7 Shopping has never been so popular.. 2 nd June 2015 It is important that the NHS acts together to ensure we achieve the most from our collective bargaining power and work together to reduce these pressures where we can. David Williams Director General, Finance, Commercial and NHS 4 th June 2015 In a speech at the NHS Confederation conference, Jeremy Hunt said the Carter review s findings would shortly be announced and they are absolutely staggering. The review by Lord Carter has investigated procurement practices across trusts. In September Lord Carter will set out the amount of money trusts could save by adopting best practice procurement processes. Jeremy Hunt said the hard bit starts from January, when trusts will be expected to make specific savings

18 Why do we need to make changes? The overspend in the NHS has increased from 100m in 2013/14 to nearly 800m in 2014/15*. The funding gap is expected to reach 2bn in the next 5 years. The NHS spends 4.5bn per year on clinical products in the acute sector alone All trusts are under pressure to make savings, yet we must work together to protect patient safety and the frontline as a priority. Every 30,000 saved equates to one Band 5 nurse If we can buy products more efficiently the NHS, the trust and the patients will benefit There are over 58m savings available for the NHS just on the products which nurses purchase In total trusts could save over 100m in 2015 by working with us on the Compare and Save and Core List programmes (standardisation of clinical consumables and use of the best value clinically viable product) NHS Supply Chain have delivered over 75m of savings for the NHS so far, with a further 75m needed within the next 12 months working with the support of the Customer Board and the NHS we will achieve this together. 18

24 Nurses, working alongside their trust procurement professionals are the right people for the job It makes perfect sense for nurses to buy in the products that help us deliver safe patient care that is cost effective. We only hear about costs when we are over spent. The nurses position on the frontline of patient care, using a vast range of clinical products on a daily basis, leaves them uniquely qualified to offer detailed insights on what items do and do not work. Raise awareness that saving money is not all about cutting quality.

26 NHS Core List 6.51m of savings available for the NHS on these products alone. with the list of products set to grow The NHS Core List aims to achieve best value by reducing the range of like-for-like products that the NHS purchases. This allows the NHS to commit to larger volumes of a smaller range of products which in turn allows suppliers to provide improved prices to the NHS as greater economies of scale in manufacturing can be achieved Products which are available through the Core List are; Exam gloves Dry wipes Underpads Stationery Medicare exam gloves case study 99.9% of staff across the trust are using it as a non sterile glove. You would expect to have some problems reported with any change like this we ve had just one, which has not required any support from Occupational Health. 41 What is a Clinical Procurement Specialist? A Registered Nurse or similar other clinical professional who works either within the procurement structure of their organization, or assist it, in the procurement of clinical supplies or eqpt.

45 My Role as a Procurement Nurse A conduit/interface between our clinicians and procurement & commercial activity (supplier representatives). Promoting a culture and environment whereby clinicians can approach the procurement team and have innovative ideas addressed quickly. A robust product trial, evaluation and implementation process. 47 Procurement Trial Process A product trial should be a structured systematic process, to reduce risk to patients, staff, and the Trust. The Hospital has a policy of product standardisation within the Trust. This reduces the margins of errors through unfamiliarity with products, promotes and supports Clinical Governance, Clinical Benchmarking, Care Formularies and the opportunity for Procurement to obtain Best Value for Money. Trials should be coordinated from a single source. This is via the trust s Clinical Procurement Specialist Nurse initially. It is not possible, nor necessarily beneficial, to trial ALL products. The Trust therefore developed a Justification for Trial form/procedure.

48 Completion of the request to trial a product form enables the procurement team to: Anticipate usage within the Trust and therefore overall cost. Decide if the trial needs to be part of a Procurement competitive exercise as dictated by the Trust's Standing Financial Instructions (SFI's). Identify the area(s) that will trial the product. Identify the Trial Lead Person(s). Decide upon the period of the trial. Consider the involvement of other specialities (e.g.: Infection Control, Tissue Viability, EBME, Manual Handling, Finance, Laser Safety Officer). Agree the evaluation and feedback process (evaluation forms from the Clinical Procurement Specialist Nurse).

49 Standardising and Reviewing Practice and Products Working with the relevant Clinical Leads to: - Standardize products throughout the trust wherever possible. - Justifying reasons for variation (if applicable). Identify training needs and advise upon appropriate product use - Informed interpretation of usage and spend reports to identify inappropriate usage or inappropriate products! - Coordinate supplier audits to evaluate correct product usage.

50 Ensuring a Clinically Focussed Workplan Quality Agreeing quality standards and performance for products in use. Advise, question and challenge and support product selection if required. Utilising clinical networks and market place awareness. Measuring benefits of procurement programmes in both clinical and cost benefits. Safety Reducing variation and standardising practice across Trust. Identifying & promoting safer ways of working (e.g. Safer Sharps). Developing training and implementation plans with clinical teams and suppliers. that ensure staff are competent with new and existing products. Value Managing the supplier market & sales activity. Benchmarking pricing in the NHS. Utilise Procurement Team expertise to ensure Best Value for Money. Working with NHS Supply Chain liaison with their Clinical Advisors.

The survey of 850 nurses found that over 80% of respondents thought there was scope to save money in their trust, and a further 86% believed patient safety would be improved if nurses had greater involvement in purchasing Those behind the idea have estimated that nurses working closely together with procurement managers could save more than 30m the equivalent of 1,000 nursing jobs by streamlining the buying of basics, such as wipes and incontinence products. However, the findings also revealed barriers to nurses getting involved in purchasing, with a quarter saying they were currently not allowed to and a further 28% that they did not have enough time. There has never been a better time to look at what nurses are buying and find those opportunities to save money
