is back in control. Next we will explore a process for achieving a long-term resolution. Now that we have all reached Tension Reduction, we may be ready to process through the incident. Postvention will be our opportunity to change behavior or the situation for next time. 24 8

9 Ideas for Your Refreshers Use incident reports for situational role-play scenarios. Tag in: If a staff member in the role-play becomes stuck, he can raise his hand and another person will tag in and take over. When an observer would like to try an intervention, she can tap on the shoulder of the staff person and take over the role. Bad situation/good situation. 25 Ideas for Your Refreshers Additional ideas for role-plays: Provide behavioral challenges for small groups to role-play possible interventions. Simulate an incident that ends in a physical restraint to use for documentation and review. Provide participants with cards that have key concepts written on them: Empathic Listening, Rational Detachment, Precipitating Factors.
