2 Our Purpose s program exists to prepare mission-minded pilots for Global Ministry. Our graduates are currently serving with industry and mission aviation groups around the world. Preparing pilots for ministry around the world is demanding and the staff at the aviation department is working hard to meet that challenge. The summer flight camp is a unique opportunity that Prairie Bible Institute offers to youth (ages 14-18) to get a glimpse at the world of aviation. is offering you the chance to learn some of the basics skills of flying in a summer camp environment. Our hope is that you catch the aviation bug and pursue your dream of flying. Our Plan Our busy flight camp week will include ground instruction covering topics such as Attitudes and Movements, Climbs and Descents and Navigational Preparation, flight instruction in our Cessna 172s, as well as outdoor activities, devotions and campfire time. Our camp instructors are licensed flight instructors who will be teaching in our Cessna 172s and are the same instructors that teach our program degree students throughout the college semester. The ground portion of the instruction is the same material that is covered in the Canadian Private Pilot License ground school course and the in-flight training follows our regular flight curriculum. For this reason, the experience gained at PSMA Flight Camp can be counted toward a future pilot s license. Our goal is to not only have fun with airplanes but to encourage a personal relationship with Jesus Christ. It is our desire that every camper be encouraged in their walk with God, as well as receive a good introduction into the aviation industry.

3 What You Need to Do 1. Complete the application form either by printing page 4 of this packet, or online at edu/flightcamp 2. If you are a minor, the Flight Camp Waiver form (page 5) needs to be signed by your parent or guardian. It can be signed by you if you are 18 years of age or older. 3. Complete the medical information form on page Send in these three forms either by mail or electronically with your (non-refundable) $50.00 deposit. More Information The cost for the flight camp is $1,200 CAD. This includes accommodations, meals, ground training, flight time, and all planned activities for the week. A $50.00 deposit is required at the time of application and is part of the camp fee. Final payment should be received by PSMA on or before June 12, Registrations and fees received on or before May 15, 2015 qualify for an early bird discount of $100. We would like to extend an invitation to parents and/or guardians to join us all at the conclusion of flight camp for a BBQ dinner on Friday evening (July 24). This is a great opportunity for you to hear all about what was accomplished through the week of flight camp. Once we have received application forms, we will send you the acceptance package containing further information which will include travel itineraries, what to bring, etc. Thank you for your interest in PSMA s flight camp, and we hope to see you this summer!

5 Airplane Ride Waiver Form (for minors) EVENT OR ACTIVITY: IN CONSIDERATION for allowing the minor to participate in any way in the event or activity encompassed by this Agreement and in particular for permitting the minor to travel by aircraft or other conveyance in connection therewith the parent or guardian (as the case may be) for such person and for the minor, hereby agree as follows: 1. THE MINOR AND PARENT OR GUARDIAN concur that they have been provided with all information reasonably possible concerning the event or activity mentioned above and have inspected or will inspect to the event or activity may be conducted. The undersigned agree that if at any time in or upon such places they believe something is unsafe, it will be brought to the attention of a teacher or other supervisor of the event or activity and will remove themselves from the place and the minor will withdraw from participation in the event or activity. 2. THE MINOR AND PARENT OR GUARDIAN HEREBY ASSUME FULL RESPONSIBILITY FOR AND RISK OF BODILY INJURY, DEATH OR PROPERTY DAMAGE due to negligence or releases (as identified below) or otherwise, while in or upon such places for any purpose and in any way participating in the activity or event. The undersigned recognize and understand that there are risks and dangers associated with participating in the event or activity and in particular with travel associated therewith that could cause severe bodily injury, disability and death. Furthermore, the minor and parent or guardian acknowledge that Prairie Bible Institute and its teachers, supervisors, directors, and officers have no control over the maintenance or safe operation of any conveyance utilized by any common carrier in connection with the event or activity. Further, the risks and dangers associated with participating in the event or activity and entry into the places are assumed notwithstanding. 3. THE MINOR AND PARENT OR GUARDIAN, release, waive, discharge and covenant not to sue Prairie Bible Institute and its teachers, supervisors, directors and officers, participants, owners, lessees and lessors of the premises and place used to conduct the event or activity and their officers, agents and employees, all for the purposes herein referred to as releases, from all ability to losses or damages of the minor and/or parent or guardian on account of any injury, including but not limited to the death of the parent, guardian, or minor or damage to property caused or alleged to be cause in whole or in part by the negligence of the releases or otherwise. 4. THE PARENT OR GUARDIAN hereby agrees to indemnity and save and hold harmless, the releases and each of them from and loss, liability, damage or cost that may occur due to the presence of the parent, the guardian or the minor in any places or in any while participating in the event or activity and whether caused by negligence of the releases or otherwise. The parent or guardian further recognize and agree they are executing this Waiver and Release of Liability and Indemnity Agreement on behalf of themselves and on behalf of the minor. THE MINOR AND PARENT OR GUARDIAN HAS READ AND VOLUNTARILY SIGNS THE WAIVER AND RELEASE OF LIABILITY AND INDEMNITY AGREEMENT AND DOES SO VOLUNTARILY AND WITH THE UNDERSTANDING THAT SUBSTANTIAL RIGHTS ARE BEING GIVEN UP. 1.
