16 ELIGIBILITY AND ENROLLMENT Eligibility Maestro s coverage is available to individuals, families and groups of all nationalities residing in Latin America and the Caribbean. Persons between the ages of 31 days and 74 years old may apply for coverage. Persons older than 74 years of age are not eligible. Certain other restrictions may also apply. Please ask your trusted benefit advisor or IMG for further eligibility details or review the Maestro Certificate Wording, which is available upon request. Each person requesting coverage must complete the information required in the application, and all persons applying must be listed on the initial Application Form. 16

18 Renewal of Coverage Lifetime Coverage Subject to the terms of the plan, Maestro s coverage can be renewed annually and coverage is continuous when renewed. Prior to the end of each 12-month Period of Coverage, eligible insureds will receive renewal information. You must continue to meet the eligibility requirements in order to renew. You have the option to renew online, or you may complete a paper renewal form. There are no additional medical questions at renewal. Your renewal premium is based on the age category applicable to you. Please select your initial deductible carefully, as you will be unable to select a lower annual deductible when renewing coverage. All Maestro plans have a maximum age limit and will automatically terminate upon the insured s 75th birthday. If you are enrolled in a Maestro plan by your 65th birthday and maintain continuous coverage to age 75, coverage under a senior version of the Maestro Vital plan will be made available to you. There are no additional medical questions to apply; you simply need to review the benefits and complete and return the application with your premium. 18

Efficient Claims Settlement Upon approval of your Maestro application, you will receive a Fulfillment Kit containing coverage information, the insurance contract and Identification Card, which should be presented to providers and enables IMG to process your claims efficiently and even arrange payment directly to your medical provider. Itemized bills of your charges must be sent to IMG within 180 days of that treatment in order to begin the Proof of Claim process. Claim payments may be made in most currencies directly into your bank account or VISA or MasterCard account. 20 How to Apply To apply for a Maestro plan, simply complete and return the application along with payment information to your trusted benefit advisor or to IMG by mail, or fax. You may also obtain a quote or apply online at If you are applying as a family, you may include yourself, your spouse and dependents under age 18 on one application. Dependents 18 years of age or older must complete a separate application. You are required to notify IMG if you or any family member suffers from or is treated for any illness, injury or medical condition between the time of your application and the issuance of the certificate. If your application is not approved, any funds received will be returned. For additional information regarding the application process, please contact your trusted benefit advisor or IMG. 20

To ensure your satisfaction, once you are approved for the coverage, we provide a 15- day period to review the Fulfillment Kit contents. If during that 15-day period you find that you are not satisfied with the coverage for any reason, and as long as you have not already made a claim, you may submit a written request for cancellation and receive full refund of the premium paid.
