1 LEGAL NOTICE TOWN OF GRISWOLD REQUEST FOR PROPOSAL ( RFP ) AMBULANCE SERVICES TO PROVIDE SERVICE IN THE AREA COVERED BY THE TOWN OF GRISWOLD AMBULANCE DISTRICT (SEPTEMBER 2014) SECTION 1: PROJECT INTRODUCTION 1.1 The Town of Griswold, CT ( the Town ) is interested in receiving proposals from professional and qualified Ambulance Service Providers, which includes the provision of Advanced Life Support Care ( ALS ) and Basic Life Support Care ( BLS ). The selected provider will be the primary provider of ambulance service, and shall respond to all ambulance calls within the geographic boundaries of the Town. Alternatively, the selected provider will be the primary provider of ambulance service and shall respond to all BLS ambulance calls, and shall subcontract with an other EMS provider to provide ALS and paramedic services. All vehicles and equipment necessary to carry out such services shall be equipped and staffed by the selected provider pursuant to Connecticut General Statutes and State of Connecticut Regulations. The selected provider will render the highest level of ambulance services to the Town that it is certified by the State of Connecticut Department of Public Health ( DPH ) to provide. 1.2 The provider will participate with the Town in updating its local emergency medical services plan, in accordance with Conn. Gen. Stats. 19a-181b, et. seq. 1.3 The selected provider will be submitted to the Commissioner of DPH pursuant to section 22 of Public Act Ultimate commencement of the selected provider as primary service area provider for the Town is dependent on approval by the Commissioner of DPH. 1.4 This RFP is pursuant to Griswold Gen. Code 5-2(A). SECTION 2: DEFINITIONS 2.1 The definitions are contained in Conn. Gen. Stats. 19a-175, and applicable emergency medical service regulations, as amended, established by the Commissioner of the DPH shall be applicable to and incorporated in this agreement. In addition, the following definitions shall apply.

2 2.2 EMS: Emergency Medical Services 2.3 Other EMS Provider: shall be any provider other than the selected Proposal who is providing service within the Town of Griswold as allowed by State Regulation and Statutes, either acting on a sub-agreement or Mutual Aid Basis. 2.4 BLS: Basic Life Support; ALS: Advanced Life Support SECTION 3: SCOPE OF SERVICES 3.1 The selected proposer shall be the primary provider of ambulance service in the Town. The selected proposer shall respond to all ambulance calls within the geographic boundaries of the Town, provided that the selected proposer is in compliance with all state statutes and regulations pertaining to licensing, certification and performance of emergency medical services. Alternatively, the selected provider will be the primary provider of ambulance service and shall respond to all BLS ambulance calls, and shall sub-contract with an other EMS provider to provide ALS and paramedic services. 3.2 The selected proposer shall provide ambulance and emergency medical services within the geographic boundaries of the Town. The selected proposer shall maintain two ambulances for responding to ambulance calls in the Town. (See Section 8.2 below for more details.) All vehicles and equipment necessary to carry out such services shall be equipped and staffed by the selected proposer pursuant to Connecticut General Statutes and State of Connecticut Regulations. 3.3 The selected proposer will be required to maintain necessary licenses, certifications, etc., as required by ordinance, law, or by contract. 3.4 The selected proposer will participate with the Town in updating its local emergency medical services plan, in accordance with Conn. Gen. Stats. 19a- 181b, et. seq, 3.5 The selected proposer will comply with the disclosure requirements of Section 21 of Public Act , and will agree to promptly disclose any proceedings, past or pending, to revoke its primary service area or designation fails to comply with performance standards by the commissioner of the Department of Public Health. 3.6 The selected proposer will maintain a mutual aid agreement for backup EMS service.

3 SECTION 4: HOUSING 4.1 The Town will provide a smoke-free and alcohol-free facility located at 20 Soule Street, Jewett City, Connecticut to headquarter the selected proposer s operations, and to house the vehicles and equipment necessary for the provision of ambulance service. The selected proposer shall utilize this facility to house vehicles and equipment. The selected proposer shall be required to pay for utilities, maintenance and non-capital repairs of the facility. SECTION 5: INSURANCE AND SECURITY REQUIREMENTS 5.1 The selected proposer shall, at its own expense, maintain insurance to meet the following standards. 5.2 GENERAL LIABILITY: $1,000, per occurrence $2,000, General Aggregate 5.3 PROFESSIONAL LIABILITY: $1,000, per occurrence $1,000, General Aggregate 5.4 CONTRACTUAL LIABILITY: $1,000, AUTOMOBILE LIABILITY: To cover owned, hired and non-owned automobiles $1,000, liability; $1,000, personal injury protection 5.6 UMBRELLA LIABILITY: $1,000, above the foregoing limits. 5.7 WORKERS COMPENSATION: Statutory Coverage. 5.8 HEALTH INSURANCE: Statutory Coverage. *All limits shown are combined single limits. SECTION 6: DURATION OF AGREEMENT 6.1 The selected proposer will be required to execute an agreement with the Town which shall have an initial term of three (3) years. Renewals shall be for no more than two (2) years at the option of the Town and the selected proposer. This agreement will be reviewed, to ensure compliance with requirements, no later than the first business day of July of each year.

4 SECTION 7: SUBMISSION PACKAGE 7.1 Provide a brief history of your firm. Provide a description of your firm, including its emergency dispatch center, its ambulances, and the number of its employees. Identify the number of fully staffed and equipped ambulances which will be used to respond to Town s calls and identify where these ambulances will be based in the event that your firm is awarded the contract. Specify the first date on which your firm can begin providing ambulance services to the Town. 7.2 Describe how your firm will assist the Town in updating its local emergency medical services plan, in accordance with Conn. Gen. Stats. 19a-181b, et. seq, and the role that it would play in such a plan update. 7.3 Describe how your firm will comply with the disclosure requirements of Section 21 of Public Act , the procedures to be employed to achieve such compliance; and how your firm will avoid proceedings to revoke its primary service area or receiving a designation fails to comply with performance standards by the commissioner of the Department of Public Health. 7.4 Provide any additional information which would serve to distinguish your firm s proposal from other proposals. 7.5 In accordance with section 22 of Public Act (a)(1), attached, provide information about how your firm s proposal will meet the standards outlined in Griswold s local emergency services plan, under C.G.S. 19a-181b. 7.6 The Town may make such inquiries it deems necessary to determine the ability of each proposer to perform the services contemplated by this RFP. Proposers shall promptly furnish all information and data for this purpose as may be subsequently requested by the Town. SECTION 8: FEE PROPOSAL 8.1 Provide the proposed fee arrangement in detail providing for all services as outlined in section 3 (Scope of Services) above. If the proposal includes subcontracting with an other EMS provider for service, including ALS and paramedic services, disclose the name, location, capabilities, and other relevant information about the subcontractor, and provide a detailed estimate of that cost and service. 8.2 Provide estimates for the following two (2) scenarios: a. Your firm houses two (2) fully staffed and equipped ambulances at the 20 Soule Street, Jewett City facility. b. Your firm houses one (1) fully staffed and equipped ambulance at the 20 Soule Street, Jewett City facility, and arranges for an additional one (1) ambulance to be available as needed. Provide details about the second

5 ambulance, including its location, and estimated travel time to representative locations within the Town. SECTION 9: REFERENCES 9.1 Identify similar municipalities where the contractor has undertaken to perform similar services in the past ten (10) years and list at least three (3) municipal references. SECTION 10: RECEIPT OF PROPOSALS 10.1 Ten (10) copies of the proposal shall be submitted to the Town no later than 11:00 AM, on Monday, November 17, 2014 at the following address: Office of the First Selectman Town of Griswold 28 Main Street Jewett City, CT Packages should be marked GRISWOLD AMBULANCE SERVICES PROPOSAL Those submitting proposals do so entirely at their own expense The Town reserves the right to accept or reject any and all proposals in whole or in part, to waive any and all informalities, and to disregard all non-conforming, non-responsive, or conditional proposals The Town reserves the right to award the ambulance services work, in whole or in part, to one or more firms and individuals Any award of ambulance services work shall be conditioned on the later execution of a formal written contract and approval by the Commissioner of DPH. The Town reserves the right to revoke or rescind any award at any time prior to the full execution of a formal written contract Any questions should be directed to the First Selectman s Office via at firstselectman@griswold-ct.org, and should be received ten (10) days prior to the deadline.
