National Identity Card (issued by NADRA) b. Machine Readable Passport c. Medical Fitness Report d. Visa Application Form (attached) with photographs 2. e. Departmental No Objection Certificate, NOC/ ex Pakistan leave (the process to obtain these documents be initiated earlier and preferably deposited with complete documents) Affidavit from Guardian (for lady doctors ) Details / Attestation Formalities The original with two photocopies. a. The original with two photo copies b. Old passport(s), if held, are also required to be attached with the existing passport. c. Minimum validity period of passport required is two years. a. Medical examination will be done through GAMCA. The GCC approved medical centers for Saudi Arabia are at Rawalpindi / Islamabad, Lahore, Gujranwala, Multan, Peshawar and Karachi (the city wise details with telephone numbers are given at Annex A). The candidate will report to the concerned medical center for medical examination. The GAMCA number will be allotted by the medical center directly or obtained by the candidate on telephone through area GAMCA office. b. Original passport will be produced at the time of medical examination. Candidate will deposit the original copy of visa application form issued by OES along-with eight (8, passport size/4x6 cms) photographs with sky blue back ground with OES office at the time of depositing complete documents. Candidates serving in government / semi- government departments will deposit departmental NOC /ex- Pakistan leave. Candidates retired from Armed Forces will deposit departmental NOC (ex-army candidates to obtain NOC from GHQ, AG s Branch, W&R Directorate (W&R-4). Documents to be provided to the Directorate are given at Para 1 of Annex-B. Other candidates will produce an Affidavit on stamped paper of Rs.20/- stating that they are not government / semi-government employees. Two original copies of affidavit on rupees 20/- stamped paper to be obtained (from her husband and un-married from her parents) stating that he has no objection to the lady s proceeding abroad. An attested
3 3 photocopy of affidavit is also required Family Documents a. Nikahama and Children s NADRA Registration (Form B) b. Medical Fitness Certificate & Vaccination Card c. Photographs with Sky Blue background d. Visa Application Form e. Original Computerized Passport f. Original ID Card g. Family Visa Service Charges Document Attestation & Arabic Translation Facilities The original translated in Arabic and duly attested from Foreign Office. Two photocopies each of attested documents are also required. The original medical fitness certificate of wife /husband and children above 12 years. Children Vaccination Card for less than 12 years is also required. A photocopy each of above documents is also required. Eight copies (passport size, 4x6 cms) each of wife /husband and children. (attached) for each family member with minimum validity of 2 years alongwith all old passports (if held) with two photo copies. ( wife or husband) with two photo copies.
