To simplify the process of applying for admission, 51 of the State University s 64 colleges (see page A8) use this application. You may apply to up to four colleges at once. Please mail the completed application and fee to the Application Processing Center (APC) at: Application Processing Center The State University of New York P. O. Box Albany, NY Failure to include the application fee may delay processing of your application. The APC will prepare one application for each of the colleges to which you are applying and mail it to the college along with your transcript. Once your application is forwarded to the college, the APC will notify you that your application has been processed. This letter is your receipt for any application fees submitted. Individual colleges may require additional application materials, such as supplemental forms, letters of recommendation, and mid-year grades. These materials should be sent directly to the college requesting them. Transcripts If you are applying to the University as a first-time freshman, please send one transcript per college choice to the Application Processing Center along with your application fee ($40 per college choice) and completed application. If your transcripts are not available at the time you submit your application, please have your transcripts forwarded directly to each college choice to which you applied. If you are applying to the university as a transfer student, please note that academic transcripts are required for all colleges you attended or from which you received credit. Please send a copy of all transcripts directly to each college choice to which you a p p l i e d. If you are transferring from another SUNY school, please request a General Education Transcript Addendum be sent with your official transcript. Notes for Freshman Applicants For admission purposes, you are a freshman if you have not taken college-level work since high school graduation. If you have taken college-level credits during high school you are also considered a freshman. Freshmen may also include students who have received the General Equivalency Diploma (GED) and adult learners entering higher education for the first time. While there is no formal application deadline for freshman applicants, we recommend you apply by December 1 to maximize your chances for financial aid and campus housing. Please have your counselor complete the bottom section of the last page of the application. Notes for Transfer Applicants The State University of New York considers you a transfer student if you have taken at least three credits of college-level work since high school graduation. (College work taken before high school graduation does not apply.) Academic transcripts are required for all colleges you have attended. Please be sure to provide academic transcripts of all previous college work, even when not relevant to current studies. Failure to do so could jeopardize your application. If you have earned less than the equivalent of one full year of study (generally, credit hours), you should arrange for your former high school to send transcripts directly to the colleges to which you are applying. Notes for International Students If you are an international student, you may apply to the State University using this application. When doing so, be sure to supply us with the Non-U.S. Citizen information requested in questions of the application. Applicants requiring an F-1 visa must provide a permanent address outside the U.S. on question 5. Also, please be aware that you will subsequently receive requests for additional information directly from the campuses to which you apply. Financial Aid To apply for financial aid, you must first complete the Free Application for Federal Student Aid (FAFSA) form as soon after January 1 as possible. The form is available from high school guidance offices or campus financial aid offices and on the web at Requests for financial aid do not affect admission decisions. See page A11 for more information. Notes for High School Counselors and College Advisors Please complete the academic information requested in the box at the bottom of the last page of the application. Please enter only one rank and average and provide a transcript for each college choice. Three campuses (Binghamton, University at Buffalo, and ESF) require that SAT or ACT scores be sent directly by the testing agency. Supplemental materials requested by campuses, such as letters of recommendation and mid-year grades, should be forwarded directly to the admissions office of the requesting campus. A2

2 STEP-BY-STEP GUIDE TO THE APPLICATION I n s t ructions for certain questions on the application are given below, with numbers corresponding to the number of the question on the application. Please read care f u l l y, follow all directions, and type or print all inform a t i o n except your signature. 2 Applicant Social Security Number Your Social Security Number is used to coordinate the collection of information for all your student records. Authority to collect the Social Security Number is granted under Section 355 of the New York Education Law. Your response, however, is voluntary. Please double check the number for accuracy. 9a 9c Residency These questions concern your residency for tuition purposes as well as eligibility for the Educational Opportunity Program (EOP). At the State-operated campuses (University Centers, University Colleges and Technology Colleges), students are generally considered New York State residents if they have established their domicile in New York State for at least twelve months prior to the last day of the registration period. Generally, individuals who have maintained their domicile in New York for a period of less than twelve months prior to the end of registration are presumed to be out-of-state residents and are not eligible for the resident tuition rate. Individuals who are financially dependent and whose custodial parent(s) live in a State other than New York are generally not eligible for the resident tuition rate. Individuals do not meet the twelve-month residency requirement if domiciled in New York State for the sole purpose of attending college. Generally, only U.S. citizens (question 7) and permanent residents are eligible for the New York State tuition rate (see page A11). Dependent students of divorced or legally separated parents may acquire a New York State domicile if the custodial parent is a New York State resident or if the student resides with a non-custodial parent who is a New York State resident and the student intends to continue to reside with that parent throughout their attendance at SUNY. Please direct your questions concerning state residency to the campus offices of student accounts. 10 Name and Address of Parent or Guardian If your parents share custody and you would like both parents to receive official correspondence, you should list two names in the space provided. Please list the address of the second parent here. 11 Alumni in the Immediate Family Information gathered from this question does not affect admissions decisions. 12 Family Income Range Indicate the letter or number corresponding to your family s gross income, as reported on the federal income tax form. If your parents are separated or divorced, you should report the income of the household in which you reside for most of the year. If your situation is described by the letters S, T, or W above, enter one of those codes rather than a letter or number indicating an income range. Please note: Your answers here will not impact financial aid decisions. However, if you wish to be considered as an EOP applicant, you must answer questions 9a, 9b, 12 and 13. In the event you choose to answer these questions, the State University of New York will keep your responses confidential and will not use the information provided in a discriminatory manner. The failure to respond to these questions will not subject you to any adverse treatment. The State University seeks to enroll students of diverse racial and ethnic heritage and students with disabilities. 19a 19b Felony/Dismissal Information A felony in New York State law is defined as a crime for which more than one year in prison may be imposed. The felony question applies if you have been convicted as an adult. If you have been adjudicated as having juvenile delinquent or youthful offender status, you are required to respond to the felony question 19a by indicating a response of no. Suspension is defined as a sanction imposed for disciplinary reasons that results in a student leaving school for a fixed time period, less than permanently. Dismissal from a college for disciplinary reasons is defined as permanent separation from an institution of higher education on the basis of conduct or behavior. An affirmative response to either question will not automatically prevent admission, but you will be asked by your college choices to provide additional information. This information will be reviewed by a campus committee to ensure campus safety. If you are a transfer student seeking the fee waiver, you must complete questions (continued on page A4) A3

For private colleges in New York, enter NY in the two spaces provided. For out-of-state colleges, enter the respective state code. For Puerto Rico enter PR. Please consult with your college advisor to determine if you qualify and to which campuses you should apply. You will be asked to complete additional application materials and financial aid forms by your college choices. Be sure to meet all deadlines. For a full program description and eligibility criteria, see page A Early Decision Program The Early Decision Program is for fall freshmen and can be used for one college choice only. The application includes a request for a parent and counselor signature in addition to the student s signature indicating an understanding of the Early Decision commitment and agreement to abide by its terms. Participants in the Early Decision Program are Brockport, Buffalo State College, University at Buffalo, College of Ceramics, Cortland, Environmental Science & Forestry, Fredonia, Geneseo, Maritime, Old Westbury, Oneonta, Oswego, Plattsburgh, Potsdam, and Purchase (except for fine and applied arts programs), and SUNYIT-Utica/Rome. Please note that you cannot apply for Early Decision and EOP at the same time. To apply for Early Decision, your application must arrive by November 1. The campus will notify you of its decision by December 15. If you are accepted Early Decision, you will be asked to make a tuition deposit by January 15 and withdraw your applications at all other campuses. Note: Early Action Early action is a non-binding early application/notification program for fall freshmen available only at PAGE A8 OF THE APPLICATION You may apply to as many as four State University college choices on this application. Use additional forms for additional choices. Please complete all information for each college choice. Make checks payable to SUNY APC at $40 per college choice (i.e., $80 for two choices, $120 for three, $160 for four, etc). You may also pay by MasterCard or Visa. Campus Code Whenever possible, spell out the name of the college and avoid abbreviations. Please be sure to enter the right code for campuses which share similar names, such as Buffalo State College (Code 33) and the University at Buffalo (Code 20); Alfred State College (Code 91) and the College of Ceramics at Alfred University (Code 30); Genesee Community College (Code 81) and the College at Geneseo (Code 36). Curriculum Code Please refer to the curriculum index at the back of the A p p l i c a t i o n Viewbook and specify a curriculum code for each college choice. You Albany, Binghamton, Stony Brook, New Paltz and F.I.T. If you are accepted Early Action, you need not withdraw your applications at other institutions until May 1. To apply Early Action, your application should reach us by November 15. The campuses will notify you of their decisions by January 1. If accepted Early Action, you will have until May 1 to finalize your enrollment decision. Please indicate your desire to apply Early Action where indicated in the Choosing Colleges section on page A8. may also indicate that you do not wish to declare a major. Please note that the health science centers and most community colleges, including F.I.T., require that you declare a major. Early Action Please check here if you wish to apply Early Action at Albany, Binghamton, Stony Brook, New Paltz or F.I.T. (see note above). Please note that you cannot apply for Early Action and EOP at the same time. Special Campus Project A campus may request that you use a local code in this space. If you have completed one or more tech prep courses, please insert TPREP in this space. Joint Admissions Joint admissions programs guarantee SUNY students graduating from two-year colleges admission to parallel programs in four-year colleges, subject to certain requirements. Please enter the code of the four-year college to which you intend to transfer. A4

4 CURRICULUM SELECTION An online SUNY CURRICULUM FINDER is available at Choosing a Program For each college to which you apply, you may select a specific program of study or indicate that you do not wish to select one at this time. Please note that the Health Science Centers and most community colleges, including FIT, require that you declare a major. Some programs have limited space or require portfolio review, auditions, or special academic preparation. If you are interested in a program you think may fall into this category, please contact the campus directly. Using the Curriculum Index The Curriculum Index in the Application Viewbook lists programs of study at SUNY s colleges and universities. You may use the Index to choose a program of study at each college of your choice. Choose only one program for each college (except Purchase and HSC Stony Brook; see note on page 76). If you later want to change your program, contact the director of admissions at that college. Special Program Considerations Jointly Registered Programs (Teacher Education) A number of State University community colleges have developed teacher education programs in conjunction with State University four-year colleges. Jointly registered programs are so noted in the Curriculum Index. Purchase College offers jointly registered programs in Childhood, Early Childhood, Middle Childhood, and Adolescent Education with Manhattanville College. Cooperative Programs Many cooperative programs exist between State University colleges and other public and private colleges. In the Curriculum Index, cooperative programs are listed with the number of years of study at the first campus plus the number of years of study at the second campus (i.e., 1+1, 2+3, 3+3). For 1+1 programs, only the campus offering the first year of study is identified; contact specific colleges for more information. Joint Admissions Program Freshmen may apply for Joint Admissions, a cooperative program between two-year and four-year SUNY colleges. The program guarantees junior-level admission in a parallel program at a fouryear college after completing an associate degree program at the two-year college. When applying for Joint Admissions, enter the college code of the four-year college where you plan to continue your studies, as indicated on page A8 of the application. Health Science Centers The Health Science Centers at Brooklyn, Stony Brook, and Syracuse offer upper-division programs. Admission is by program and is generally limited to those who have completed specific requirements. Supplemental application forms are required. Contact admissions offices directly for details. ESF Guaranteed Transfer Program High school students may apply for admission through the Environmental Science & Forestry Guaranteed Transfer Admission Program. Students accepted into this program will enroll at any college of their choice and complete prerequisite courses. See the entries for Pre-Environmental Science and Forestry (0202) on pages 79 and 89 of the Curriculum Index for a list of transfer colleges. Transfer colleges are listed under Forest Technology with ESF 1+1 (0620) on page 91 of the Curriculum Index. Students may also obtain the AAS degree in Forest Technology through a unique 1+1 program at the NYS Ranger School at Wanakena. The Office of Admissions at ESF can supply details. Occupational Therapy The Health Science Center at Stony Brook offers Occupational Therapy as an entry-level master s degree program. The multi-award B.S./M.S. degree program is available to upper division transfers with a minimum of 57 credits. Pharmacy Pharmacy at the University at Buffalo is a six-year PharmD program. Freshmen, transfers, baccalaureate, and advanced degree holders may all apply using this application. Applicants who hold a B.S. in Pharmacy should call the School of Pharmacy at Physician Assistant The Health Science Center at Stony Brook offers a Physician Assistant bachelor s degree program. You cannot apply to this program using the enclosed application. Please apply directly at Physical Therapy Physical Therapy is offered as master s and doctoral level programs. The Health Science Center at Stony Brook offers an entrylevel Doctor of Physical Therapy (D.P.T.) program for applicants with a baccalaureate degree. For information call The Health Science Center at Brooklyn offers a joint B.S./M.S. degree program to upper division transfer students. The Health Science Center at Syracuse offers a joint BS/MPT degree program for upper division transfer students. While a specific undergraduate major is not required, a lengthy list of prerequisite courses is required. Please select curriculum code 0294 to apply. The University at Buffalo recommends Exercise Science (Code 1226) as the undergraduate major and asks interested students to apply to the six-year professional doctor of physical therapy (D.P.T.) program in their junior year (for more information, call , ext. 408). State University Transfer Guarantee An opportunity to continue fulltime study at a four-year State University college is guaranteed to all NY residents who transfer directly from a SUNY or CUNY twoyear college with an AA or an AS degree. To be eligible, you must: File your application with the Application Processing Center by March 1 for fall admission, and by Oct. 1 for spring admission. Provide four-year campuses with an official two-year college transcript, showing three semesters of completed studies, by March 15 for fall admission, and by Oct. 15 for spring admission. Complete all required supplemental application materials by April 15 for fall admission, and by Nov. 15 for spring admission. The transfer guarantee becomes effective if you are denied admission at all of your four-year college choices. Remember, all applications on file at baccalaureate campuses must be complete. A9

5 EDUCATIONAL OPPORTUNITY PROGRAM (EOP) Note: You cannot apply for both the EOP and Early Decision/Early Action programs. The State University of New York s Educational Opportunity Program (EOP) provides academic support and financial aid to students who show promise for mastering collegelevel work, but who may otherwise not be admitted. Offered primarily to full-time students who are New York State residents, EOP accepts students who qualify, academically and financially, for the program. Note: You cannot apply for both the EOP and Early Decision/Early Action programs. Eligibility To be eligible for admission to EOP you must be: A New York State resident for 12 months prior to enrollment; Ineligible for admission under traditional standards, but demonstrate potential for completing a college program; and In need of financial assistance within established income guidelines. In selecting students for the program, priority is given to applicants from historically disadvantaged backgrounds. If in the year prior to enrollment your family income falls within EOP guidelines due to unemployment, separation, divorce or death of a parent, etc., you may still not qualify for EOP if you are not from an historically disadvantaged background. Required Records As part of the EOP admission process, your high school record and test scores (if needed) will be reviewed, as well as information about clubs, hobbies, jobs and any other experiences which made an impact on your education. This information will help college counselors evaluate your potential for success in a college program. Support Services When you enroll as an EOP s t u d e n t, you receive special tutoring and academic, career, and personal counseling. In addition, you receive financial aid to help with the cost of room, board, tuition, books and personal expenses. Fee Waiver The $40 application fee is waived for those eligible for admission to EOP. If you think you are eligible, check yes on Question 54. If you are found to be ineligible, you will be billed for the fee. Note: Applicants who are deemed ineligible for EOP may still qualify for admission and for financial aid as traditional candidates. Contact the admissions or EOP office at your college choice before you apply. Note: The following campuses do not offer EOP programs: Adirondack, Cayuga County, Clinton, Columbia-Greene, Herkimer County, Jefferson, Mohawk Valley, Nassau, Niagara County, Sullivan County and Tompkins Cortland Community Colleges. Transfers You are eligible for EOP as a transfer student provided you have been enrolled in EOP, College Discovery, HEOP (Higher Education Opportunity Program), SEEK (Search for Education, Elevation and Knowledge) or a similar program at your previous college. If your previous college did not have EOP or a similar program, you may be considered for transfer eligibility and should notify the college to which you are transferring. Income Guidelines The table below lists financial eligibility requirements for EOP admission during the academic year. To be economically disadvantaged, a student must belong to a household that is defined in Category A, B, C, or D or the exceptions noted. Household Categories A Supported by one individual who is part of a couple (i.e. parents of student(s) or student and spouse) or by one or more individuals whose income is from Social Security or sources other than employment and which does not exceed the applicable amount under Category A below. B Supported by more than one worker whose income does not exceed the applicable amount under Categories B and C below. C Supported by either one worker (yourself) or a single parent worker whose total annual income does not exceed the applicable amount under Categories B and C below. 6 COSTS AND FINANCIAL AID You can obtain general financial aid information, and information about your own financial aid application by calling Federal Financial Aid Application Status at or New York State TAP and Loan Application Status at How Financial Aid Works Financial aid is available at all State University campuses to assist students who are unable to pay college costs. Eligibility for financial aid is based on the amount which you and your family are expected to contribute toward your college expenses. This amount is determined by analyzing your family s financial circumstance, including such factors as income, assets, and number of dependents in college. Each State University college uses the federal calculation to determine your financial need and eligibility for assistance. State University campuses participate in a variety of state, federal, and private financial aid programs. Assistance is available for full-time and part-time students based on financial need and availability of funds. If eligible, you may be offered aid for tuition and other college expenses from programs such as education loans, Federal Pell Grants, New York State Tuition Assistance Program (TAP), institutional grants and scholarships, and others. Read this information on how to apply. You or your family may also qualify for certain tax benefits associated with college costs. You may wish to get professional tax advice. Tuition, Fees and Other Costs Costs vary depending on the college, its location, full-time or part-time enrollment, program of study, housing accommodations, personal expenses, and other individual factors. State University colleges have higher tuition rates for students who are not New York State residents (see chart below). Contact the Office for Student Accounts at your college choices for details about residency requirements. Technology colleges charge a reduced non-resident tuition rate of $5,000 per year for the associate degree and the standard nonresident rate of $8,300 for the bachelor degree. Community college tuition ranges from $2,200-$2,600. The sample budgets below can be used to estimate costs for the academic year. Students who enroll in a community college not in their home county, must provide the college with a certificate of county residency to benefit from the lower, in-district tuition rate. For students taking fewer than 12 semester hours (part-time study), New York State resident tuition is $137 per credit hour for baccalaureate degree programs and $128 per credit hour for associate degree programs. Most State University campuses have time-payment plans to enable you to pay in monthly installments. Campus business offices can provide you with information on payment options. How New Students Apply for Financial Aid 1. Read each college s catalog and financial aid literature and contact the college financial aid office if you have specific questions. This is the best source of financial aid information. 2. Apply for financial aid: Check with the college to see which forms to use. All students must complete the Free Application for Federal Student Aid (FAFSA). If you have Internet access, you can file the FAFSA online at FAFSA applicants who are New York State residents and who plan to attend a college in New York will automatically be sent an Express TAP application. Complete the FAFSA after January 1, 2003, even if you have not yet filed your 2002 tax returns. Use estimates if necessary. If you are unable to file on the web, you can obtain the FAFSA application from your high school guidance office. Be sure to meet all deadlines. 3. Investigate other sources of aid. Several free scholarship search services are available on the Internet. 4. Keep a file of records and copies of applications. How Transfer Students Apply for Financial Aid 1. If you did not submit a FAFSA for the current academic year, you should follow the application instructions for new students. 2. If you did apply for financial aid this year, you will automatically receive a Renewal FAFSA from the U.S. Department of Education for next year. 3. You can complete the Renewal FAFSA on the web at (You will need the pin number that was sent to you with the results of your application.) Or you can complete the paper Renewal FAFSA. 4. To apply for financial aid at the college you are transferring to, use the college code for the new campus on the Renewal FAFSA. 7 UNDERGRADUATE APPLICATION AND GENERAL INFORMATION An online undergraduate application is available at Application Fee The non-refundable application fee is $40 for each college choice. Please make checks payable to SUNY APC. International applicants must pay in U.S. dollars drawn on a U.S. bank. If you apply to more than four campuses using additional application forms, you may pay with one check. To pay by MasterCard or Visa, please complete the credit card information on the application. Applicants to the College of Arts at Purchase College and to the Health Science Center at Stony Brook may apply to more than one program of study but must pay a separate $40 fee for each curriculum choice. Both freshman and transfer applicants to the Educational Opportunity Program (EOP) may waive the fee. If you are ineligible, or if your college choice does not offer EOP, you will be billed for the fee. The application fee is for processing services; it is nonrefundable and independent of an admission decision or a decision to withdraw or decline an offer of acceptance. When you submit an application, you assume responsibility for prompt payment of all fees due. Returned checks will be subject to an additional processing fee of $15. All appeals for refunds must be put in writing to the Director of the Application Processing Center. SUNY Application Status Checking System To check the status of your application online, visit student/apply/apcstatus.cfm and key in your social security number and date of birth. The system will verify that your application was received at APC and confirm your campus and program choice(s). You will also be provided with specific contact information should you have questions or need additional details. Please note that this system does not provide admission decisions. For this information contact the campus admissions office directly. Admission Decisions Admission decisions are made at the college, based on individual evaluations of your academic record. Each college makes an independent decision and is not informed of your other choices or the order in which you listed them on your application. The APC does not make admission decisions. Supplemental Applications Many campuses are interested in recommendations or other supporting credentials. In particular, most of the four-year colleges and universities require completion of a supplemental application which they provide. These may be sent directly to the appropriate campus or to the APC. Tuition Deposit Tuition deposits must be postmarked by May 1. You should make a tuition deposit at only one SUNY college. This deposit is required by May 1 for the summer or fall terms, by November 1 for winter or spring terms, or 30 days after acceptance, whichever is later. In most cases, deposits are refundable until May 1. For information, call your college choices directly.
