1 The UNIVERSITY of NEWCASTLE BACHELOR OF BIOMEDICAL SCIENCE (HONOURS) Faculty of Health and Medicine Application Form Semester 1 or 2, 2015 INSTRUCTIONS The Bachelor of Biomedical Science (Honours) program is an academic extension of the Bachelor of Biomedical Science. Applicants are required to select a project from the range offered and undertake a research program specialising in a particular discipline from among the Biomedical Sciences. Admission will take into account the proposed project and the Supervisor, as well as the provision of an equitable balance of students across the School and between Schools. The course is offered over one year of full time or two years of part-time study within the Faculty of Health and Medicine. To be eligible to undertake the Bachelor of Biomedical Science (Honours) program, student must have: i. qualified for the award of a Bachelor degree (level 7); and ii. iii. completed the requirements of an approved major in the field of study, to be their focus in the Honours program; and achieved the approved GPA listed in the ehandbook (Program Management System) for the program with a minimum of: a. a cumulative GPA of 5.0 or higher ; or b. a GPA of 5.0 or higher in the approved major; or c. a GPA of 5.0 or higher in 60 units of approved courses at the 3000 level; and iv. met any supplementary criteria listed in the ehandbook (such as presentation of portfolio, interview, auditions); and v. had a written application approved by the relevant Faculty Pro Vice-Chancellor (or nominee) responsible for the program. APPLICATION Step 1 Complete The University of Newcastle Direct Admission 2015 Application Form: Note: proof of secondary studies is not required as you will be assessed on your Bachelor degree studies. Step 2 Complete Part One of the attached Bachelor of Biomedical Science (Honours) form and have your Supervisor complete Part Two. Step 3 Return Direct Admission 2015 Application Form along with both parts of the attached form to the Bachelor of Biomedical Science (Honours) Program Officer, Hunter Hub, Student Services Centre, University of Newcastle, Callaghan, NSW, 2308 by Semester 1, 2015 (closing date 11 January 2015) or Semester 2, 2015 (closing date 14 June 2015) Step 4 If necessary, obtain the appropriate Ethics Clearance application form, complete and return it according to instructions. The Application for Ethics Approval for Research Involving Humans for Undergraduate and Postgraduate Coursework Students: Step 5 If necessary complete the Safety Review Assessment Form and risk assessments for the specific research processes, determine the risk level and implement the relevant procedure for the local and/or escalated assessment.
