Life Group Term Life Voluntary Group Term Life (VGTL) Term Life insurance provides benefits to a designated beneficiary for loss of the covered employee s life. Supplemental Life Supplemental Life is elected on a voluntary basis to augment term life insurance for those employees who desire higher levels of coverage. Disability Group Short Term Disability (STD) Voluntary Short Term Disability (VSTD) STD insurance coverage provides partial income replacement for eligible employees who become disabled as a result of a covered sickness or accident, including pregnancy. Group Accidental Death & Dismemberment (AD&D) Voluntary Accidental Death & Dismemberment (VAD&D) The AD&D policy is a companion coverage option to term life insurance. The benefit covers customers who suffer loss of life or dismemberment resulting from a covered accidental injury. Dependent Life Dependent Life insurance provides benefits to the employee for the loss of a covered dependent s life. Group Long Term Disability (LTD) Voluntary Long Term Disability (VLTD) LTD coverage offers partial income replacement under much of the same eligibility requirements as STD plans. However, the benefit duration periods are longer and benefits may include rehabilitation, vocational assistance, family care, survivor benefits and workplace modification. Accident Accident* insurance provides supplemental protection to cover the costs of covered accidental injuries occurring both on and off the job. Benefits include: emergency room treatment, follow-up physician visits, physical therapy, hospital confinement and more. Cancer Cancer* insurance provides benefits as a result of cancer or other covered specified diseases, depending upon the customer need. Payments are made in addition to health insurance and go directly to the insured. Critical Illness Critical Illness* insurance pays a lump sum to insured individuals upon the diagnosis of a covered critical illness, which may include cancer, heart attack, stroke, end stage renal disease and more. Coverage is available to eligible family members as well. Group Dental** Group Vision Voluntary Dental** Hospital Confinement Hospital Confinement* insurance is available for customers and eligible family members who need assistance with deductible and copayment expenses; the plan can also provide supplemental benefits for inpatient hospital expenses. Life For customers who want additional choices for life insurance, the Individual Term Life* plan provides coverage for 10, 15, 20 or 30 year term periods. Term to age 80 is also available and provides the same amount of coverage for the same price until age 80. Premiums are level and guaranteed for the initial term period. Online Services Through online enrollment and billing, group administrators can ensure employee information is kept accurate and up-todate even between billing cycles. Additionally, the status of disability claims and medical underwriting can be monitored, all in an online, secure, paperless environment. (With Group Life, AD&D, Dependent Life, Supplemental Life, Group STD, Group LTD, VGTL, VAD&D, VSTD and VLTD) Online Will Preparation Online Will Preparation services help employees make important legal decisions that protect families, safeguard assets and ensure wishes are carried out. Employees are provided guidance in creating Will, Living Will and Power of Attorney documents at no cost. (With Group Life and AD&D) Travel Protection With Travel Protection and one simple phone call, employees have access to trained multilingual and medical personnel to quickly advise and assist employees in a medical emergency while traveling 100 miles or more away from permanent residence, or in another country. Services are provided 24 hours a day, 365 days a year. (With Group Life and AD&D for groups with lives) Identity Theft This service provides an online source of information concerning identity theft, such as prevention tips, consumer guides and ID Theft Tests. In the event an identity is stolen, the identity theft resource provides employees with victim support, including letter templates to inform and lessen the impact of the situation. (With Group Life and AD&D) Employee Assistance Programs (Online or Enhanced*) Employee Assistance Programs (EAP) help employees and their family members identify challenges that might adversely impact their work performance, health and well-being. Through counseling, assessments, referrals, resources and an interactive website, EAPs can offer solutions to help find balance. (With Group STD and Group LTD) FICA Matching* This service is offered with Group Short Term and Long Term Disability plans and saves the employer time by paying their share of Social Security and Medicare Taxes (FICA). Also includes printing and mailing of W-2 s to claimants. (With Group STD for groups with 50+ lives; with Group LTD - group size varies) W-2 Services To save valuable time at year end, Forms W-2 containing disability claim payment information can be produced for employees use in filing individual income tax returns. These forms are filed with the IRS under our name and Tax Identification Number. (With Group STD, Group LTD, VSTD and VLTD) Express Claims Employees can receive a lump sum payment at the beginning of the eligible disability period, rather than a weekly benefit payment. Express Claims service allows for flexibility in meeting the needs of individuals and quickly provides income replacement for eligible claims. (With Group STD and VSTD) FMLA Administration* This service offers a convenient alternative to internally administering FMLA. A staff of HR specialists and attorneys manages cases from start to finish shouldering the burden on behalf of the employer. The outcome is fair, accurate and consistent FMLA administration that reduces costs and liability risk. Disability Claims Management Easy filing for claimants. Claims can be submitted by , mail or fax. Fast claim payments. Ninety percent (90%) of filed STD claims are paid within 3-5 business days. For all disability claims, we stay in contact with the physician and employer, as well as the claimant to determine if their return-to-work status has changed. One hundred percent (100%) of all LTD claims receive clinical review and are acknowledged within 2 business days, making contact with the claimant and employer. For LTD, we bring a full array of claims professionals including clinical, vocational and rehab specialists working to devise the best plan for the individual s needs.
