2 Dear Prospective Practical Nursing Students: Thank you for your interest in the Practical Nursing (PN) Program at Fletcher Technical Community College. Applications for the fall 2014 Practical Nursing Program are currently being accepted. The application deadline for the PN program will be Friday May 15, 2015 at 4:00 p.m. at St. Charles location. Applications received after this deadline will be deemed automatically ineligible for the PN program. Students accepted into the PN program will complete the program in five semesters. Successful completion of the Practical Nursing Diploma prepares you to take the National Council Licensure Examination for Practical Nurses (NCLEX-PN) for licensure as a Practical Nurse. Specific information regarding the practical nursing curriculum can be found in the current academic year s catalog located on Fletcher s website ( Course descriptions for each course listed in the curriculum are available in the catalog. Applicants should verify the curriculum before submitting the application as this information is subject to change. Please read the following document fully. Students who are not admitted for fall 2015 must re-apply for admission for a future entrance term. It is the student s responsibility to retain copies of documents and resubmit a new application packet for future classes. Fletcher does not maintain a waiting list. Admission requirements are reviewed annually and are subject to change. Please ensure you are using the correct guide for the semester you are applying. Student Learning Outcomes: Upon successful completion of the Practical Nurse Program, the student will: 1. Provide nursing care that contributes to the enhancement of the health care delivery setting and protects clients and health care personnel 2. Demonstrate competency in laboratory and clinical skills Students with Prior Practical Nursing Credit: In order for a student to request advanced standing/re-entry into a nursing program, the student must meet all admission criteria for the requested semester of entry listed in the Admission Guide. You must submit the Advanced Standing/Re- entry Request Form found on the Fletcher website under Program Practical Nursing Advanced Standing/Re-entry Request form. You must also submit an admission application to Fletcher Technical Community college and the Nursing and Allied Health Application. 4

5 We do not have a separate class for students requesting advanced standing. Your official transcript will be reviewed and placement will be made in the on-going class provided there is a vacancy and all admission criteria have been met. Each transcript will be reviewed and handled on an individual basis. College Admission Requirements Applicants who are interested in the Practical Nursing Program should complete a college admissions application prior to completing this admission application. The application can be found on the Fletcher website at The cost is $10. Applicants should apply for pre-practical nursing fall If you are accepted to the program, your major will be changed to practical nursing. Practical nursing applicants must meet all general admissions criteria and be full or unconditionally admitted to Fletcher. Full admission is granted once a student has submitted all materials required by the Office of Admissions and these items have been positively evaluated. This may require you the applicant to submit items twice: once to the Office of Student Affairs and once in the admission application to the Department of Nursing. Full admission cannot be granted to a student with outstanding holds from any school in the Louisiana Technical Community College System (example: Delgado). New and returning students (those who did not attend in Spring 2014) must submit a completed admissions application in order to be considered for the Practical Nursing Program. Academic Probation Students currently under academic probation with Fletcher Technical Community College will remain on academic probation upon acceptance into the Practical Nursing Program. Students will be required to sign a waiver stating they understand the terms of academic probation and accept those terms prior to enrolling. Students on academic probation will be subject to all of the criteria of academic probation as outlined in the college catalog. Program Admissions Requirements Application for Admission to a Nursing or Allied Health Program A completed Application for Admission to a Nursing or Allied Health Program must be submitted along with the other required documents listed in this guide to the Department of Nursing by the deadline. The Practical Nursing Program Admission Checklist and Instructions sheet must be attached. This document can be found on the last two pages of this document. Testing/Academic Requirements Any student wishing to enter into the Practical Nursing Program must achieve the following minimum scores: ACT test score within the last 3 years (no later than July 2012): Reading 20 English 18 Math 19 5

6 COMPASS test score within the last 2 years (no later than July 2013): Reading 85 Writing 70 Algebra 40 Test scores must be submitted in the application. Test may be scheduled at the Main Campus located in Schriever by calling Student Affairs at (985) Test preps can be obtained through Kahn Academy, algebrahelp.com, purplemath.com, and test-guide.com. Transcript Evaluation Official high school /Hiset/GED transcripts must be sent directly to the Nursing and Allied Health Department from the high school in which the PN applicant attended even if High School/ Hiset/GED transcripts were sent to Fletcher s Student Affairs/Admission department. Due to the nature of this document, high school transcripts are due to our office by June 15 th. A copy of your diploma will not be accepted. Transcripts must be requested from your high school and mailed to: Fletcher Technical Community College Nursing and Allied Health Department Attn: Allison Adams 1407 Hwy 311 Schriever, LA College Transcripts If you attended any college or university other than Fletcher prior to applying at Fletcher, you must also submit official transcripts from that college. You will need to request the college or university send two copies of official transcripts to both Fletcher Student Services and Allied Health Department via E-Script or regular mail (address listed above). If the transcript is hand delivered, it must be sealed by the issuing institution. Transcripts must be submitted even if credit was not granted. Official college transcripts must be sent directly from all previous institutions to Fletcher Technical Community College for a formal evaluation of credits earned to determine what previous coursework will transfer to Fletcher. Transcripts should include course work that is in progress. Immunizations Students are required to submit copies of 2 MMR vaccines as well as the tetanus vaccine (TD or DTaP). The tetanus vaccine must be within 10 years and after September 1, Legal documents Students are required to submit copies of their Driver s License or Identification card as well as their selective service card (for males). The name and address on the driver s license or ID should match legal name and have your correct address. Go to the DMV and change it now if it needs to be changed. A copy of their birth certificate and any other name changing documentation (marriage license, divorce decree) must also be included in the admission packet. The copy of the birth certificate should contain all information on the birth certificate. If the student submits a birth certificate card, the card must contain the full middle name, not simply an initial. If information is cut off, illegible or does not contain the full name, the application will be deemed ineligible. 6

7 Certified Background Check All applicants are required to obtain a background check from their local sheriff s office. The approximate cost is $10. The results must be submitted with your program application packet. Program Admissions Application Deadline A completed Application for Admission to Nursing & Allied Health Programs and all required supporting documents must be submitted to the Department of Nursing no later than Friday, May 15, 2015 at 4:00 p.m. Absolutely no applications will be accepted for the Fall 2015 PN class after this date. It is recommended that you hand-deliver your information to Roye Matherne or Allison Adams at the Nursing and Allied Health Department at the St. Charles location 310 St. Charles St. Houma, LA Only completed applications will be reviewed for admission. It is the applicant s responsibility to make sure that all application materials are completed (no blanks) and received at Fletcher Technical Community College by the deadline dates. If the blank on the application does not apply, place N/A in the blank. Please be sure all information provided on the application is up to date and accurate including address and telephone numbers, as it will be used as contact information for notification of acceptance. Incomplete applications will be automatically rejected. It is strongly recommended that you submit all documents together in a professional and organized manner in a manila folder. Do not submit in a notebook, binder, etc. You should keep photocopies of all documents submitted. A completed application does not guarantee acceptance into the program. If you chose to mail your Practical Nursing Admission packet, The Application for Admission to Nursing & Allied Health Programs, supporting documents, and official transcripts should be sent certified mailed to Fletcher Technical Community College, Attn: Practical Nursing Program 1407 Highway 311, Schriever, LA What to expect after applications are submitted? Approximately two weeks later, students who have complete applications turned in on time will be sent a letter inviting them to the second phase of the admissions process: Psychological Services Bureau (PSB) test. The applicant will be assigned a specific date and time for testing. Any student that is not present on their assigned date and time will not be allowed to move any further in the process and will have to reapply for the following year. PSB scores are only valid for one year. Once all tests are administered, PSB and COMPASS/ACT scores will be weighted, added, and ranked. The top total scores will be selected for admission to the PN program. Applicants with the top total scores will then be given information regarding orientation and registration, usually scheduled the first week of July. All applicants will receive written notification of their status. Accepted students must respond by the indicated date on the acceptance letter that they are accepting the invitation to the PN program. Failure to respond by the indicated date will result in cancellation of your acceptance. 7

8 What Happens If I Am Offered Admissions to the Program? Practical Nursing Students offered admissions to the PN program are required to attend a program orientation session held early in July. At that time, students will be given information regarding registration for practical nursing courses and other valuable program information. The specific orientation date, time, and location will be sent to admitted applicants. If you do not attend this orientation, it will be assumed you are declining acceptance to the program. Due to the fast pace, challenging, and rigorous academic demands of the PN program, the nursing faculty strongly urge students to avoid full-time work commitments. Clinical schedules are subject to last-minute revisions each semester based on facility request. Clinical rotations are scheduled at a variety of clinical sites that may be over 20 miles or more from campus. Therefore, students personal commitments must remain flexible to meet course objectives and accommodate program requirements. Reliable transportation is needed due to the distance of some of the clinical sites. Due to the nature of the program, specific requirements must be met prior to enrollment into Practical Nursing. These items are not required for the application process but are included for you to prepare yourself financially should you be accepted. They include but not limited to: History and Physical Form stating the student is free and clear to participate in clinical duties completed by his/her medical care provider and submitted before registering for classes. FBI Background Checks and fingerprinting Proof of not having a criminal record of pleading guilty or no contest to, or a conviction of neither a felony nor currently serving a court imposed order of supervised probation, work-release, school release and/or parole in conjunction with any felony conviction, plea agreement or any agreement pursuant to the Louisiana Code of Criminal Procedure, Article 89 Complete a Negative Drug screen and be subject to random drug testing. Applicants who receive positive results for synthetic street drugs, cocaine, and marijuana will be dismissed from the program immediately if the letter was not presented to the department prior to testing. Positive results accompanied by a letter from the prescribing doctor for amphetamines, barbiturates, benzodiazepines, opiates, phencyclidines, Ritalin, or Adderall may be reviewed. Provide information about currently prescribed medications must be presented to the Department of Nursing prior to drug testing Criteria for Dismissal from the Practical Nursing Program Although a student may be accepted into the program, the Louisiana State Board of Practical Nurse E xaminers may deny eligibility to enter/progress in clinical nursing courses and/or take the state board exam based on prior criminal activity and/or inability to meet state board requirements. It is important that the student understand that successful 8

9 completion of the practical nursing program does not guarantee eligibility to take the NCLEX exam (LSBPNE.com). Items that will result in a student being dismissed from the program are: 1. Failure to achieve at least an 80% final average in any required nursing courses. Once a student fails any nursing course, he/she is automatically terminated from the course and will not be allowed to progress to the next semester. If the student is readmitted with advanced standing (pending there are openings) and again fails or resigns from a nursing course, the student must sit out of the program for 5 years before reapplying. 2. Failure to achieve at least an 80% on the course related math exam taken prior to clinical rotation. 3. Participation in academic cheating, plagiarism, and/or unauthorized possession of an examination. 4. Falsification of patient, personal, and/or agency records and/or documentation. 5. The unlawful and/or unauthorized use, abuse, possession, distribution, transportation, manufacture, concealment, consumption, promotion or sale of alcohol, illegal drugs, substances, or designer drugs. 6. Illegal possession of weapons on campus or at a clinical site. 7. Theft, vandalism, acts or threats of violence, or any other criminal activity while on campus or in any clinical setting or through social media sites. 8. Lack of professional demeanor and/or unsafe clinical practice as identified by the Nursing faculty. 9. Failure to receive Louisiana State Board of Practical Nurse Examiners (LSBPNE) approval to be admitted into clinicals or permission to progress in the program. 10. Failure to maintain HIPPA standards regarding patient confidentiality in clinical or on social media sites. 11. Failure to provide or be truthful regarding any necessary and/or required documentation to LSBPNE, Fletcher, or the Nursing Department including but not limited to immunizations, criminal background history, etc. 9

10 Program Admission Checklist and Instructions Fall 2015 Entrance Term Practical Nursing Applicants to the Fletcher s Practical Nursing Program must complete and submit the following information in order to apply. If at any point, the candidate does not comply with any part of this process, the candidate will not be eligible for the program and will have to re-apply to the PN program. It is highly recommended that information be submitted together in a professional and organized manner in a folder, not in a notebook or binder. It is recommended that you hand-deliver all information including official transcripts to the St. Charles location. Please make sure official transcripts are left in the sealed envelopes; they will not be accepted if they are opened. New (did not attend Fletcher in spring 2015) and returning students must submit all of the following: 1. This check list signed and dated. 2. Complete application to Fletcher Technical Community College online at Apply now Admissions Application and pay the application fee. 3. Be fully or unconditionally accepted to Fletcher by completing all documentation requested by Fletcher Technical Community College. 4. Complete an Application for Admissions to a Nursing or Allied Health Programs Form (located on the last two pages of this document) and return to the Department of Nursing at St. Charles Street location along with required documents listed below. If an area is left blank, the entire application is null and void. 5.
