1 PARENT PERMISSION LETTER REGARDING INTERNET USE AGREEMENT Please read this document carefully before signing the attached permission form. (Name of the Computer Center) believes the Internet offers vast, diverse, and unique resources to youth. Access to the network and the Internet will enable students to explore thousands of on-line resources and exchange messages with users throughout the world. Students can tour the world s museums, conduct research at all the major colleges and universities, the President, or simply chat with students in other countries to practice foreign language studies and learn about other cultures. We are pleased to offer youth of the (Name of the Computer Center) access to the network resources, , and Internet. To access the Internet, all youth must obtain parental permission. An Internet Permission and Agreement Form must be signed by the youth, the parent, and the Computer Lab Instructor. Families should be aware that some material accessible via the Internet may contain items that are illegal, defamatory, inaccurate, or offensive to some people. We believe that the benefits to students from access to the Internet, in the form of information resources and opportunities for collaboration, exceed any disadvantages. Ultimately, parents and guardians of minors are responsible for setting and conveying the standards that their children should follow when using media and information resources. To that end, (Name of Computer Center) supports and respects each family s right to decide whether or not to apply for access. (Name of the Computer Center) is proud of its youth and the exemplary behavior they display when representing our installation. To ensure the same behavior is shown while students are using the network, access to network services is given to youth who agree to act in a considerate and responsible manner, following the attached guidelines. Each person who uses the Internet will receive information as to the proper use of the network by a Computer Lab Instructor before signing the attached Internet Permission and Agreement Form. The (Name of the Computer Center) will make every effort to ensure that all users comply with appropriate on-line behavior. The Computer Lab has implemented software to filter and monitor youth from accessing inappropriate or offensive Internet sites. Ultimately, however, the (Name of Computer Center) will not be held responsible for restricting, monitoring, or controlling the communication of individuals using the network.

2 INTERNET ACCEPTABLE USE POLICY 1. Inappropriate language, swearing, abusive language, or other vulgarities are strictly forbidden. 2. Using another s login ID or password without permission is prohibited. 3. Illegal activities are strictly forbidden. It is illegal to hack or gain illegal entry into other computers. 4. Use the network in such a way as to not disrupt the use of the network by others. Hardware or software shall not be destroyed, modified, or abused in any way. Use limited system resources wisely. 5. All messages must be signed by the writer of the message. Messages may not be sent anonymously. 6. Understand that privacy is not guaranteed when using . Storage areas may be reviewed by system administrators or others to ensure that users are using the system responsibly. 7. Any use of the network for product advertisement or political lobbying is prohibited. Youth may not order books, newsletters, or other products on the network unless authorized by the Computer Lab Instructor. 8. Subscriptions to ListServs must be approved by and reported to thecomputer Lab Instructor. 9. Mail ListServs will be monitored daily and deleted from the personal mail directory to avoid excessive use of fileserver hard disk space. 10. Personal addresses and phone numbers of youth are not to be revealed over the Internet. 11. Users must abide by all copyright laws. 12. The Computer Lab Instructor working with the School-Age Service Coordinator and Youth Services Director have the right to modify these policies for network and Internet usage at any time. Any changes to these policies will be posted by each computer lab. 13. The Computer Lab Instructor reserves the right to remove a user account on the network if these policies are not followed.

3 The use of networks and the Internet is a privilege, not a right. Inappropriate use will result in cancellation of those privileges. The Computer Lab Instructor, working with the School Age Service Coordinator and Youth Service Director, will deem what is inappropriate use. Such decisions are final. Any of these individuals may deny, revoke, or suspend specific user accounts or close accounts at any time as such action is warranted or required. Neither the Computer Lab, School-Age Services, or the Youth Service Centers will assume any costs, liability, or damages caused by the way in which the person assigned to an account uses the network/internet access. Violations of the Internet Acceptable Use Policy may result in a loss of access as well as other disciplinary or legal actions.

4 INTERNET PERMISSION AND AGREEMENT FORM YOUTH AGREEMENT I understand and will abide by the Internet Use Agreement and Internet Acceptable Use Policy. I further understand that any violation of the regulations can be a violation of local, state, and federal laws and that I can be prosecuted for violating those laws. Should I commit any violation of the Internet Use Agreement and Internet Acceptable Use Policy previously listed, my access privileges may be revoked, center disciplinary action may be taken, and/or appropriate legal action may be taken. User s Signature Date: Parent s Signature Date: COMPUTER LAB INSTRUCTOR AGREEMENT I have read the Internet Use Agreement and Internet Acceptable Use Policy agree to promote this agreement with the youth. Because the youth may use the network for individual work or in the context of a class, I cannot be responsible for the youth s use of the network. I cannot monitor every use of the Internet by each youth in my lab every day. As the Computer Lab Instructor, I do agree to instruct the youth on acceptable use of the network and proper network etiquette. Computer Lab Instructor s Signature Date PARENT AGREEMENT As the parent of this youth, I have read the Internet Use Agreement and Internet Acceptable Use Policy. I will discuss with my youth the rules of using the Internet and E- mail as stated in the Internet Acceptable Use Policy. I understand that network/internet access is designed for educational purposes. (Name of Computer Lab) is taking efforts to monitor usage; however, I recognize it is impossible for them to restrict access to all controversial materials, and I agree not to hold them responsible for materials accessed on the network.

5 I agree to hold the (Name of Computer Lab) and the Computer Lab Instructor harmless for any consequences resulting from the use of the Internet and . I, hereby, give permission for my child to access the Internet, and I certify that the information contained on this form is correct.
