3 1. Background The Federation Handicap International (HI) is an independent international aid organisation working in situations of poverty and exclusion, conflict and disaster. Working alongside persons with disabilities and other vulnerable groups, our action and testimony are focused on responding to their essential needs, improving their living conditions and promoting respect for their dignity and fundamental rights. HI promotes an inclusive approach by focusing on access to services for all whenever possible rather than developing specific actions for people with disabilities. Handicap International has been working in China since 1998.The programme has been operating in Yunnan since 2011, in cooperation with its partner, the Yunnan Disabled Persons Federation (YDPF) and its branches at Shizong and Chengjiang counties, through the set-up of the Community Based Rehabilitation (CBR) projects. HI with collaboration with Yunnan Rehab Center (as affiliated organization of YDPF), a rehabilitation project started in January 2014, for the duration of 3 years. The project specific objective stipulates that: The quality of and access to functional rehabilitation services in Yunnan is improved, addressing the needs of people with disabilities. This assignment aims capacity building of rehabilitation professionals (OT, PT. mid-level therapist and CBR workers) and management staff to facilitate Multi-Disciplinary Team (MDT) coordinated service and improved quality rehabilitation services of the Yunnan Rehabilitation Centre (YRC). The YRC invited children with disabilities and their caregiver for 1.5 periods of time to receive appropriate rehabilitation services. Most of the clients came at chronic stage and 90% of them are children with cerebral palsy below 7 years old. Currently children are coming for a 3 months session at the YRC with the aims of parents education and provision of comprehensive rehab service for people living in remote areas. The CP department has different units respectively on Physiotherapy, Occupational Therapy, Conductive Education, Cognitive Therapy, Hydrotherapy, Speech and Language Therapy, Electro-magnetic Feedback room, Nursing Station, Sensory Integration and a space for Botox injection, etc. Currently hydrotherapy therapy unit is not functional, though it is equipped.rehab professionals are more comfortable with passive exercise and joint compression, which are often painful. The development milestones are not well respected in therapy programme, for instance; practicing sitting when a child didn t achieve rolling. There are enough toys and functional tools but therapists seldom use these items rather use electric stimulation and passive exercise. Nevertheless, there is much scope to improve the effectiveness of stretching/passive exercise. 2. At a glance the functional rehabilitation project Title: Strengthening of quality rehabilitation services in the Yunnan Province -2014/2016 Overall objective: People with disabilities improve their quality of life and their social participation. 4 Estimated results: 2 local Community Based Rehab (CBR) centre and their CBR workers Rehab professionals/technical expert at provincial and national level institutions 300 children with Cerebral Palsy (CP) People with disabilities are identified from the community level (Shizong and Chengjiang) and referred to the functional service 1. Health and rehabilitation professionals in Yunnan improve their competences and abilities to provide appropriate services to People with disabilities and access to the sustainable technical support resources 2. Access to and quality of functional rehabilitation (mainstream, specific and/or support services) is improved in Yunnan 3. A support system is organized at community level in Yunnan 4. The experience and tools developed during the project implementation are consolidated, shared, and adaptable to new contexts Yunnan Province, China Project Area: Local Partners: Yunnan Disabled Peoples Federation (YDPF) and its Rehabilitation Unit (Yunnan Rehabilitation Centre - YRC) Financial The Belgian Development Cooperation Support: Duration: 36 months (January 2014 December 2016) 3. Description of the mission 3.1 Objective of the mission To equip project rehabilitation professionals to offer comprehensive functional rehabilitation services for children with disabilities. With the specific activities, a) In-house training and on the job support to rehab professional at the YRC b) Organize sensitization session for management staff of YRC to ensure better coordination, monitoring, improve referral mechanism and quality control c) Organize one week (3 days + 2 days) training for CBR workers on management of children with CP at community settings. Trained therapist from YRC will play co-facilitator role, part of their capacity building. 3.2 Detail activities Training for rehabilitation professionals Profile of rehabilitation professionals: Graduate OT + PT and mid-level therapist, some of them are fresh and some of them have 3-4 years of experience. Total Participants: 8-10 Training methods: Theory, demonstration and on the job training. Page 4 of 9
5 - Organize at least eight half a day theoretical training over the two months duration. If you can negotiate and agree with YRC for more theoretical training session, will be appreciated. - On the job support to individual therapist during specific client management. - Organize group demonstration session. Objective of the training: By the end of the two months training and on the job support, participants will be able to - Demonstrate confidence to provide functional rehabilitation support to children with CP following principles of PCA - Explain the role of rehab professional to maximize functional independence and social inclusion of children - Describe the importance of multi-disciplinary team support according to the individual need of the children and parents, ensuring service protocol and service quality criteria. - Explain how to bridge between institutional rehab service and social inclusion through interacting with CBR workers, using parents as co-therapist, appropriate referral and follow-up mechanism. Key content: Rehab cycle (Initial assessment/mdt assessment, rehab planning, implementation, monitoring, follow-up, evaluation), role of parents as a cotherapist, how to facilitate child development (importance of following sequence of milestone, and use of purposeful activity), management of abnormal tone, positioning, play therapy, deformities and contracture prevention, how to teach new skill to children with disabilities, feeding management, ADL training, therapeutic exercise, referral mechanism, back care of paediatric rehab professionals, etc. Handicap International promoting Persons Centered Approach (PCA) in this current project and some of the therapist and head of YRC are oriented on it. This mission will strengthen previous training on PCA, thus all training materials, tools and recommendation should be well respected. The training will also specially focused how rehab professionals could engage parents in the rehab process and build their capacities. HI guideline on PCA is available for reading/use Workshop with management team Profile of management team: Medical doctor, the director of YRC,person in charge of nursing/pt/ot, CBR centre manager Participants: 5-7. Methods: - Organize two half a day workshop - Follow-up/preparatory one-to-one meeting for follow-up and awareness workshop contents and how to operationalize service protocol and quality criteria in service provision, which is developed under the project. 6 Objective of the training: By the end of two aware workshop sessions, participation will be able to - Explain the importance of Multi-Disciplinary Team work and how they can promote it in YRC, respecting service protocol and service quality criteria. - Explain the social model of disability and importance of PCA in rehabilitation process to promote functional independency and social inclusion of children with disabilities - Identify list of recommendation to promote functional rehabilitation service of YRC, especially on MDT, PCA, referral, social inclusion, etc. Key content: - Working protocol of CP department should be in place - Importance of MDT approach and how to make it functional, followed by interactive discussion/game on (re)identify the role of each professionals in the rehab process, scope of strengthening MDT approach in YRC. - Importance of using PCA approach in rehabilitation process - Importance of referral for specific and mainstream service along with followup and how to promote it - Social model of disability and how strengthen linkage between YRC and CBR centre. How to strengthen referral mechanism for specific rehabilitation/ medical need along with mainstreaming services Training for CBR workers Profile of the CBR workers: CBR workers are the local employees of YDPF, most of the CBR workers have basic training disability & community based rehabilitation. Methods: - In-house training with practical demonstration (2+2 = 4 days) - One day field visit Expected result: By the end of the two months training and on the job support, participants will be able to - Demonstrate confidence to provide basic functional rehabilitation service aiming to promote functional independence and social inclusion of children with disabilities, following principle of PCA. - Explain how to promote appropriate and successful referral mechanism to access specific rehab/medical services and mainstream services (e.g., schooling, vocational training, social service, etc.) - Explain how to build capacity of parents and engage them in rehabilitation and inclusion process. - Importance and scope of awareness rising for social inclusion. Key content: Simplified version of the same training for rehab professionals, Rehab cycle (Initial assessment, rehab planning, implementation, monitoring, follow-up, evaluation), child development, how to handle children with abnormal Page 6 of 9
7 tone, positioning, play therapy, deformities and contracture prevention, training new skill to children with intellectual disability, feeding management, ADL training, referral mechanism, caregiver training, social inclusion, user training of assistive device, simple home modification, etc. 3.3 Deliverables - Mission report - 2 training modules, respectively for rehab professional and CBR workers (Training module on CP which will documented for the YRC further use) - 2 training reports: including training needs assessment report, pre-post training report, observation, follow-up action plan - Workshop report and recommendations with management staff 3.4 Suggested methods and activities - Use first week for observation and rapport building with team members. - Organize one half a day theoretical training session per week, for rehab professionals - As there will be limited theoretical session, thus some of content should be covered by demonstration and on the job training - Organize 2 days + 2 days training session for CBR workers and a one day follow-up at the field (If necessary, more field visit could be organized) - Share materials and documents with HI technical department regularly for feedback e.g., share training session plan before development of training materials. In order to develop ownership and ensure the involvement and interest of the stakeholders for sustainable changes and future developments, the consultant should use participatory methods and builds on existing practice and mechanism rather develop a complete new system. 3.5 Reporting Regular debriefing/update to HI, especially after achieving each milestone of the assignment. Administrative and logistic information 4.1 Context and working condition China has fast economic growth for the last decade and it has improved in infrastructure, transportation, communication and health system. The consultant will Page 7 of 9
8 enjoy internaional standard lifestyle irrespective the concern of air and water pollution. There is internaional SoS office based in Beijing with strong relaionship with Hong Kong SoS for advance medical management or evacuation. The position will be based in Kunming, the capital city of Yunnan province at the southeast region, 3 hours flight from Beijing. Yunnan is one of poverty prone province in the South, though Kunming is well developed capital city; economy mainly depends on agriculture, tourist and tobacco production. Handicap International does not have an office base in Kunming; however, HI Project Officer (Occupational Therapist) for this project based in Kunming and YRC accommodate her at their office. There is also no HI guest house, the person has be to stay in hotel for this short mission. Project Manager is the line manager of this position with strong functionalinterface with Technical Coordinator. There will be pre-post and mid-term review briefing with PM and Technical Coordinator. However there may require more frequent meeting with TC, may be weekly Skype meeting initially. Nevertheless, the Project Officer based in Kunming will be the focal person to manage local day-to-day issues. 4.2Management and Logistics HI China will be responsible for all logistical arrangements, including travel, accommodation, etc. for field visit, training and workshop. HI staff members involved in the project will accompany the consultant in partners meeting and during field visit. The consultant will arrange for his/her own laptop and other required equipment. The consultant will bear the final responsibility for achievement of the objectives, report submission and necessary presentations. 4.3 Confidentiality All project documents, outputs, reports; information, etc. product by this assignment will be treated as the property of HI, and will remain confidential. The above mentioned outputs or any part of it cannot be sold, used or reproduced in any manner by the assigned organization / individual without prior permission from HI. 5. Profile of consultant A consultant with rehabilitation background (OT or PT) and at least 5 years of clinical experience on Cerebral Palsy. Experience adult learning principles and development of training materials Consultancy experience in the same field in China will be an added value. Experience and knowledge of HI s program will be an asset. Language: Chinese and English Personal qualities: Abilitiesto demonstrate tolerance, patience, flexibility, team-work and self-reliance is the most important for this position. 6.
