1 CHECKLIST FOR APPLICATION SECOND DEGREE ACCELERATED BACHELOR OF SCIENCE IN NURSING PLEASE BE SURE THAT YOU HAVE COMPLETED EVERY STEP BELOW, AND THAT YOU HAVE ENCLOSED ALL DOCUMENTS BEFORE SUBMITIING YOUR APPLICATION. 1. Apply for admission to Arkansas State University (Jonesboro Campus) for Summer 2016 undergraduate post degree. You must be admitted to A-State-Jonesboro before accepted to the nursing program. 2. Complete in full, sign, and date the Second Degree Bachelor of Science in Nursing Application for Admission, mail application to: A-State CNHP ATTN: Carla McCain P.O. Box 910 State University, AR Beginning, Spring 2015, an Admission Exam will be required. The Admission Exam includes, but is not limited to, Reading Comprehension, Vocabulary, and Math. Please review the School of Nursing website for Admission Exam test dates and for information on registering for the exam. 4. Submit a copy of your Spring class schedule if you are enrolled at any institution. 5. Request three official transcripts from each institution you have attended since high school (including ASU) and submit: a) one in the original sealed envelope(s) along with the School of Nursing application, b) one to the Admission Office and c) keep the third unopened for future use. 6. If you have withdrawn, been dismissed, or otherwise not completed any other nursing program, obtain a letter of good standing from previous nursing program director/chair (letter must be sealed and in official university envelope) 7. YES NO If yes, what country? *NOTE: All foreign born students MUST take one of the following tests 1) Test of English as a Foreign Language (TOEFL) with a minimum TOEFL score of 83 on the preferred internet-based test (ibt), 570 on the paper-based test, or 213 on the computer-based test; 2) International English Language Testing System (IEL TS) with a score of at least 6.5 and a spoken band score of 7; or 3) Pearson Test of English Academic(PTE) with a score of 56 (see website for details).

3 Applicants for admission will be selected by the Admission Committee using the following criteria: prerequisite GPA and Admission Exam score. Based upon the above criteria, the top 40 applicants will move on to the interview process. All prerequisites courses must be completed by the end of the Spring term. See website ( for application requirements. If your GPA is below 3.0 and/or you do not have university acceptance, PLEASE DO NOT SUBMIT AN APPLICATION AT THIS TIME. All applicants will be notified in writing of the decision of the Accelerated BSN Admissions Committee. NO results will be given over the phone. APPLICATION PACKET DOCUMENTS: Applicants must submit the following documents together as a nursing application packet (1-4). Again, INCOMPLETE OR LATE APPLICATIONS WILL NOT BE CONSIDERED. 1. The completed College of Nursing and Health Professions Second Degree Accelerated BSN Program Application for Admission form. 2. Make payment and register to take the Admission Exam. Please see website for test dates and payment form. 3. Submit a copy of your Spring class schedule if currently enrolled at any institution. 4. Official (sealed) transcript(s) of all college work completed (including A-State Jonesboro) with grades on courses taken in the last semester of enrollment. You must request these from EACH institution (including A-State). 5. Letter of Good Standing: If you have withdrawn, been dismissed, or otherwise not completed any other nursing program, obtain and submit a letter of good standing from the director(s) of the nursing program(s) previously attended.
