3 Solution Overview Summary of Application/Product Unity-integrated patient engagement platform. Dragonfly provides physician prescribed patient management plans for high cost chronic diseases. Physicians receive timely notifications when patients are not healthy. Summary of Client/Patient benefits from using Product Dragonfly allows physicians to effectively supervise health measures of chronic disease populations. The patient mobile application guides patients in the self-management of their chronic disease Detailed Description Dragonfly naturally forms a partnership between the patient and the physician through every day technology. Physicians order Care Plans using standard orders processing from right inside the EHR. Patients are reminded to record health measures on their smart phones. The measures are then submitted to the patient s chart within the EHR. 3
4 Solution Overview Detailed Description (cont.) Alerts are generated for the care team in the event that patients reach critical thresholds. Notifications are also generated if the patient is non-compliant. The goal of this application is allow the care team to provide timely interventions to keep patients out of the ER. Dragonfly Care Plans are pre-loaded with Evidence Based Medicine goal and alert measurements as default values. Physicians can adjust these levels when ordering Care Plans, thereby tailoring Care Plan Goals and Alert thresholds for each patient. The following Dragonfly Care Plans are currently available: Congestive Heart Failure (CHF) Chronic Obstructive Pulmonary Disease (COPD) Diabetes Type I & Type II Hypertension Obesity Asthma Organizations may build customized plans 4
5 Solution Overview The application developed addresses the following categories: Applications that improve management of high cost chronic diseases Innovative approaches to addressing value-based care imperatives. Healthcare/Business problem addressed: Patients and families engage in a partnership with their physician on the journey to effective disease management. The mobile app guides patients to adopt new self-management habits through automated reminders and the ability to submit information to their physician s EHR. Providers are able to comply with fee for performance initiatives by monitoring patient reported values and proactively providing interventions to stabilize patient measures; ultimately preventing future disease complications, ER visits, and hospital admissions. 5
