2 Actions Person(s) Responsible Indication of Success Resources Due Date Ct Adult Reporting System June 2015 Program attendance records monitored monthly. -Prior to the monthly attendance data entry into the CT Adult Reporting System, the respective program facilitator will target students who have attended less than fifty percent of the time. -Individual sessions between the school counselor and students in the diploma completion options will occur; building a commitment to secondary school completion. -Students in English Second Language or Adult Basic Education with missed successive classes will receive and snail mail messages encouraging their return. - Student absence and consistent tardiness with no prior notification receive immediate contact. School Counselor ESL students mean usage of available instructional hours will increase to 54% or greater in Percentage of ESL students achieving a significant reading gain will increase to 48%. Adult Basic Education/GED preparation students mean usage of available instructional hours will increase to 61% or greater in District s messaging systems Develop a student success plan for each student in the diploma completion options. Train Credit diploma staff in preparation of course syllabi with fidelity to College and Career Readiness Standards (CCRS). School Counselors Each student in a diploma completion option will have career profile and follow it. Course syllabi with explicit representation of CCRS.
