1 Requirements to register for the Massage Therapy Program: 1. (a) Must be at least 18 years old (b) Proof of high school diploma, GED or college degree (c) Take Test of Adult Basic Education (TABE) test - in accordance with Rule 6A FAC the minimum basic skills grade level required for this post secondary adult vocational (PSAV) program is: - reading A level - language A level - math A or D level 2. Complete the entire school admission application and the following documents: - criminal history & background questionnaire: - applicant s personal history may affect his/her ability to receive state license - typed written answers of entrance essay questions. - financial aid requirements. 3. Attend a personal interview process with the selection committee prior to being accepted to the program. The applicant will receive more information about the program and have an opportunity to ask questions at this time. (interview dates are pending) 4. All Massage Therapy Program applicants will be notified by letter of acceptance or nonacceptance into the course a minimum of 30 days prior to the start date. - Please DO NOT call and ask if the letters have been sent yet or to inquire if you were or why you were NOT selected for the program. - The exact dates for beginning and ending of the program will be included in your acceptance letter. Additional Information: (a) The Massage Therapy Program generally starts in August of each year and will complete in April of that same school year. (subject to change) - Classroom hours are 07:30am to 02:00pm Monday Thursday - The Massage Therapy Program is 750 total clock hours - Student are required to maintain minimum 77% passing grade on all subjects (b) Massage Therapy students are allowed to be absent for only 24 classroom hours during their entire course of study. A minimum of 726 hours are required on the official transcript for graduation from the massage therapy program. - There are no excused absences from the hourly count.

2 (c) This course is a Florida state licensure program of study in Massage Therapy and operates under the rules and regulations of the state of Florida Department of Education, Department of Health and Board of Massage Therapy. (d) A maximum of 25 students will be selected to begin the Massage Therapy Program each year. The Massage Therapy Program is designed to prepare students for employment as a Florida state licensed massage therapist (LMT). After completion of the program, graduates are eligible to take an approved National Examination for Florida state licensure. The content includes, but is not limited to, theory and practice of massage therapy, human anatomy and physiology, kinesiology, theory and practice of hydrotherapy, Florida laws and rules, hygiene, practical demonstrations, leadership and human relations skills, health and safety, CPR and employability skills. Colonic Irrigation is optional as an LMT but will NOT be taught at this school. Reinforcement of basic skills in English, Mathematics, Science and appropriate research and computer technologies for job preparatory occurs through vocational classroom instruction and applied laboratory procedures or practice. 4 Frequently Asked Questions Q-1 = How do I apply to the Massage Therapy Program? A = 1. submit completed application packet and other documents (criminal history, essay questionnaire, etc.) 2. take TABE test 3. complete personal interview 4. financial aid processing Q-2 = How will students be selected? A= All applicants will be evaluated equally based on completed application packet and the information contained within, the interview, essay responses and criminal history. BCSB Policy Q-3 = When and how will I be notified if I have been selected for the Program? A = A letter of acceptance or non-acceptance will be sent to each applicant at least 30 days prior to the beginning of the next class. A reason will NOT be included if you were not selected for the program. Q-4 = What is the deadline for me to apply to the Massage Therapy Program? A = There is only one day time class per school year. The completed application and accompanying documentation must be delivered to the Student Services office no later than last day of May. (subject to change) Check with Student Services for more information. Q-5 = Can I apply if I do not live in Bay County? A = Yes, you can apply Q-6 = I am very anxious to find out about my application. Can I call the instructor or Student Services to find out if I have been selected? Can they me? A = No. This information will be sent by postal mail. It will NOT be released by phone or Q-7 = If I am not selected this year, can I reapply in subsequent years? A = Yes, you may reapply each year Q-8 = I was picked as an alternate for the class, but was not chosen. Does this mean that I will automatically be selected the following year? A = No, anyone not selected for admission, as an alternate or otherwise must still reapply for subsequent years.

5 Frequently Asked Questions (cont) Q- 9 = If I am selected but drop out of the Massage Therapy program during the school year, will I be able to re-enter and complete the class with the same group of students? A = No. Once you have missed 24 hours of classroom time you will have to reapply for the class in the next school year. Q-10 = I took the Test of Adult Basic Education (TABE) but did not meet the minimum requirements of the program. Can I retake the TABE to try and get a higher score? A = Yes. However, if you are not enrolled in remediation classes, you must wait a minimum of six months before you are eligible for retaking the TABE. - in accordance with Rule 6A FAC the minimum basic skills grade level required for this post secondary adult vocational (PSAV) program is: - reading A level - language A level - math A or D level - it is preferred to have this score prior to entry into the program. Applicants who do not meet this required score may enroll in remediation classes to assist them in meeting these requirements. Q-11 = Once I graduate from the Massage Therapy Program is there anything else I am required to do to receive my Florida state license? A = Yes. You must apply to either one of the National Examination organizations (NCBTMB or FSMTB) to take an approved exam. Also, you must apply separately to the state of Florida for licensure. An additional fee (over and above the school tuition) is paid to each of the individual organizations for testing or licensing. Q-12 = Will I be able to practice massage therapy in any other state with my Florida massage therapy license? A = No. Each state in the U.S. has different requirements for massage therapy licensure. Contact the city and / or state to which you are moving to find out their specific requirements. Q-13 = Do I ever have to renew my massage therapy license once I obtain it from the state of Florida? A = Yes. You will renew your massage therapy license in August of every odd numbered year. You will be required to obtain 24 hours of continuing education units (CEU)

6 State of Florida Criminal History Questionnaire The following questions will be asked by the state of Florida on the application for massage therapy licensure. Please read through these questions and determine if they apply to you in any way: CRIMINAL HISTORY (Review Questions & Answers section in instructions) A. Have you EVER been convicted of, or entered a plea of guilty, nolo contendere, or no contest to, a crime in any jurisdiction other than a minor traffic offense? You must include all misdemeanors and felonies, even if adjudication was withheld. Driving under the influence (DUI) or driving while impaired (DWI) is not a minor traffic offense for purposes of this question. B. Have charges ever been brought against you by any branch of the United States Armed Services? Pursuant to Section (2), Florida Statutes, the following questions are being asked. If you answer Yes to any of the following questions, explain on a separate sheet providing accurate details and submit copies of supporting documentation. 1. a. Have you been convicted of, or entered a plea of guilty or nolo contendere, regardless of adjudication, to a felony under Chapter 409, Chapter 817, or Chapter 893, Florida Statutes; or 21 U.S.C. ss or 42 U.S.C. ss ? (If No, do not answer 1b.) b. Has it been more than 15 years prior to the date of this application since the sentence and completion of any subsequent period of probation for such conviction? 2. a. Have you ever been terminated for cause from the Florida Medicaid Program pursuant to Section , Florida Statutes? (If No, do not answer 2b.) b. If you have been terminated but reinstated, have you been in good standing with the Florida Medicaid Program for the most recent five years? 3. a. Have you ever been terminated for cause, pursuant to the appeals procedures established by the state or federal government, from any other state Medicaid program or the federal Medicare program? (If No, do not answer 3b and 3c.) b. Have you been in good standing with a state Medicaid program or the federal Medicare program for the most recent five years? c. Did the termination occur at least 20 years prior to the date of this application? If you answered YES, you are required to send a letter in your own words describing in detail the circumstances surrounding each offense; including dates, city and state, charges and final disposition. You must submit documentation from the Clerk of Courts in the jurisdiction (state/county) in which the offense occurred, including disposition/final results. Your application will not be considered complete until these records are received. If the records are no longer available, you must obtain a letter of their unavailability from the county Clerk of the Court.
