1 HealthFund Basics What is a HealthFund? Funds from Cedars-Sinai, deposited into an account (your HealthFund) that you can use to pay for eligible healthcare expenses that are not paid by your Cedars-Sinai healthcare insurance. How much is the annual HealthFund contribution? The full annual amount is shown in the table below. First, you must be enrolled in a Cedars-Sinai medical plan the Blue Cross HMO, Blue Cross PPO or Vivity HMO. Then, you must complete the following 2 healthy actions by the deadline on your personalized enrollment checklist. 1. Get a health screening From your doctor or From LabCorp Visit HealthAdvocate.com/Cedars-Sinai to download a LabCorp voucher or form for your doctor to complete and submit. * *Measurements taken by your doctor or LabCorp or by Health Advocate (the onsite screenings) on or after April 1, 2015 count toward the HealthFund. 2. 2 I can t remember whether I got a HealthFund contribution this year how can I find out? To find out if you received a HealthFund contribution, call the MBC Benefits Help Desk at To find out if you ve already taken one or both of the healthy actions (and the date), call Health Advocate at Health Screenings Where can I get a health screening? From your doctor Go to HealthAdvocate.com/Cedars-Sinai and print the Physician Health Screening Form for your doctor to complete. Measurements taken by your doctor on or after April 1, 2015 count toward the second chance HealthFund. From LabCorp Go to at HealthAdvocate.com/Cedars-Sinai to find a nearby lab and print the Lab Voucher (invitation code: CedarsHealth2015), which you must take to your appointment. Your health screening will be logged automatically into the HealthFund system. What if I need help scheduling an appointment or using the Health Advocate website? For assistance contact Health Advocate: Phone: Answers@HealthAdvocate.com How long will the health screening take? An onsite health screening from Health Advocate takes about minutes. Please allow enough time for travel and wait time. What measurements are included in the health screening? The required measurements include cholesterol, body composition, blood sugar and blood pressure. If I go to LabCorp, do I have to pay? Not as long as you take the Health Advocate Lab Voucher to your appointment for the technician to complete. To print a voucher, go to the HealthAdvocate.com/Cedars-Sinai home page and click: WELLNESS VIEW OPTIONS (next to Complete My Health Screenings) VIEW SITE (under Request Lab Voucher); here you will create a BioIQ account (use invitation code: CedarsHealth2015) HealthFund FAQs 1/29/2016 Page 2

3 If I go to my doctor, will the screening be covered by my medical plan? You will need to ask your doctor this question. To get a health screening at no cost, go to LabCorp (and be sure to take the Lab Voucher from HealthAdvocate.com/Cedars-Sinai with you). How do I find out the results of my health screening? If you want your results, you must register for a personal account at HealthAdvocate.com/Cedars-Sinai. You will receive an when your results are available to view. If you do not have a computer, call Health Advocate at and a wellness coach will be available to review the results with you (immediately or you can set up an appointment for a later time). Personal Health Profile (PHP) Do I have to do the personalized health profile (PHP) on the computer? There are several good reasons to do your PHP on the computer: It s quicker, the system will automatically log that you completed it, and you ll be able to access your results online in the future. Go to HealthAdvocate.com/Cedars-Sinai to complete the PHP online. If you do not have access to a computer, paper PHPs in both English and Spanish will be available at the Cedars-Sinai onsite health screenings. Where do I return the paper PHP? The PHP paper kits have a postage-paid, return envelope included all you need to do is drop it in the mail. How long will it take? The PHP takes about minutes to complete. Is the Benefits Portal available in Spanish? No. However, Spanish speaking representatives are available to help you by phone. Call for assistance. Is the Health Advocate website in Spanish? No. However, the PHP is available in Spanish, and Spanish speaking representatives are available to help you by phone. Call for assistance. HealthFund FAQs 1/29/2016 Page 3

4 Spending Your HealthFund Where is my HealthFund money? It s deposited into an account with ConnectYourCare, Cedars-Sinai s HealthFund administrator. You may access your account by: Computer at ConnectYourCare.com Mobile phone or tablet app (search for ConnectYourCare or CYC mobile at your mobile device store) Phone ConnectYourCare at Your first visit to the website or using the app, choose NEW USER REGISTRATION and follow the prompts. How do I access my HealthFund? When you pay for an eligible healthcare expense (that s not paid for by your medical plan), you can: Pay with your healthcare payment card Pay with your own money and submit the receipt to ConnectYourCare for reimbursement from your HealthFund What types of eligible healthcare expenses may I pay for with HealthFund money? In addition to co-payments and co-insurance for medical, dental and vision care, account funds can be used to pay for things like (this is only a partial list): Blood pressure monitoring devices Chiropractic care Contacts Crutches Dental treatment Dentures Drug addiction treatment Eye examination Eye glasses and related Fertility treatment Flu shots Hearing aids Insulin Laser eye surgery Orthodontia (not for cosmetic reasons) Physical therapy Prescription drugs Psychiatric care Smoking cessation program Transportation for medical care Go to the ConnectYourCare website for a longer list of eligible healthcare expenses: ConnectYourCare.com/tools/eligible-expenses (Select HRA for HealthFund expenses). 5 Can I pay for my spouse and children s eligible healthcare expenses with my HealthFund, even though they are not covered under my Cedars-Sinai medical plan? Yes! You can use these funds to pay for eligible healthcare expenses for your spouse (same or opposite sex), children to age 27 or anyone you can claim as a dependent on your tax returns. They do not have to be covered (or even eligible to be covered) in a Cedars-Sinai medical plan. Healthcare Payment Cards How do I get a Healthcare payment card? A card will be mailed to you the first time you receive a HealthFund contribution. Usually it takes a few weeks after your HealthFund start date for the cards to be issued and arrive in the mail. If you received a HealthFund contribution (or enrolled in the Health Care Reimbursement Account) your contribution will be loaded on to the card you already have. I lost my card. How do I get a new one? Request a new card by logging on to your online account at ConnectYourCare.com or calling ConnectYourCare at Do I have to have a computer or smartphone to use my healthcare payment card? No, you may use your card without registering for an account with ConnectYourCare. How do I know how much is in my HealthFund Account if I don t have a computer or smartphone? You can call the number on your healthcare payment card to find out your account balance. ConnectYourCare also mails quarterly account statements to your home address. Where can I use my healthcare payment card? Your healthcare payment card can be used nationwide at pharmacies, doctors offices, vision center facilities and approved merchants (such as grocery store pharmacies). Visit ConnectYourCare.com/tools/merchants/ to find merchants accepting the healthcare payment card. Your card should only be used to pay for medical expenses eligible under your plan, and you should always save your receipts. Note this is an opt-in system; not all healthcare providers accept healthcare payment cards. HealthFund FAQs 1/29/2016 Page 5

6 When will my card be activated? Unless otherwise indicated, your card will be automatically activated the first time you use it and will remain active until the expiration date shown on your card (or if you leave Cedars-Sinai). Be sure to keep your card (even after your funds are depleted) for future plan years! You ll need to pay the co-payment or expense with your own credit card or money, and then submit a claim for reimbursement to ConnectYourCare. When I use the healthcare payment card, do I need to keep the receipts? YES! Always keep your receipts. The IRS requires ConnectYourCare to verify that every expense is eligible. Although some expenses can be automatically verified, some require documentation showing the expense is eligible for payment from your HealthFund. See How do I submit documentation? on the next page. Submitting Receipts and Documentation I thought if I used the healthcare payment card I wouldn t have to submit documentation. Not so. Because income taxes are waived on HealthFund contributions, the IRS requires ConnectYourCare to verify that every expense is eligible. Even when you use your healthcare payment card, you may need to submit receipts. The primary benefit of the healthcare payment card is that it gives you immediate access to the money in your account. Sometimes expenses can be automatically verified. Other times you will need to submit proof that you used the card for an eligible healthcare expense. You probably won t know if the expense can be automatically verified, so always save your itemized receipts in case they are requested. How do I know if I need to submit a receipt? There are several ways to find out: ConnectYourCare will send you 3 notices: First, an reminder to your Cedars-Sinai account (or personal account, if you gave that address to CYC to use instead). If you don t respond to the , they ll send a reminder by mail to your home address. Last, if you still don t respond, ConnectYourCare will send you a healthcare payment card suspension notice by mail. Documentation needed alerts are always posted on your online account at ConnectYourCare.com and in the mobile app. The mobile app makes it quick and easy to snap a picture of your receipt using your phone s camera and upload it directly to your account. Sign up for CYC mobile text alerts. That way you ll get a text message immediately after using your healthcare payment card if a purchase requires documentation. Texts are a good way to know whether you need to save itemized receipts. Sign up by logging in to your ConnectYourCare.com account, and then clicking on the MOBILE ALERTS link in your online account. 7 What if I don t have a Cedars-Sinai account? You do! You just might not know it. All employees have individual Cedars-Sinai accounts. However, if you have not logged into your account, after 6 months your account is frozen. Contact the Cedars-Sinai EIS Help Desk to re-activate your account at If you prefer, you may have ConnectYourCare send notices to your personal address instead of your Cedars-Sinai address. Sign in to ConnectYourCare.com with your username and password and add your personal address in the MY PROFILE section. What types of documentation will ConnectYourCare accept? ConnectYourCare can accept: Itemized receipts Explanation of benefits (EOBs) from an insurance company Any documentation that has all of the following: Merchant or provider name and address Name of person receiving the service or purchased the item Item purchased or service received Date of purchase or service Amount of purchase or service For prescription drugs you must also include the prescription number (for medications) To be reimbursed for most over-the-counter medications, you must also include a copy of the prescription from your doctor ConnectYourCare cannot accept credit card receipts. How do I submit documentation? Submit receipts online at ConnectYourCare.com: Scan your receipt to make an electronic copy Sign in to ConnectYourCare.com with your username and password Click I want to Reimburse Myself Complete the online form Upload your receipt Submit receipts using the ConnectYourCare smartphone app: Open the app and select MAKE A PAYMENT/ADD A CLAIM Fill in the online form Take a photo of your receipt and upload Submit receipts by fax or mail: Collect your itemized receipts and documentation HealthFund FAQs 1/29/2016 Page 7

8 Complete a ConnectYourCare claim form. To get the form, call ConnectYourCare at or MBC HR Benefits Help Desk at Fax the claim form and documentation to ConnectYourCare at or Mail your form and documentation to: Claims Department Connect Your Care P O Box Orlando, FL When you fax the claim form and supporting documentation, there is no need to follow up with a hard copy in the mail. (If you log on to your online account, you can see the scanned images of your documentation shortly afterward.) Remember to keep the original claim form and supporting documents for your records. I used my healthcare payment card and then lost the receipt. Now my card has been frozen and I can t use it for any expenses, even though I still have money in my HealthFund. If you fail to submit required receipts within 45 days of request from ConnectYourCare, your healthcare payment card will be deactivated and payments for future claims may be reduced by the amount you owe until the amount owed is completely paid off. How long do I have to submit documentation before my healthcare payment account is suspended? 70 days from the date of service or purchase (or 45 days from the date ConnectYourCare sends the first reminder). What happens if I don t submit the receipts or repay? If you fail to submit receipts within 70 days from the date of service or purchase (or 45 days from the date ConnectYourCare sends the first reminder), your healthcare payment card will be deactivated and payments for future claims may be reduced by the amount you owe until the amount owed is completely paid off. If funds are still due, the owed amount could be added to your W-2 or withheld from your pay. How do I unfreeze my healthcare payment card? To lift the suspension, you may either: 1. Reimburse your HealthFund account. Make a personal check, money order or certified check equal to the amount of the expense payable to: Repayment Department Send the check along with identifying documentation to ConnectYourCare at Claims Department Connect Your Care HealthFund FAQs 1/29/2016 Page 8

9 P O Box Orlando, FL ConnectYourCare must be able link the payment to you and your account, so be sure to include some type of identifying documentation, such as the number on your healthcare payment card, the claim number or a copy of the Request for Documentation letter from ConnectYourCare. 2. Submit substitute documentation. ConnectYourCare must have receipts or documentation for eligible healthcare expenses equal to the amount you spent using your payment card. If you don t have the receipts from when you used your card, you may submit receipts or documentation for different healthcare expenses. Ask ConnectYourCare to reduce the amount you owe by the amount of the substitute documentation. For example, if you used your healthcare payment card for a $50 expense, but could not provide documentation for the expense, you owe $50 of documentation to the HealthFund. If you then have two doctors office visits, each with a $25 copayment, you could pay those copayments with your own funds, and then submit the receipts to CYC to cover the $50 you spent using your healthcare payment card. Once ConnectYourCare has documentation equal to the amount you spent using your healthcare payment card, your card will be unfrozen. How will I receive my reimbursements? Usually by check, but for quicker reimbursements, sign up for direct deposit with ConnectYourCare. How do I set up direct deposit? Log into your online account and select DIRECT DEPOSIT under My Profile. Complete the short, secure form. Be sure to have your bank account and routing numbers on hand. Choose DIRECT DEPOSIT as your preferred method of Claim Reimbursement and click the CONFIRM button. What if I have a medical appointment before my card arrives? You ll need to pay the co-payment or expense with your own credit card or money, and then submit a claim for reimbursement to ConnectYourCare. HealthFund FAQs 1/29/2016 Page 9

10 When HealthFund Participation Starts and Ends Can I use my HealthFund to pay medical bills for an appointment I had before my HealthFund start date? No. If your start date is July 1, only services received or items purchased on or after July 1 can be paid with HealthFund money. If I had a HealthFund last year ( ) and I receive another HealthFund contribution for this year ( ), can I use this year s HealthFund contribution to pay for a medical service I received in June 2015? Yes. Your HealthFund start date was July 1, You may use the HealthFund to pay for any eligible healthcare service received or purchased after your HealthFund start date (as long as you are still employed by Cedars-Sinai). Is there a deadline for spending the HealthFund money? There is no deadline for spending the funds in your account as long as you are employed at Cedars-Sinai. You can roll over your account balance from year to year. However, if your account reaches $1,000, Cedars-Sinai will stop contributing until you spend some of your funds. I had a HealthFund last year ( ) and then switched to a Cedars-Sinai job without benefits. Even though I m no longer eligible for the HealthFund, can I still pay for healthcare expenses with the money in my HealthFund? Yes. There is no deadline for spending the funds in your account as long as you are employed at Cedars-Sinai. What happens to my HealthFund if I leave Cedars-Sinai? Shortly after your employment ends, your healthcare payment card will be de-activated. You have until the next Sept. 28 to submit claims for healthcare services you received from your HealthFund start date through your last day as a Cedars-Sinai employee.
