2 Blue Cross of Idaho provides a wide range of web-based tools you can use to learn more about your health insurance, healthcare and improve your health. This brochure highlights many of the tools on our website and indicates if you must log in to access the tools. When you see this symbol you do not have to log in to access the information. When you see this symbol you do have to log in to access the information. To access the Blue Cross of Idaho website, go to members.bcidaho.com. Find a Provider Tool It s easy to find Blue Cross of Idaho contracting providers with this tool. From the member page, simply select the Find a Provider from under the Search Tools menu. From the Provider Directory page, select the Provider Type you are looking for and your healthcare plan under the product/network type dropdown. You can further narrow your search by name, specialty, gender, city, zip code or county, but do have to fill in these fields. When you are finished making selections and entering criteria, select Search. C 1

3 Depending on the Provider Type, the results will include the information below and may list more information: Name Address Telephone number Specialty Networks for which he/she is contracted When you select the following Provider Type, we transfer you to another website to complete your search: Hospital/Medical outside of Idaho Hospital/Medical outside of the U.S. Pharmacy Vision (you must first register and log into this site to find a vision provider) Claims, Eligibility and Benefits You can view your claims history or print out an Explanation of Benefits (EOB) and more on our website. Because your claims history and benefit information is specific to you, you must log in to the secure member portal to access it. Log in to your member home page. From here, you will have access to: Member Information Coverage Information Other Health Insurance Eligibility Deductibles & Maximums Claims & EOBs Benefits Hospital Admissions Prior Authorizations Pharmacy Benefits Similar to the medical and/or dental claims lookup you have through Blue Cross of Idaho, you can look up prescription drug claims. To access this tool, select Pharmacy Information from under the prescription drugs menu, then Access Your Pharmacy Benefits Now. We will transfer you to a secure web portal hosted by CVS Caremark, Blue Cross of Idaho s pharmacy benefits manager. Each website will have specific instructions on how to find the information you re looking for. 2 3

4 From this site you can: View your prescription claims history Fill new or refill existing mail order prescriptions View prescription drug costs Find a pharmacy Find out if a drug is covered under your benefits Find out how to save money on the drugs you re taking Forms You can find forms in a variety of places on our website, but we maintain most forms associated with claims in the member portal. Wellness Support Tools (powered by Wellness & Prevention, Inc.) Blue Cross of Idaho provides you a suite of online wellness support tools. These tools will assist you in assessing your health and give you the knowledge and encouragement to improve your health and overall quality of life. Wellness support tools are available to you via the member or guest portal once you log in by supplying a username and password. From the member home page select the WellConnected Tools under the Health and Wellness menu option. Once you ve logged into the member portal, select Resources and Forms, then Applications and Forms to access the following printable forms: Accidental Injury Questionnaire (electronic version available on the member page prior to log in) Coordination of Benefits Medical Claim reimbursement (print version available on the member page prior to log in) Prescription Drug reimbursement VSP (vision) out-of-network reimbursement You will also find other common forms in this section. You will transfer to a secure site hosted by Wellness & Prevention, Inc. You must complete a short registration process the first time you enter the site. You will then have access to the personal health assessment. Some data is passed securely to Wellness & Prevention, Inc., such as your name, address, age and gender, but the tool requires more information to give an accurate health appraisal. The personal health assessment includes questions that will take about 20 minutes to finish. To answer the questions, you may need information about following health measures: Blood pressure Total cholesterol HDL (good) cholesterol LDL (bad) cholesterol 4 5

5 Triglycerides Fasting glucose level You must also enter your height, weight and waist measurement so the tool can calculate your body mass index. If you do not complete the assessment, you can save your work and go back to complete it later. After you complete the assessment, the tool will provide you a report rating your overall health on a scale of 1 to 100. The report compares your results to what is considered desired and recommends ways you can improve your health. You also have access to a variety of interactive tools, videos (most of which can be downloaded to an ipod or Blackberry), recipes (also downloadable), newsletters and a complete medical library. Once you submit your assessment, you may not modify your answers. The tool automatically triggers a retake date approximately six months from the most recent assessment date and saves the two most recent assessments for your review and tracking purposes. When you complete your personal health assessment, you have access to several online health coaching programs (called digital health coaching) designed to help you make lifestyle changes that will improve your health. These include: RELAX stress management MOVE fitness/physical activity NOURISH nutrition BREATHE smoking cessation BALANCE weight management Health Summary Thanks for sharing so much about yourself. But instead of throwing it all at you, we've given you a personalized recommendation of where to start. Earlier we asked about your motivation and confidence for these behaviors. Why? Because knowing what you want to change and what you think you can change helped us figure out how to help you best. We think you should start with the Top Priorities and work down the list to make a lasting change. But start with what makes sense to you. This is your plan, and your health. Top Priorities Depressive Symptoms Weight Management Physical Activity Lower Priorities Getting on Your Feet Starting to Roll Hitting Your Stride Staying Strong You are here: Hitting Your Stride Karri, you told us you're getting the exercise you need. Good guess! Right now, you're "hitting your stride." You do the equivalent of 175 minutes of moderate exercise (every minute of vigorous activity counts for two minutes of moderate activity) a week. Adding two days of strength training to your weekly routine is all it'll take for you to "stay strong." Move could help you: Improve your sense of well-being Manage stress Maintain your weight Tone your body You're not alone A lot of women in their 40's lose sight of their own needs. Maybe it's because you're putting in a lot of time in the office and you're busy being a mom. Whatever the reason, many women don't get the physical activity they need. Sound familiar? But believe it or not, a lifestyle like yours doesn't have to crowd out exercise, or vice versa

6 Wellness & Prevention, Inc. designed the digital health coaching tools to mimic a personal health coaching session but digitized. Each program begins with an in-depth consultation, asking questions about your personal situation, motivation to deal with health issues, selfconfidence and perceived barriers to success. From this, the tool builds a personalized plan by combining: 1. What the coach has learned about you 2. All the clinical training and experience the coach has on this topic 3. What behavior change models are most likely to get you to change The tool provides you a personalized action plan, tools and resources to help you meet your goals, and follows up over time to see if the plan has achieved the desired results. Health Library (powered by Wellness & Prevention, Inc.) The Blue Cross of Idaho website gives you access to medical self-help information from Healthwise, an innovator and industry leader in health information for more than 35 years. The tools housed in the Health Library will help you understand your current health condition, give you information about when and where you need care and give you the knowledge to make better decisions, improving your overall quality of life. Medical self-help tools are available to you via the member or guest portal once you log in by supplying a username and password. From the member/guest home page select Health Library under the Health and Wellness menu, then Access the Health Library now. A new website will open. Once you select the icon, we transfer you to the Healthwise site developed for Blue Cross of Idaho. On the site, you have access to: Interactive tools and quizzes Symptom checker Health topics Learning centers Decision support tools Information on medical tests and medications Healthwise designed the site for use by the nonmedical layperson, written in easy to understand language and complete with illustrations. According to Healthwise, The best formula for making health decisions is to combine the most reliable medical facts with your personal feelings. Put more simply: Medical Information + Your Information = Better Health Decisions. Your decisions influence your overall well-being as well as the quality and cost of your care. By taking an active role in your health, you are likely to be happier with the care you receive and the results you achieve. 8 9
