1 Davenport University awards scholarships funded by private donations to the Davenport University Foundation. These scholarships are awarded based on which campus location you attend and/or special requirements established by the donors. Correct use of this application allows you to be considered for any Davenport University Foundation scholarship, provided that you meet the criteria. If you are eligible to receive a scholarship from the Davenport University Foundation, you will be notified of your award by the Financial Aid Office. DIRECTIONS FOR COMPLETING THIS APPLICATION: 1. Type or print clearly in ink. Completing this application does not guarantee scholarship funds. 2. Read each question carefully and complete all areas of this application. 3. You must complete a FAFSA (Free Application for Federal Student Aid) if you have not done so already. 4. Applications will be accepted throughout the academic year; however, scholarships are awarded on a first-come, first-served basis to students enrolled half-time or more. Contiuning students must have a minimum CGPA of 3.0. Primary consideration will be given to earliest applicants meeting the scholarship criteria. 5. 3 ACCEPTANCE OF SCHOLARSHIP AGREEMENT I understand and agree that if selected as a Foundation scholarship recipient, I will communicate with my donor, remain enrolled at least half-time for the academic year, attend the Foundation Scholarship Dinner (if requested to do so), maintain satisfactory status based on the Davenport University standards of progress, and have my photo taken for use in scholarship publications and the website. Withdrawal from a class may result in a decrease in my award, so I understand that I should check with Financial Aid before withdrawing. I certify that the above information is true and complete. 4 SCHOLARSHIPS REQUIRING AN ESSAY (Deadline for submission is July 1) Matthew Brian Feko Memorial Scholarship Eligible Campus: All If you demonstrates pasion and enthusiasm for your field of study, and you have no involvement in any incidence related to drinking and driving, you may be eligible for the Matthew Brian Feko Memorial Scholarship. Please attach your typed essay on the following topic, 2-3 pages in length, to your application. Describe why you selected you area of study, some challenges that you expect to face in that industry, and what you hope to contribute to your field upon graduation. Thomas Fisher Reed Memorial Scholarship Eligible Campus: All If you are pursuing an associate's or bachelor's degree, are a sophomore, and have maintained a minimum GPA of 3.0, you may be eligible for the Thomas Fisher Reed Memorial Scholarship. Please attach your typed essay on the following topic, at least one-page in length to your application. What actions would you take if you knew you had a terminal health condition? Senator Glenn D. Steil Endowed Scholarship Eligible Campus: Grand Rapids If you attend the Grand Rapids campus and would like to be considered for the Senator Glenn D. Steil Scholarship, please complete the following 1000 word essay on an additional sheet of paper and attach it to your application to be considered for this scholarship. Your essay must be typed. As a young Republican, how would you develop strategies for change in the West Michigan community? Dearborn Alumni Club Endowed Scholarship Eligible Campus: Dearborn If you have at least a 3.0 GPA, you may be eligible to receive the Dearborn Alumni Club Endowed Scholarship. Please answer the following essay question on an additional sheet of paper and attach it to your application to be considered for this scholarship. Your essay must be typed and at least 300 words. Why should you receive this scholarship and how would it benefit you? Davenport University Northern Michigan Staff Annual Scholarship Eligible Campus: Gaylord, Traverse City If you are a returning Davenport student, earned a minimum GPA of 3.2, and have enrolled for at least 9 credit hours, you may be eligible for the Davenport University Northern Michigan Staff Annual Scholarship. Please attach the following essay to your application. The essay must be typed, double-spaced, and words. Describe how you will use your Davenport University education to Make a living. Make a life. SM C.F.M.A. Scholarship Eligible Campus: Bay City, Midland, Saginaw If you are a junior or senior and have maintained a 3.5+ GPA, you may be eligible to receive the C.F.M.A. Scholarship. 5 SCHOLARSHIPS REQUIRING AN ESSAY (Deadline for submission is July 1) Mary Walsh Kole Memorial Nursing Scholarship Eligible Campus: Midland If you are a nursing major at the Midland campus, are enrolled in clinical courses (Level I or Level II), have satisfactory performance during the fall semester within the nursing sequence, have satisfactory standing in clinical rotation, and have an overall GPA of at least 2.5, you may be eligible to receive the Mary Walsh Kole Memorial Nursing Scholarship. Please attach the following essay to your application to be considered for this scholarship. Your essay must be typed and double-spaced. Remember to attach a reference letter from an extended care facility or nursing home if currently or previously employed. Note: BSN students and students on probation will not be considered. Alfred and Eleanor Dinsmore Nursing Scholarship Eligible Campus: Midland If you are a nursing major at the Midland campus, are enrolled in clinical courses (Level I or Level II) in fall 2006, have satisfactory performance during the fall semester within the nursing sequence, have satisfactory standing in clinical rotation, have an overall GPA of at least 2.5, and commit to working in the Tri-City area for a period of two (2) years, you may be eligible to receive the Alfred and Eleanor Dinsmore Nursing Scholarship that requires an essay and list of references as part of the application process. Please complete the following essay on an additional sheet of paper and attach it to your application to be considered for this scholarship. Your essay must be typed and 100 words or less in length. Remember to complete your list of references below. Note: BSN students and students on probation will not be considered. Students must prepare a one-page double spaced appeal letter as to why they are in need of the support applied for. ABWA Indinois Chapter Scholarship Eligible Campus: Merrillville If you are a female, have a 3.0+ GPA, have earned 30+ credit hours, and have demonstrated leadership and professionalism, you may be eligible to receive the ABWA Indinois Chapter Scholarship. Please complete the following essay on an additional sheet of paper and attach it to your application to be considered for this scholarship. Your essay must be typed, double spaced, and one to two pages in length. Describe your academic and personal achievements, the challenges you face in obtaining an education, and the way(s) you would use this scholarship to help you reach your professional goals. 5

6 SCHOLARSHIPS REQUIRING AN ESSAY (Deadline for submission is July 1) Susan Margaret Kovach Cultural Diversity Scholarship Eligible Campus: All If you are involved in diversity or service learning in a study abroad or local experience, you may be eligible to receive the Susan Margaret Kovach Cultural Diversity Scholarship. Please attach your typed essay on the following topic, no more than one-page in length to your application. Describe how this scholarship would be used for diversity or service learning in a study abroad or local experience. 6
