2 The information provided in this document is based on our understanding of the information that is currently available from the Centers for Medicare & Medicaid Services (CMS). This information continues to evolve and will be updated periodically as new information becomes available. Q: What is the CG-CAHPS survey? A: The Clinician & Group CAHPS survey, or CG-CAHPS, is part of a family of standardized Consumer Assessment of Healthcare Providers and Systems surveys that assess patients experience with healthcare. CAHPS is a registered trademark of the Agency for Healthcare Research and Quality (AHRQ). AHRQ funds the development and testing of CAHPS surveys. There are a few versions of the CG-CAHPS survey available, including the 12 month retrospective version, visit version and Patient-Centered Medical Home (PCMH) version. DSS is focused on offering the 12 month retrospective and PCMH versions. All versions share some core survey questions focused on topics related to Getting Timely Care, Appointments and Information, How Well Providers Communicate, Courtesy and Helpfulness of Office Staff and Test Results Follow Up. CMS recently released a version of the CAHPS survey for Accountable Care Organizations participating in Medicare Initiatives such as the Medicare Shared Savings Program (MSSP). DSS anticipates that the version used as part of Physician Quality Reporting System (PQRS) initiatives will be very similar to the ACO version. The CAHPS for ACOs Survey includes the core CG-CAHPS questions, PCMH questions, and supplemental items about Access to Specialists, Health Promotion and Education, and Shared Decision Making. Q: What programs is CMS developing that will involve information from Patient Experience CAHPS surveys? A: CMS is launching or planning to launch several survey programs involving information collected as part of a CAHPS survey process. Pioneer ACOs: This program will use the CAHPS for ACOs Survey. Pioneer ACOs will need to choose a CMS-certified vendor for the 2013 reporting period (2014 administration) and pay to have the survey conducted. Medicare Shared Savings Program (MSSP): This program will use the CAHPS for ACOs Survey. CMS will use a single survey vendor and pay for MSSPs to have the survey conducted for the 2013 reporting period (2014 administration). MSSP ACOs will need to choose a CMS-certified vendor for the 2014 reporting period (2015 administration) and pay to have the survey conducted. Physician Quality Reporting System (PQRS): This program is expected to use a survey that is very similar to the version used by ACOs as part of the MSSP program above. This program is expected to include practices with 100+ eligible providers under the same Tax Identification Number (TIN) who use the PQRS web interface Group Practice Reporting Option (GPRO). Smaller groups (i.e. 25+ providers) may also be eligible to conduct the survey. Specific timing is currently unknown. PhysicianCompare.gov: This program will involve public reporting of patient experience of care survey results. We expect that the survey version adopted as

3 part of the survey programs above will be used to collect and, eventually, publish this data. Specific timing is currently unknown. More detailed information on each of these initiatives is included later in this FAQ document. Q: What are the survey guidelines for Pioneer Accountable Care Organizations (ACOs)? For MSSP ACOs? The goals of this initiative are to improve the quality of care and reduce the cost of care for Medicare beneficiaries. CMS administered the survey on behalf of ACOs for the 2012 reporting period. For the upcoming 2013 reporting period (2014 administration), Pioneer ACOs will need to choose a CMS-certified vendor and pay to have the survey conducted. For the upcoming 2013 reporting period (2014 administration), CMS will again administer the survey on the MSSP ACO s behalf using a single vendor. MSSP ACOs will need to choose a CMS-certified vendor for the 2014 reporting period (2015 administration) and pay to have the survey conducted. Our current understanding of survey requirements is as follows: Frequency: The ACO CAHPS survey is conducted annually survey administration for the 2013 reporting period will start in January 2014 and continue through March. Eligible patients: CMS will identify beneficiaries that are eligible for the survey. They will select a random sample of original Medicare beneficiaries who received primary care services from the ACO. However, CMS will limit the sample to beneficiaries age 18 and older, who are known not to be institutionalized or deceased. Survey instrument: ACOs will field a six-month retrospective survey that includes core CG-CAHPS (Version 2.0) items plus additional supplemental items. The patient survey measures: o Access to and use of specialist care o Experience with care coordination o Patient involvement with decision-making o Experience with a health care team o Health promotion and patient education o Patient functional status o General health Reporting of results: CMS will generate results at the ACO level. In future years, CMS may allow oversampling to yield clinician-level results. CMS provides reports through their portal, portal.cms.gov. Both narrative reports and an Excel workbook of scores based on the survey data are available. For Pioneer ACOs that choose to work with us, we will also provide unofficial survey results through our online reporting portal to be used for ACO quality improvement purposes. Our goal is to help ACOs understand where they stand, and most importantly, provide clear direction for improvement.

4 Sample size: The sample size is 860 beneficiaries, but CMS may decide to adjust this number in future years. Modes of data collection: The ACO CAHPS uses a Mixed-mode survey administration approach, which consists of a pre-notification letter, two survey mailings and up to six follow-up telephone calls to non-respondents or those who have not completed the survey. Q: Is DSS Research a CMS-certified vendor for the CAHPS for ACOs Survey? A: Yes. DSS Research has been granted full approval as a CAHPS for ACOs survey vendor for the 2014 survey administration. Q: What are the expected guidelines for the Physician Quality Reporting System (PQRS) program? A: The PQRS program gives eligible providers a financial incentive to report data for a wide range of quality measures, one of which will involve reporting on group-level patient experience measures. We anticipate that CMS will administer the survey for Group Practice Reporting Option (GPRO) participants in first quarter 2014 (about 2013 patient visits) and first quarter 2015 (about 2014 patient visits). After that time, it is expected that groups will need to contract with a CMS-certified survey vendor to administer the survey on their behalf. Our current understanding of survey requirements for the PQRS program is as follows: Eligible patients: Only Medicare patients will be included in the survey. Survey instrument: Groups will field a retrospective survey that is very similar to the CAHPS for ACOs Survey used for Pioneer ACOs and MSSP ACOs. Some specialties may be excluded. Reporting of results: Results will initially be available at the group level. In future years, provider-level results may be encouraged or required. However, CMS has no definite timeline regarding provider-level reporting that we are aware of at this time. Sample size: CMS is expected to require at least 300 surveys for each group. Modes of data collection: TBD. We anticipate the preferred approach may involve Mixed-mode (mail/telephone) outreach like the CAHPS for ACOs Survey program. Q: Will patient experience results be made publicly available on Physician Compare? A: Yes, CMS plans to make results publicly available to healthcare consumers through Physician Compare (PhysicianCompare.gov). Timing is uncertain at this time. Q: Why should my organization start conducting a patient experience survey to measure patient experience? A: Given the impending CMS requirements, we recommend that health care organizations start collecting survey data as soon as possible to be well positioned when the requirements take effect. The CMS required surveys will involve one time data collection each year. However, DSS is always ready for action so your off-cycle study can be in and out of the field in a

5 hurry and give you the edge you need to get to the next level. However, keep in mind that CMS strongly encourages ACOs not to conduct other surveys of beneficiaries four weeks prior, during or four weeks after the CAHPS for ACOs Survey administration period. For off-cycle measurements, we recommend that our clients consider separately collecting data at the group and/or provider level to give more meaningful individualized feedback. Regardless of CMS requirements, administering a patient experience survey provides valuable data that can benefit your organization. Good patient experience has been shown to have a positive relationship to: Patient loyalty, engagement and compliance with medication and other care regimens Better health outcomes from clinical processes for disease management and prevention Stronger financial performance Lower malpractice risk Improved employee satisfaction Some information in this document was provided by the Centers for Medicare & Medicaid Services, Baltimore, MD.
