(see requirement for additional details posted on Bid deadline is Dec 4, 4pm. NO LATE BIDS will be considered after deadline. If revised bids submitted last version will be considered final document. This RFP is for a Turn-key project. Our goal is to offer school district a cost effective and best available bandwidth. Service provider preference is within 100 miles radius (approx. 1 hour) of school for service support and issues. Products and services must be operational and compatible with all network equipment brands used by Oakdale Public School. Requested products and services may be contingent upon school board approval and Erate funding approval.. SERVICES START DATE: Current contract ends Dec 13, 2016; PAYMENT TERMS: Requesting a proposal no longer than 36 months. USAC funding commitments may experience a temporary funding delay depending on PIA review process. Service provider must agree to begin internet access upon school order request and no later than by Dec 13, 2016, including any special construction or considerations (if any). The School acknowledges if USAC denies funding, the school will be liable for costs for the funding year.

2 ERATE TERMS: All terms and conditions as required by Universal Service Administration Company Schools and Library (E-Rate Program) must be met. Service Provider must have USAC portal access and SPIN# (Service Provider Identification Number) and provide on bid response. Oakdale School is seeking a Service Provider Invoice Credits for Universal Service Administrative Company (USAC) Erate funding. Service Provider must agree to apply any Oklahoma Universal Service Fund (OUSF) for credits for the internet access services within 90 days of completed service start date. Must include all configuration, installation, special construction information and costs by each site location. Provide all available/additional options provided by the service provider for consideration, this sample list included but not limited to; (1) Internet filtering; (2) leased router(s) & other required equipment; or (3) purchase equipment cost. Any service not eligible for E-Rate discounts must be identified clearly. These services may be executed by the entire consortium or as individual members of the consortium. (each school district would need to evaluate and pay for additional services separately). Internet Connection Internet Access Service Level Agreement Minimum 100mb Up to 750mb (see increments page) Include offer of services (if applicable) Minimum Connection: 100mb Connection to the internet at the DMARC location. DMARC location Oakdale Public School N Sooner Edmond, OK Minimum throughput: 100 Megabits per second (Mbps) download and 100 Mbps upload or greater, with the option to increase bandwidth up to 750mb within the contract term. Data caps: Connections with a limit on data transmitted or received (data cap) will not be considered. Public IPs/NAT: Range of at least ten public IP address translated to internal servers. * This bid may be used as an alternative for SPIN change or service substitutions, if needed. USAC does not allow school districts to receive discounts on redundant services.
