1 Online Associate of Arts and Science Degree Worksheet Use this worksheet as an advising tool to keep track of your progress through the associates degree program. Have this document available when talking with your advisor. If you transferred courses to UW Colleges, your advisor will add them to this form and send you a new copy upon completion of the application process. TO USE THIS FORM: Use your tab key to move through the gray boxes and fill in the information requested or if an arrow appears when you tab into the box click on the arrow and make your selection. A minimum of 60 credits is required. 2. The UW Colleges General Education Requirements must be fulfilled. 3. At least 24 of the 60 credits must be completed within the UW Colleges, or at least 12 of the last 24 credits, must be earned within the UW Colleges. 4. A student must have a cumulative grade point average (GPA) of 2.0 earned at the UW Colleges and a 2.0 GPA in credits applied to the Associate of Arts and Science Degree. 5. Students must be enrolled at a UW College during the last semester in which the degree requirements are completed or have earned 60 degree credits prior to transferring from a UW College to a baccalaureate degree granting institution.
