SEATING Page 4

Page 5 Seating principles Sitting correctly the child is able to take part in everyday activities. Children with special needs require special seating in order to have the correct support. Supported correctly the child is free to move and can interact with their peers. Some children might not be able to change position themselves. Therefore, it is very important that the seating system is adjustable - e.g. the tilt in space and recline functions. Further it is important to notice that the bests possible seating position: maintains bodily functions such as digestion and respiration minimises fatigue and promotes functional activity maximises independence, communication and activity A good seating position must: promote correct posture prevent pressure points be comfortable be supportive A well-fitted seating postion gives: stability comfort adequate and essential support freedom of movement the user the ability to interact with peers Head - arms - hands MOVEMENT Trunk - shoulder - head STABILITY + MOBILITY = BALANCE Pelvis, thighs, feet placement = STABLE BASE Three core elements Stable base Proper support of the pelvis, thighs and feet forms the foundation for stability. Balance The placement of the trunk, shoulders and the head are crucial to balance. Movement The free movement of the head, arms and hands give the possibility of increased movement and improved function.

SEATING/x:panda Page 6 x:pandatm x:panda is a multi-adjustable dynamic seat, delivered in 4 different sizes - each seat with up to 4 in seat width and depth adjustment coupled with sliding back growth

Page 7 The back is dynamic, making it possible for the child to extend and push back against relatively light resistance and then regain the desired sitting position. The back is lockable which could be preferred in feeding situations and required when being used in transportation. The back can also be angled to make it possible to seat the child with either an open or closed hip angle. The x:panda seat complies with ISO /4 10½ -26½ max 176 lb Dynamic back The x:panda has a unique dynamic back that absorbs the energy of extension and returns the child to upright sitting without compromising pelvic positioning Multi-adjustable Overlap in sizing means that the starting option always offers the maximum growth potential coupled with individual leg positioning Growth Individual growth in width, depth and back height extend the life of the seat and accomodate the greatest range of sizes for any chair on the market Patented! The point for rotation (recline of the back) is close to the axis of the hips which means that the back support maintains the correct position and minimizes shear

Standing Safe support while standing increases both physical and mental well-being

STANDING Page 100

Page 101 Standing principles Wheelchair users are at high risk of developing secondary conditions because they often sit for long periods of time. These secondary conditions can reduce their quality of life significantly. The advantages of putting users into a standing position include increasing bone mass, strengthening muscles, furthering respiration, and enhancing development of the hip socket. The upright position also increases a user s viewing area and enhances their feeling of well-being. When standing: When positioning children in standing frames it is important to use the pelvis as a key point. The pelvis must be symmetrical and it must be neutrally tilted. The user must stand symmetrically with equal weight on both legs and with pelvis in a neutral position. Head and body should be in midline. Weight bearing is important to the user when developing and strengthening the hip joints. Standing can: increase bone density decrease joint contractures improve cardiopulmonary function improve digestion increase bone growth increase alertness Many CP patients are users of standing frames. They say the spasticity in their legs decreases after regular use of the standing frame. In recent years science has shown that standing has a positive effect on reducing and preventing spasticity. Standing often is more important than the length of time standing. In normal development the child starts crawling/pulling themselves at eight months, standing at ten months and walking at months. Prone standing: Prone standing stimulates the user to more active extension, epecially if the frame is tilted a few degrees forward. Standing supine is chosen if the user has poor head control or to make transfer easier to the standing frame. We recommend that disabled children get their first standing frame at about the same time most children start total weight bearing on their legs, i.e. around the age of one year.

Walking can give a child the opportunity of getting around, socializing and interacting with its peers

WALKING Page 140

Page 141 Walking principles To walk is a skill most people consider as a natural phenomenon and therefore do not really think about. Learning to walk is a long process, which lasts over 3-4 years, starting with few uncontrolled steps ending up as a controlled skill. Walking is the simplest way for a person to move. The whole body is involved, skeleton, joints, muscles, nerves, heart, lungs etc. Disabled children can have difficulties in walking for various reasons, they must therefore be guided and stimulated in taking their first steps. Start walking: Putting the weight on one leg, leaning forward to unbalance the body (the centre of gravity falls in front of the body s plumb line), swinging the other leg forward and placing it on the ground in front of the body s centre of gravity (it becomes the supporting leg), lifting the first leg from the floor and swinging it forward... Walking can give a child the opportunity of getting around, socializing and interacting with its peers. Walking can: improve bone density decrease joint contractures improve cardiopulmonary function improve digestion increase leg muscle strength decrease spasticity improve hip development promote psylogical wellness In normal development the child starts crawling at eight months, standing at ten months and walking at months. At R82 we recommend that disabled children get their first gait trainer at around the time an able bodied child would start to weight bear on their legs and are ready to take their first steps. The human way of walking differs from person to person but the mechanical process is the same. This process is important to know because this is what we aim for when teaching a child to walk. During the walk: Rotation of the hip, tilting of the pelvis, body weight shifting from side to side and swinging of the arms, to name just a few. All these movements happen in order to keep the balance of the body centred around the midline.

It offers flexibility, growth and ease of use while children are learning to walk and adapting to their changing needs. 219 Accessories The right accessories are important in obtaining the correct position

accessories Page 218

Page 219 The right accessories To achieve the best sitting position, the disabled child often needs different types of accessories. A stable base means a well supported pelvis. To achieve that, you can build up the seat from beneath with a contured cushion that supports from beneath, a correctly positioned lumber support and a hip belt. The tray can be useful in different ways. Children with poor trunk and head control may need a tray in front of them for extra stability. Of course the tray will be useful for play and when feeding the child. The trunk might need extra support to make it stable or to relieve scoliosis (asymetry). This can be achieved by either using side supports or a vest. Furthermore, some children need a head support to keep the head in midline or simply to support the head.
