2 Kackaamin Family Development Centre 7830 Beaver Creek Road Port Alberni, BC V9Y 8N3 Phone: Fax: Dear Referral Workers, Kackaamin Family Development Centre is committed to the family unit. Our aim is to provide families with knowledge, skills and tools to effectively deal with substance misuse and the effects it has on the family. In order to maximize the benefits of our program, thorough preparation of clients is essential as it alleviates stress and confusion enabling the families to focus on their healing journey. We recognize the importance of your job as a referral worker, since Kackaamin is unable to meet clients directly prior to admission; our link to them is through you. The information you provide for us is very important. This checklist was created to assist you, the referral worker in preparing your client s for our program and aiding in the success of their healing journey. If you are not available for long term process/relationship with the client please refer them to another agency (such as Alcohol and Drug programs, Friendship Centre or Indian Band Office.) Please submit the following checklist with your referral. Referral Source fills out the application I have gone over the Kackaamin Guidelines with each of my client(s), (Adults, Youth & Teens). My client(s) understand these guidelines and agree to abide by them. I have discussed the importance of budgeting grocery money for the entire six weeks. My client(s) understand they will be asked to leave, should the family run out of grocery money. Each of my clients are aware that they will be held accountable for their actions and behaviours while at Kackaamin including the following: Family Violence Vandalism and/or theft of property. I understand the importance of informing Kackaamin in writing of any pertinent information that was not included in the original application/assessment, such as recent medical concerns, legal issues, MCF situations and general information. As the referral worker, I take responsibility to ensure all paperwork has been sent to Kackaamin. (Please note: it is not the Intake Coordinators responsibility to keep referral workers updated on missing information, incomplete packages will be sent back). Each client, (Parents, Youth & Teens) have attended a minimum of 6 counselling sessions prior to attendance. Clients Signature: Referral Workers Signature Date: Date:
3 Financial and Travel Arrangements: Living and grocery allowances have been confirmed. (Clients are responsible for purchasing their own groceries for their family while at Kackaamin). Travel to and from Kackaamin has been confirmed and Kackaamin has been confirmed. Mental Health and Addictions Services (attached Accommodation Fee Subsidy Request Approval. Ministry of Housing and Social Development: (Funding Verification Form given to Client). Assessment and Referral Applications; SAIS forms completed and submitted: Adults (18 years and older) Youth and Teens (10 to 17 years) Children (Newborn to 9 years) SAIS forms for each adult that will be attending treatment Medical examination completed and submitted (including TB test results): Adults (18 years and older) Youth and Teenagers (10 to 17 years) Children (Newborn to 9 years) Immunization records only (If Applicable) Legal Documentation submitted: Written approval for children to attend treatment with parent/guardian whom child does not reside fulltime. Probation or parole order Other: Nuu-chah-nulth members apply for per diem funding for each client age 5 and up, through Nuuchah-nulth Tribal Council Office. (Please note that funding must be secured before an Intake Date will be confirmed). Have supervision orders and other documentation from Ministry of Children and Family Development attached with application. Reviewed completed Assessment and Referral Application; SAIS forms with clients to help them determine what they want to accomplish in treatment. Have maintained regular pre-treatment contact with clients. Have ensured that client s admission date has been confirmed. Referral Workers Signature: Date:
4 ADMISSION CRITERIA 1. All family members may attend. 2. All clients must have a minimum of 3 weeks abstinence from any substance and a desire to remain clean and sober. 3. Clients are required to attend a minimum of six - 1 hour long, pre-treatment counselling and/or other support sessions in preparation for treatment. 4. Adult family members (one or both) must have had a problem with alcohol or drugs. Extended family members are encouraged to attend treatment. 5. Family must be free from any obligations that will require absence from Kackaamin (i.e. court appearances, doctor appointments, MCFD etc. All COURT APPEARANCES MUST BE DEALT WITH PRIOR TO COMING TO KACKAAMIN. 6. All clients are to have an after-care plan in place before arriving for treatment. 7. Client Family must be physically and mentally able to participate in an intense counselling experience. 8. KFDC does not accept clients with a history/criminal conviction of sexual offences as we do not provide treatment for offenders. 9. KFDC is not under any obligation to accept a person who has been legally ordered to attend our program. 10. The client family is responsible for their return travel if they leave or are discharged from treatment early. 11. Families must arrive on Intake Day, no late arrivals. PRE-ADMISSION MEDICAL CRITERIA 1. Client must not require detox facilities 2. Pre-admission medical is Completed and signed by a physician/rn/chn prior to Admission 3. All adult clients must have a completed TB test or Chest X-ray prior to admission. Results of this exam must be sent to Kackaamin prior to admission. 4. Client does not require hospital or frequent medical care. 5. Client must not be more than 23 weeks pregnant upon entering treatment. 6. All diseases are under control, managed or in remission. 7. Clients must have a valid Personal Health Care Number (PIN #) and Status Number. GUIDELINES Upon entry of Kackaamin, each client agrees to and signs a written contract which includes the following: ALCOHOL AND DRUGS The possession and/or use of alcohol and /or drugs by clients while attending treatment is not acceptable and will result in automatic dismissal from the program. Personal belongs and unit check will be conducted by the staff on scheduled and at random times. All medications are to be turned in to our staff upon admission. The counsellor will be responsible for the distribution of medications.
5 HEALTH AND SAFETY Violence of any kind and/or destruction/damage of property may result in dismissal. Violence is abuse and is a reportable offence. TELEPHONE CALLS In the first two weeks of the program there are no calls permitted. After two weeks clients can make calls after their session ends at 4:00 pm. Clients need to bring calling cards, phone cards, etc. VISITORS Visitors are permitted to attend the Closing Ceremony but must be invited by the clients and adhere to KFDC regulations regarding ceremony schedule. Visitors are not permitted to stay on site over night. There are no visitors allowed throughout the session. SCHEDULE Upon arrival the clients are informed of the daily schedule and guidelines during orientation. INTAKE PROCEDURE 1. Please ensure Assessment and Referral Applications, medicals, TB Tests for all adult family members are completed BEFORE sending your package to Kackaamin. Intake priority is based on the date we receive the complete package. 2. Please fax us a copy if you wish and mail us the original Application Package. 3. Once the complete application for treatment is received and the clients meet the admission criteria, the Intake Coordinator will contact the referral worker/counsellor by letter or phone to verify client s admission. An intake date will be confirmed in writing (confirmation package) prior to the intake date. PER DIEM FUNDING 1. Kackaamin charges $40.00 per day for each client 5 years of age and older for each day clients attend the program. 2. Non-status and Métis clients who are currently receiving income assistance are eligible for per diem funding through the Ministry of Human Resources (MHR). The Employment and Assistance Worker can assist the client with travel money and comfort allowance. 3. Non-status clients (who are not receiving IA) may apply for per diem funding through a First Nations Band or may self-pay. In this case, Kackaamin must receive full payment (cheque or money order preferred) two weeks prior to beginning treatment. 4. Clients may apply for a User Fee Subsidy through your regional Alcohol and Drug Program office. We request that you submit the approval form with the Assessment and Referral Application. We need approval with application. 5. Travel arrangements made to and from Kackaamin must be provided by the referral agency; or other source, including taxi fare. Travel moneys are usually supplied by the subsidy funding source and travel arrangements are to be coordinated with the referral worker. CLIENTS MUST PAY FOR THEIR OWN RETURN TRAVEL IF THEY DO NOT COMPLETE TREATMENT. Kackaamin Family Development Centre does not pay for client travel. FUNDING FOR GROCERIES 1. Kackaamin does not provide meals for its clients. Families are required to cook their own meals in their units. 2. Before an Intake Date will be confirmed, Kackaamin Family Development Centre requires signed
6 documentation to verify that the client has been provided with sufficient funds for purchasing groceries during their six-week treatment session. Kackaamin Staff have no responsibility to complete this task with the client family when they arrive for treatment. In the event the client family does not arrive with or have sufficient grocery money while they are in treatment, the family may be asked to return home. Kackaamin cannot provide grocery money for families. Health Canada - Medical Services Branch is not responsible for providing grocery money for clients. 3. Recommendations for financial needs for groceries and personal items are as follows: $ $ 150 a week for a one or two parent family with one child $ $ 175 a week for a one or two parent family with two children $ $ 225 a week for a one or two parent family with three children $ $ 275 a week for a one or two parent family with four or more children ** PLEASE ENSURE THAT CLIENT WILL BE ABLE TO BUDGET SUFFICIENT FUNDS FOR GROCERIER FOR THE 6 WEEKS AND THAT THEY HAVE A FINANCIAL PLAN IN PLACE FOR THE ENTIRE PROGRAM** 4. Cheques issued for client s grocery money should be payable to Kackaamin, as we can administer their grocery money on a weekly basis. Clients are responsible for having the first week s worth of groceries with them on Intake Day. For clients arriving by bus on Intake Day, shopping for their first week of groceries can be done at our local Co-op Grocery store upon their arrival in Tofino. INFORMATION WE NEED TO KNOW RE: GROCERY FUNDS: Amount & Period Covered ex: first two weeks. Source(s) BC Benefits etc. What portion is for groceries? In some cases they are to keep a portion of their support cheque for their return home or to pay bills. Additional Monies Child Tax Benefits, GST, Family Bonus etc. Direct Deposit or will arrangements have to be made for deposit? Date(s) of arrival / Date sent Is the Grocery cheque being issued to Kackaamin or to the client? Please consider that cheques sent directly to Kackaamin Family Development Centre works the best, as we can distribute their grocery money on a weekly basis.
7 CLIENT CHECKLIST VIDEO GAMES, VHF, PAGERS, CELL PHONES, TV S, VCR S, BIKES, KNIVES OR WEAPONS (REAL OR TOY) ARE NOT PERMITTED AT KACKAAMIN PLEASE FEEL FREE TO BRING A COMPACT STEREO AND BOARD GAMES IF YOU ARE TRAVELING TO KACKAAMIN IN YOUR PERSONAL VEHICLE, KEYS FOR THE VEHICLE MUST BE LEFT WITH KACKAAMIN STAFF UNTIL COMPLETION OF THE TREATMENT PROGRAM. Please have the following items with you upon arrival at Kackaamin Family Development Centre: Identification Required Status Card Medical Cards Bank Card Calling Card Picture ID Personal Toiletries Hand Soap Shampoo Toothbrush and Toothpaste Feminine Hygiene Products Baby Diapers, Formula, Bottles, etc First Aid Supplies (e.g. Band-Aids, ointments) Hobbies and Interests Fishing Rods and Tackle Hiking Boots and other supplies Beading Material Arts and Crafts Supplies / Board Games Towels Dish and Tea Towels Facecloths, Hand and Bath Towels Cleaning Supplies Tissue Dish Soap All Purpose Cleaner Garbage Bags Kitchen Supplies /Food/Staples Spices, Flour and Sugar Coffee and Tea Salt and Pepper Staple goods (e.g. milk, cereal, bread, etc.) ** BRING THE FIRST WEEK S WORTH OF GROCERIES WITH YOU ON INTAKE DAY** Extras Items that WILL be supplied for you Rain Gear and Warm Jacket Bed linen Slippers Dishes (plates, bowls and cups /glasses) and cutlery Inside Running shoes and sneakers for the gym Coffee pot Writing Paper, envelopes and stamps Baking pans and utensils Alarm Clock / Clock Radio Cook ware Camera and Film Flash light Your personal medication (to be checked in with Kackaamin staff upon arrival) PLEASE REMEMBER THAT KACKAAMIN WILL BE YOUR HOME FOR SIX WEEKS. THEREFORE, YOU WILL WANT TO BRING MOST ITEMS WITH YOU THAT YOU WOULD NORMALLY USE AT YOUR OWN HOME. **This is only a guideline** Please ensure that you look after the following appointments before coming to Kackaamin. You will not be allowed to leave Kackaamin except on designated shopping days, unless it is an absolute emergency. (Staff decides what constitutes an emergency).
