2 Sectra MEI for efficient information management The challenge Working with information cross-enterprise means having access to information from several hospitals in addition to the information available in your own systems. In radiology, this means making information in RIS and PACS visible for others, even if they do not have the same type of RIS or PACS as you have. Requests and reports combined with images should be accessible from other places. The challenge is to build a system that can do this efficiently and without unnecessary costs, while always ensuring that any RIS or PACS can connect, regardless of make and model. Information access In providing the best and most efficient patient care, radiologists and radiographers need access to complete patient medical history. This means being able to see requests and reports, as well as having access to images from other RIS and PACS systems. For radiology, access should be manageable since both images and patient information are stored digitally in most places. Accordingly, although the information is stored in many systems, often in different locations, it should be possible to access this information easily. Nevertheless, in the real world this is not the case, mostly because RIS and PACS systems are not always designed to handle cross-enterprise information management in an efficient manner. Reporting workflow Information access alone is sometimes not enough. To achieve an even more efficient workflow, it would be valuable to be able to work with other systems in a more direct manner. If the user can view images and reports from another hospital, the next step would be the ability to work using this information and to create new reports from a remote location. The ability to work with distributed reporting is a key factor in reducing costs and increasing effectiveness within radiology. For example, it could facilitate remote reporting outside office hours, or spreading the task of reporting among several hospitals within a region. 2

Development of the first version of Sectra MEI started in 2004 and was delivered for operation to our first customers in Since then, among other enhancements, the Sectra MEI solution has been expanded with functionality for distributed reporting workflow. In Skåne, the southernmost region of Sweden, Sectra MEI has been able to integrate all hospitals in the region, making it possible to view reports and requests as well as images from any RIS or PACS. This overview is provided to the users without having to manually search for the information, since the system does the searching for them. Sectra MEI collects information from the other RIS and PACS systems automatically and displays this information together with the local RIS and PACS information, regardless of which system being used.

4 Vendor independent integration Connecting to Sectra MEI requires systems to communicate based on the DICOM or HL7 standards, which are a part of the XDS-I framework established by IHE in late The DICOM and HL7 standards provide a favorable platform for integration, and the XDS-I framework is even more specifically suited for cross-enterprise information management. The strategy for Sectra MEI is to increase the use of the XDS-I framework as well as other applicable IHE recommendations as the system is developed further. Using standards, new and improved functionality is secured, while still ensuring that any RIS or PACS can connect. Performance Successful management of the potential performance issues when transmitting large amounts of information such as radiology images across a network is a key factor in the implementation of a cross-enterprise workflow. Sectra MEI has many features designed to eliminate unnecessary data transport over the network to minimize the network load. One of the techniques that has been proven in live hospital environments is the use of intelligent prefetch, in which relevant requests, reports and images are acquired in advance according to set rules. Combined with local caches that can store data short term, the availability of information can be greatly increased. Moreover, Sectra MEI transmitts information using fast streaming of data when possible, optimizing network usage. As a result, the user can always see relevant information from other RIS and PACS systems instantaneously, without having to wait.
